Søfartsstyrelsen afholdte den 26. november 2008 kontrolsyn, da KINGFISHER var<br />
kommet i havn efter ulykken. Synet fokuserede på forhold, der kunne have sammenhæng<br />
med ulykken. Der blev ved synet stillet krav om håndfang ved de hydrauliske<br />
styreringe og bedre afskærmning ved den hydrauliske betjeningspult.<br />
KINGFISHER – Occupational accident on 20. november<br />
2008<br />
Summary<br />
KINGFISHER departed Hanstholm on the 19 th of November 2008 at 1230 hrs bound<br />
for a fishing ground in the North Sea.<br />
On the 20 th of November shortly past midnight the trawl was to be launched. The wind<br />
was blowing from NW, approximately 20 m/s.<br />
In connection with extension of a bridle, connected to the port side trawl board, a fisherman<br />
got his hand squeezed between a hydraulically operated trawl pin and its guide<br />
ring. He sustained severe injuries to his hand.<br />
The injured fisherman was evacuated from the vessel approx. three hours after the<br />
accident.<br />
Conclusion<br />
There are certain distinctions in the statements regarding details of the instant accident.<br />
It is the opinion of The Division for Investigation of Maritime Accident that the accident<br />
occurred due to a combination of:<br />
• Lacking screening of the hydraulically operated trawl pin and its guide ring (6.1).<br />
• Missing screening of the stern gunwale by means of chains (6.1).<br />
• The hydraulically operated trawl pin was being used before the injured fisherman<br />
came clear of the arrangement (6.2).<br />
• The work tasks for the aft deck were insufficiently planned and the risk assessment<br />
for handling of trawl equipment was not covering the full scope of the work<br />
(6.3).<br />
Initiatives and recommendations<br />
As a starting point no work should be carried out in the area where the gunwale is<br />
missing unless it is screened off with wire or chain. The purpose of not having the stern<br />
gunwale is to avoid the numerous occupational accidents and the wearing out of the<br />
fishermen that used to prevail in connection with handling of heavy trawl equipment.<br />
