President’s Message
As I sit down to compose my president’s message, I’m looking at a calendar and realize that the end of
the year is quickly approaching. It also means that my term serving as president will have reached its
halfway point.

I made the last of my affiliate visits. You really get an appreciation of just how big our component is when
you have to drive down to Collier County one visit, up to New Port Richey and eastward to Lakeland for yet
another. All of the affiliates had their own personality, but I was continually impressed by the overall quality of
our members. I personally want to thank Drs. Churney, Palo, Cullinan and Desai for helping me out when I had
scheduling conflicts. I hope this tradition of visiting the various counties never fades.
During the Officers’ Planning Session at the WCDDA’s Summer Meeting in Naples, access-to-care was a topic on the agenda. Drs. Dan
Bertoch and Terry Buckenheimer proposed trying to create a pilot program utilizing the talents of those who provide dental services to
the underserved and the members of the WCDDA. The first step of this process occurred on October 23, 2009. A forum was held at the
WCDDA’s central office, proctored by Dr. Bertoch, with representatives from several public health settings in attendance. The purpose
of this forum was to identify where the association could be of assistance in providing dental care to the underserved.
Please register for the WCDDA’s 88 th Annual Meeting Friday, February 5, 2010 at the St. Petersburg Hilton Bayfront. The meeting will
feature the internationally renowned speaker Dr. Gerard Chiche presenting on Predictable Anterior Esthetics, Ms. Katherine Eitel on
Lioness Leadership and Angie Stone, RDH will talk about improving oral care in dependent patients and recall. CPR re-certification
will also be available in time for license renewal. There is something for all, so I hope that you’ll bring your staff. There will also be a
Presidential Cocktail Reception at the Museum of Fine Arts Thursday evening, February 4 th at seven o’clock. All are welcome to attend.
I hope everyone enjoyed the holiday season. Please keep our service men and women in your thoughts and prayers. I hope to see a lot

The Role of Organized Dentistry in Access to
Oral Health Care for Underserved Communities
For too long, dental disease has been a silent epidemic.
Further, there is a huge disparity that exists in oral health
care amongst lower socio-economic populations. It is
apparent not only in children, but the elderly, homebound
and handicapped populations as well.
What we know:
1. In general, disadvantaged groups have 2-3 times the
unmet need for dental care than average.
2. Low socio-economic status persons are 3-4 times less
likely to have an annual dental visit.
3. Only 22 percent of Medicaid–enrolled (low income)
children receive an annual dental visit in Florida.
4. There has been little increase in sealant use for schoolaged
children in low income families.
Barriers for disadvantaged persons include:
1. Low access to dental care through publicly funded and
volunteer programs.
2. Transportation difficulties.
3. Loss of work time without pay.
4. Low perceived value of dentistry.
5. Lack of understanding of the importance of routine
dental visits for prevention and early detection of
chronic disease.
Even though the problems are numerous and complex, they
are not insurmountable. Prevention and Education have to
be our main focus. Prevention, by advocating community
fluoridation, early school oral health screenings and school
sealant programs is essential. Education, with regards to
the importance of oral health to overall systemic health, and
early and regular preventative visits is equally important.
It is important to note that 90 percent of practicing dentists
in the US are in the private sector. Safety net facilities
that target the underserved population employ about 2700
dentists. Even after significant growth in health centers, that
is still less than 2 percent of the total 178,000 active dentists
in the US.
Three things we must do as members of “organized
dentistry” to improve dentist participation in Medicaid:
1. We must continue to advocate for realistic Medicaid
reimbursement rates.
2. We must advocate easing the administrative burdens for
Medicaid providers. Make the paperwork requirements
like claim submissions, look more like the private sector
plans. The complex system that exists adds to the cost
of providing care and might result in errors that trigger
costly reviews.
3. Involve all state and local dental societies and
individual dentists as active partners with existing
safety net providers in improving the access to care
issue.
Charitable volunteer programs must remain a part of the
solution. According to ADA’s 2000 “Survey of Current
Issues in Dentistry,” 74 percent of private practice dentists
provided free or reduced rate oral health care services
to one or more groups (homebound, handicapped,
low income). The value of this care was $1.25 billion
dollars. “Give Kids a Smile” was launched by the ADA
in 2003, with more than 47,000 dental teams registered
to participate, treated more than 500,000 children at an
estimated value of $29.8 million.
Political pressure has intensified by the existing Governor
and the newly appointed State Surgeon General, Ana
M.Viamonte Ros M.D., M.P.H. Under the direction of the
State Surgeon General and Deputy Secretary, Ms. Kimberly
“Kim” Berfield, (a former state legislator), the formation
of a state Oral Healthcare Workforce Ad Hoc Advisory
Committee. Its charge is to address multiple issues ranging
from public policy, professional practice issues, supply
and demand influences, educational and training matters
and regulatory questions, as they relate to the access to
care disparities that exist in our state. Your current FDA
President, Dr. Chuck Hoffman, and other members of
organized dentistry make up a part of the task group that
includes many communities of interest such as the Florida
Dental Hygiene Association, Community Colleges, and the
Public Health sector.

Volunteering and Obtaining Sovereign Immunity Made Easy
The Volunteer Health Care Provider Program, a result of the “Access
to Health Care Act” section 766.1115, Florida Statutes, which was
enacted in 1992, provides licensed healthcare professionals sovereign
immunity protection for uncompensated services rendered to eligible
patients referred by the department.
Dentists volunteering for community or faith-based organizations are
agents of the state, in that they provide services for patients referred
from the department and agree to certain contractual stipulations.
This contract provides you with the same liability protection as state
employees when you provide healthcare services. This program, with
its limited resources and restrictive eligibility, is a small but vital
component in the department’s effort to increase access to health care
for those in need throughout the state.
Volunteers are needed in every county and specialists are especially
needed. You can decide on your commitment level, even one day a
year will make a difference. The Volunteer Health Services Program
Regional Coordinators will assist you with completing the sovereign
immunity paper work. Patients are screened and scheduled through
each county program.

The WCDDA Fund
o Supports dental health education and prevention of dental diseases to all sectors of the public with a special focus on access to care
programs.
o Provides scholarships to dental hygiene, assistant, and laboratory technician students.
o Assists dental programs in recruitment activities and specific needs required to excel in the education of all students.
o Recognizes special requests from all entities of dentistry which enhance and promote the dental profession.
o Provides disaster relief to WCDDA members.
The Fund grows with support from the membership of the WCDDA through individual contributions and money raised by various WCDDA
sponsored activities. Participation in WCDDA Fund activities assures growth and longevity of the fund, provides a charitable outlet, and
guarantees success of all events. Contributions must be clearly marked and designated to the WCDDA Fund. It is advised to contact your tax
consultant to determine if your donation is tax deductible.
The WCDDA Fund is overseen by the WCDDA Fund Committee which approves all requests for assistance. These requests are forwarded to
the Florida Dental Health Foundation Board for final approval and distribution of funds.
In 2008, the Fund contributed to: The Crisis Center of Tampa Bay, Meals on Wheels, student participants and schools in Annual Meeting Table
Clinics and multiple scholarships to dental hygiene and assisting students. 2009 contributions: Donation of dental supplies to Meals on Wheels,

The Florida legislature is facing grave budget deficits.
However, it has the responsibility of the well-being of
citizens of Florida and therefore, they are looking for costcutting
measures to provide more care with less funds to
underserved populations. Their solution is to fragment the
dental profession (like the medical model). They propose
to allow mid-level providers or auxiliaries – lesser educated
providers who are not dentists to provide unsupervised
care to underserved populations. They would be restricted
to legally defined “Health access settings,” which is not
only economically unfeasible, but also works against the
prevention model. Because underserved patients often
exhibit greater degree of complications and other systemic
health conditions, the use of lesser-educated providers
risks jeopardizing the patients’ health and safety. Your
leadership, under Dr. Chuck Hoffman has fought hard
to resist this apparently illegal and unethical legislative
position. The battle is not over. It has been said that, “if
you’re not at the table then you are on the menu.” We are
currently both, at the table and on the menu.
Organized dentistry must develop a market-based solution
to address access to care. It will require collaboration
among professional organizations, local, state, and federal
governments, community organizations and other private
entities. We must stick together not as “obstructionists”
but as “constructive,” highly-ethical dentists wanting to
provide the most appropriate dental care to all the citizens
of Florida.
In summation, the challenges are many. I mentioned
earlier transportation, social disorganization and behavior
modification are all barriers. We, as a profession, have to
step to the plate and advocate for equal health care access
for all. There are many great solutions being discussed but
our profession must be the designer of the program and
processes to ensure a favorable and successful outcome for
all parties concerned.

The Last Word

Earlier this year I had the distinct honor of serving on Florida’s delegation (17 th district) to the
American Dental Association. Having both attended and helped administrate a few ADA annual
sessions in the past, I had falsely assumed I had experienced most of what these meetings had
to offer. I couldn’t have been more wrong. So much occurs in the House of Delegates (HOD)
oblivious to its surroundings. The average attendee would be remiss if all they accomplished was
attending world class CE or playing with the latest dental toys on the exhibit floor. Although, I
realize politics are not for everyone, I truly believe every licensed dentist should experience an
ADA House of Delegates, even if you only drop by for an hour or so. It is enlightening to see what
goes on behind the big banners and closed doors.
Your delegates process a tremendous amount of material before (and during) the HOD. The 17 th district represented the state
and the West Coast very well. They were involved in every priority topic and even played integral roles in some of the more
memorable moments of this HOD. The 17 th district brought resolution 32H-2009 to the floor calling for the ADA to develop
a logo for ADA member specialists who have graduated from a CODA accredited specialty program. On another front, one
of Florida’s own made a valiant effort in running from the floor against a popular incumbent for ADA Treasurer. Dr. Jolene
Paramore fell unbelievably short, by the slimmest of margins. Even though she was not elected as Treasurer, the HOD made
it known that transparency and accountability are essential ingredients in a successful ADA.
Rest assured you are very well represented by your volunteer colleagues on the 17 th district delegation. Many of the
seasoned veterans said they had never seen an HOD like that one. Being a rookie, I could only agree, but can’t wait to see
what’s in store for next year’s in Orlando.
