As I sit down to compose my president’s message, I’m looking at a calendar and realize that the end of<br />
the year is quickly approaching. It also means that my term serving as president will have reached its<br />
halfway point.<br />

I made the last of my affiliate visits. You really get an appreciati<strong>on</strong> of just how big our comp<strong>on</strong>ent is when<br />
you have to drive down to Collier County <strong>on</strong>e visit, up to New Port Richey and eastward to Lakeland for yet<br />
another. All of the affiliates had their own pers<strong>on</strong>ality, but I was c<strong>on</strong>tinually impressed by the overall quality of<br />
our members. I pers<strong>on</strong>ally want to thank Drs. Churney, Palo, Cullinan and Desai for helping me out when I had<br />
scheduling c<strong>on</strong>flicts. I hope this traditi<strong>on</strong> of visiting the various counties never fades.<br />
During the Officers’ Planning Sessi<strong>on</strong> at the WCDDA’s Summer Meeting in Naples, access-to-care was a topic <strong>on</strong> the agenda. Drs. Dan<br />
Bertoch and Terry Buckenheimer proposed trying to create a pilot program utilizing the talents of those who provide dental services to<br />
the underserved and the members of the WCDDA. The first step of this process occurred <strong>on</strong> October 23, 2009. A forum was held at the<br />
WCDDA’s central office, proctored by Dr. Bertoch, with representatives from several public health settings in attendance. The purpose<br />
of this forum was to identify where the associati<strong>on</strong> could be of assistance in providing dental care to the underserved.<br />
Please register for the WCDDA’s 88 th Annual Meeting Friday, February 5, 2010 at the <strong>St</strong>. <strong>Petersburg</strong> Hilt<strong>on</strong> Bayfr<strong>on</strong>t. The meeting will<br />
feature the internati<strong>on</strong>ally renowned speaker Dr. Gerard Chiche presenting <strong>on</strong> Predictable Anterior Esthetics, Ms. Katherine Eitel <strong>on</strong><br />
Li<strong>on</strong>ess Leadership and Angie <strong>St</strong><strong>on</strong>e, RDH will talk about improving oral care in dependent patients and recall. CPR re-certificati<strong>on</strong><br />
will also be available in time for license renewal. There is something for all, so I hope that you’ll bring your staff. There will also be a<br />
Presidential Cocktail Recepti<strong>on</strong> at the Museum of Fine Arts Thursday evening, February 4 th at seven o’clock. All are welcome to attend.<br />

The Role of Organized Dentistry in Access to<br />
Oral Health Care for Underserved Communities<br />
For too l<strong>on</strong>g, dental disease has been a silent epidemic.<br />
Further, there is a huge disparity that exists in oral health<br />
care am<strong>on</strong>gst lower socio-ec<strong>on</strong>omic populati<strong>on</strong>s. It is<br />
apparent not <strong>on</strong>ly in children, but the elderly, homebound<br />
and handicapped populati<strong>on</strong>s as well.<br />
What we know:<br />
1. In general, disadvantaged groups have 2-3 times the<br />
unmet need for dental care than average.<br />
2. Low socio-ec<strong>on</strong>omic status pers<strong>on</strong>s are 3-4 times less<br />
likely to have an annual dental visit.<br />
3. Only 22 percent of Medicaid–enrolled (low income)<br />
children receive an annual dental visit in Florida.<br />
4. There has been little increase in sealant use for schoolaged<br />
children in low income families.<br />
Barriers for disadvantaged pers<strong>on</strong>s include:<br />
1. Low access to dental care through publicly funded and<br />
volunteer programs.<br />
2. Transportati<strong>on</strong> difficulties.<br />
3. Loss of work time without pay.<br />
4. Low perceived value of dentistry.<br />
5. Lack of understanding of the importance of routine<br />
dental visits for preventi<strong>on</strong> and early detecti<strong>on</strong> of<br />
chr<strong>on</strong>ic disease.<br />
Even though the problems are numerous and complex, they<br />
are not insurmountable. Preventi<strong>on</strong> and Educati<strong>on</strong> have to<br />
be our main focus. Preventi<strong>on</strong>, by advocating community<br />
fluoridati<strong>on</strong>, early school oral health screenings and school<br />
sealant programs is essential. Educati<strong>on</strong>, with regards to<br />
the importance of oral health to overall systemic health, and<br />
early and regular preventative visits is equally important.<br />
It is important to note that 90 percent of practicing dentists<br />
in the US are in the private sector. Safety net facilities<br />
that target the underserved populati<strong>on</strong> employ about 2700<br />
dentists. Even after significant growth in health centers, that<br />
is still less than 2 percent of the total 178,000 active dentists<br />
in the US.<br />
Three things we must do as members of “organized<br />
dentistry” to improve dentist participati<strong>on</strong> in Medicaid:<br />
1. We must c<strong>on</strong>tinue to advocate for realistic Medicaid<br />
reimbursement rates.<br />
2. We must advocate easing the administrative burdens for<br />
Medicaid providers. Make the paperwork requirements<br />
like claim submissi<strong>on</strong>s, look more like the private sector<br />
plans. The complex system that exists adds to the cost<br />
of providing care and might result in errors that trigger<br />
costly reviews.<br />
3. Involve all state and local dental societies and<br />
individual dentists as active partners with existing<br />
safety net providers in improving the access to care<br />
issue.<br />
Charitable volunteer programs must remain a part of the<br />
soluti<strong>on</strong>. According to ADA’s 2000 “Survey of Current<br />
Issues in Dentistry,” 74 percent of private practice dentists<br />
provided free or reduced rate oral health care services<br />
to <strong>on</strong>e or more groups (homebound, handicapped,<br />
low income). The value of this care was $1.25 billi<strong>on</strong><br />
dollars. “Give Kids a Smile” was launched by the ADA<br />
in 2003, with more than 47,000 dental teams registered<br />
to participate, treated more than 500,000 children at an<br />
estimated value of $29.8 milli<strong>on</strong>.<br />
Political pressure has intensified by the existing Governor<br />
and the newly appointed <strong>St</strong>ate Surge<strong>on</strong> General, Ana<br />
M.Viam<strong>on</strong>te Ros M.D., M.P.H. Under the directi<strong>on</strong> of the<br />
<strong>St</strong>ate Surge<strong>on</strong> General and Deputy Secretary, Ms. Kimberly<br />
“Kim” Berfield, (a former state legislator), the formati<strong>on</strong><br />
of a state Oral Healthcare Workforce Ad Hoc Advisory<br />
Committee. Its charge is to address multiple issues ranging<br />
from public policy, professi<strong>on</strong>al practice issues, supply<br />
and demand influences, educati<strong>on</strong>al and training matters<br />
and regulatory questi<strong>on</strong>s, as they relate to the access to<br />
care disparities that exist in our state. Your current FDA<br />
President, Dr. Chuck Hoffman, and other members of<br />
organized dentistry make up a part of the task group that<br />
includes many communities of interest such as the Florida<br />
<strong>Dental</strong> Hygiene Associati<strong>on</strong>, Community Colleges, and the<br />
Public Health sector.<br />

Volunteering and Obtaining Sovereign Immunity Made Easy<br />
The Volunteer Health Care Provider Program, a result of the “Access<br />
to Health Care Act” secti<strong>on</strong> 766.1115, Florida <strong>St</strong>atutes, which was<br />
enacted in 1992, provides licensed healthcare professi<strong>on</strong>als sovereign<br />
immunity protecti<strong>on</strong> for uncompensated services rendered to eligible<br />
patients referred by the department.<br />
Dentists volunteering for community or faith-based organizati<strong>on</strong>s are<br />
agents of the state, in that they provide services for patients referred<br />
from the department and agree to certain c<strong>on</strong>tractual stipulati<strong>on</strong>s.<br />
This c<strong>on</strong>tract provides you with the same liability protecti<strong>on</strong> as state<br />
employees when you provide healthcare services. This program, with<br />
its limited resources and restrictive eligibility, is a small but vital<br />
comp<strong>on</strong>ent in the department’s effort to increase access to health care<br />
for those in need throughout the state.<br />
Volunteers are needed in every county and specialists are especially<br />
needed. You can decide <strong>on</strong> your commitment level, even <strong>on</strong>e day a<br />
year will make a difference. The Volunteer Health Services Program<br />
Regi<strong>on</strong>al Coordinators will assist you with completing the sovereign<br />
immunity paper work. Patients are screened and scheduled through<br />
each county program.<br />

Your c<strong>on</strong>tributi<strong>on</strong> helps the WCDDA Fund make a difference in access to care and educati<strong>on</strong> in our district.<br />
The WCDDA Fund<br />
o Supports dental health educati<strong>on</strong> and preventi<strong>on</strong> of dental diseases to all sectors of the public with a special focus <strong>on</strong> access to care<br />
programs.<br />
o Provides scholarships to dental hygiene, assistant, and laboratory technician students.<br />
o Assists dental programs in recruitment activities and specific needs required to excel in the educati<strong>on</strong> of all students.<br />
o Recognizes special requests from all entities of dentistry which enhance and promote the dental professi<strong>on</strong>.<br />
o Provides disaster relief to WCDDA members.<br />
The Fund grows with support from the membership of the WCDDA through individual c<strong>on</strong>tributi<strong>on</strong>s and m<strong>on</strong>ey raised by various WCDDA<br />
sp<strong>on</strong>sored activities. Participati<strong>on</strong> in WCDDA Fund activities assures growth and l<strong>on</strong>gevity of the fund, provides a charitable outlet, and<br />
guarantees success of all events. C<strong>on</strong>tributi<strong>on</strong>s must be clearly marked and designated to the WCDDA Fund. It is advised to c<strong>on</strong>tact your tax<br />
c<strong>on</strong>sultant to determine if your d<strong>on</strong>ati<strong>on</strong> is tax deductible.<br />
The WCDDA Fund is overseen by the WCDDA Fund Committee which approves all requests for assistance. These requests are forwarded to<br />
the Florida <strong>Dental</strong> Health Foundati<strong>on</strong> Board for final approval and distributi<strong>on</strong> of funds.<br />
In 2008, the Fund c<strong>on</strong>tributed to: The Crisis Center of Tampa Bay, Meals <strong>on</strong> Wheels, student participants and schools in Annual Meeting Table<br />
Clinics and multiple scholarships to dental hygiene and assisting students. 2009 c<strong>on</strong>tributi<strong>on</strong>s: D<strong>on</strong>ati<strong>on</strong> of dental supplies to Meals <strong>on</strong> Wheels,<br />
The Crisis Center of Tampa Bay and The Good Samaritan Missi<strong>on</strong>, ECHO and multiple scholarships to dental hygiene and assisting students.<br />

The Florida legislature is facing grave budget deficits.<br />
However, it has the resp<strong>on</strong>sibility of the well-being of<br />
citizens of Florida and therefore, they are looking for costcutting<br />
measures to provide more care with less funds to<br />
underserved populati<strong>on</strong>s. Their soluti<strong>on</strong> is to fragment the<br />
dental professi<strong>on</strong> (like the medical model). They propose<br />
to allow mid-level providers or auxiliaries – lesser educated<br />
providers who are not dentists to provide unsupervised<br />
care to underserved populati<strong>on</strong>s. They would be restricted<br />
to legally defined “Health access settings,” which is not<br />
<strong>on</strong>ly ec<strong>on</strong>omically unfeasible, but also works against the<br />
preventi<strong>on</strong> model. Because underserved patients often<br />
exhibit greater degree of complicati<strong>on</strong>s and other systemic<br />
health c<strong>on</strong>diti<strong>on</strong>s, the use of lesser-educated providers<br />
risks jeopardizing the patients’ health and safety. Your<br />
leadership, under Dr. Chuck Hoffman has fought hard<br />
to resist this apparently illegal and unethical legislative<br />
positi<strong>on</strong>. The battle is not over. It has been said that, “if<br />
you’re not at the table then you are <strong>on</strong> the menu.” We are<br />
currently both, at the table and <strong>on</strong> the menu.<br />
Organized dentistry must develop a market-based soluti<strong>on</strong><br />
to address access to care. It will require collaborati<strong>on</strong><br />
am<strong>on</strong>g professi<strong>on</strong>al organizati<strong>on</strong>s, local, state, and federal<br />
governments, community organizati<strong>on</strong>s and other private<br />
entities. We must stick together not as “obstructi<strong>on</strong>ists”<br />
but as “c<strong>on</strong>structive,” highly-ethical dentists wanting to<br />
provide the most appropriate dental care to all the citizens<br />
of Florida.<br />
In summati<strong>on</strong>, the challenges are many. I menti<strong>on</strong>ed<br />
earlier transportati<strong>on</strong>, social disorganizati<strong>on</strong> and behavior<br />
modificati<strong>on</strong> are all barriers. We, as a professi<strong>on</strong>, have to<br />
step to the plate and advocate for equal health care access<br />
for all. There are many great soluti<strong>on</strong>s being discussed but<br />
our professi<strong>on</strong> must be the designer of the program and<br />
processes to ensure a favorable and successful outcome for<br />
all parties c<strong>on</strong>cerned.<br />

The Last Word<br />

Earlier this year I had the distinct h<strong>on</strong>or of serving <strong>on</strong> Florida’s delegati<strong>on</strong> (17 th district) to the<br />
American <strong>Dental</strong> Associati<strong>on</strong>. Having both attended and helped administrate a few ADA annual<br />
sessi<strong>on</strong>s in the past, I had falsely assumed I had experienced most of what these meetings had<br />
to offer. I couldn’t have been more wr<strong>on</strong>g. So much occurs in the House of Delegates (HOD)<br />
oblivious to its surroundings. The average attendee would be remiss if all they accomplished was<br />
attending world class CE or playing with the latest dental toys <strong>on</strong> the exhibit floor. Although, I<br />
realize politics are not for every<strong>on</strong>e, I truly believe every licensed dentist should experience an<br />
ADA House of Delegates, even if you <strong>on</strong>ly drop by for an hour or so. It is enlightening to see what<br />
goes <strong>on</strong> behind the big banners and closed doors.<br />
Your delegates process a tremendous amount of material before (and during) the HOD. The 17 th district represented the state<br />
and the <strong>West</strong> <strong>Coast</strong> very well. They were involved in every priority topic and even played integral roles in some of the more<br />
memorable moments of this HOD. The 17 th district brought resoluti<strong>on</strong> 32H-2009 to the floor calling for the ADA to develop<br />
a logo for ADA member specialists who have graduated from a CODA accredited specialty program. On another fr<strong>on</strong>t, <strong>on</strong>e<br />
of Florida’s own made a valiant effort in running from the floor against a popular incumbent for ADA Treasurer. Dr. Jolene<br />
Paramore fell unbelievably short, by the slimmest of margins. Even though she was not elected as Treasurer, the HOD made<br />
it known that transparency and accountability are essential ingredients in a successful ADA.<br />
Rest assured you are very well represented by your volunteer colleagues <strong>on</strong> the 17 th district delegati<strong>on</strong>. Many of the<br />
seas<strong>on</strong>ed veterans said they had never seen an HOD like that <strong>on</strong>e. Being a rookie, I could <strong>on</strong>ly agree, but can’t wait to see<br />
what’s in store for next year’s in Orlando.<br />
