Arbeits- und Forschungsbericht 2011 und Planung 2012 Teilprojekte: Lungenfunktionsuntersuchung ehemals Asbest-exponierter Arbeitnehmer (Klinische Arbeitsmedizin (federführend)) Mitwirkung Reviews (AG Betriebliche Epidemiologie) Koordinationsaufgaben und Mitwirkung bei Reviews, Erstellung und Umsetzung der S2-Leitlinie „Begutachtung und Diagnostik asbestbedingter Berufskrankheiten“ (Zentralbereich). Stressprävention a) an Land, b) in der Seeschifffahrt Stressbelastung und Burnout-Gefährdung in der sich ständig verändernden modernen Arbeitswelt sind hervorstechende Herausforderungen und heute ein wissenschaftlich intensiv diskutiertes Thema. Schwerpunkte des ZfAM sind gegenwärtig die Ausprägung entsprechender Gesundheitsrisiken in speziellen Berufsgruppen, wie Erzieher, Ärzte sowie Seeleute. In einem umfassenden Projekt zur Stressprävention wird federführend von der AG Schifffahrtsmedizin ein Beitrag zur Förderung der Arbeitssicherheit an Bord erbracht. Der wissenschaftliche Fokus dieses über mehrere Jahre konzipierten Forschungsvorhabens liegt zunächst in einer Gefährdungsanalyse der Arbeits- und Lebensbedingungen an Bord und in der Entwicklung praxistauglicher Präventionsstrategien. Die AG Psychomentale Belastung wirkt konzeptionell mit. Teilprojekte: Befragung von Seeleuten an Bord zu Stressbelastung und Burnout-Gefährdung (AG Schifffahrtsmedizin (federführend)) Methodische Beratung zu psychometrischen Verfahren und Instrumentarien (AG Psychomentale Belastungen). Psychomentale Belastung und Beanspruchung in speziellen Berufsgruppen (AG Psychomentale Belastung (federführend) Auswertung der Symptomatologie des Burnout-Syndroms (AG Betriebliche Epidemiologie) Working and Research Activities 2011 and Plans 2012 Introduction The ZfAM consists of six specialist working groups as well as the port and airport medical services. These groups are coordinated, managed and led by the central executive, which also oversees occupational health policy issues. The ZfAM provides its services in many areas through close collaboration between external partners and in house groups. This endeavor relies on a successful interaction between the different professional disciplines, the various responsibilities of the scientific groups and the commitments of the service areas, such as the occupational health clinic and toxicological and immunological laboratories, as well as the governmentally imposed responsibilities placed on the Port and Airport Medical Service (Hafen- und Flughafenärztlicher Dienst, HÄD). Major Interdisciplinary Research Projects Five main interdisciplinary research projects continue to be pursued in recent times (see overview on p.34). 31

Working and Research Activities 2011 and Plans 2012 Health Risks Posed by Goods Transport The vulnerability of workers to exposure from pesticides and toxic industrial chemicals, both in ports and during the handling and shipping of cargo, represents a new, internationally relevant, publicly acclaimed, health issue and provides a focal point of the ZfAM. The Occupational Toxicology and Immunology working group continues to develop practical on-site analytical methods that are sensitive and reliable, for the prevention of potential health hazards created by container fumigation. The close cooperation with employees in the logistics industry and the Clinical Occupational Medicine investigators ensure rapid and effective intervention for the benefit of patientssuspected of being poisoned. The Port Health Service oversees health matters and fumigation procedures, assists in the identification of contaminated containers and ships, and participates in occupational health and safety committees. Side projects: Fumigant analysis, planning, design and implementation of ambient and biological monitoring of fumigants (directed by AG Occupational Toxicology and Immunology) Pulmonary function testing and clinical studies, including neuropsychological assessments after accidental toxic exposure (AG Clinical Occupational Medicine) Working Group overall design and coordination (Central Executive) External Projects: Responsibility for monitoring and identification of container and ship fumigation (Port and Airport Medical Service (Hafen- und Flughafenärztlicher Dienst, HÄD)). Specific neurological and psychometric diagnostics in intoxication (Clinic for Psychiatry and Psychotherapy at the UKE), specific early diagnosis techniques for tumors (Institute of Tumor Biology at the UKE), the development of health protection toolkits for the transport sector for use against toxic industrial chemicals (WHO Collaborating Centres). Prevention and Management of Occupational Asthma In 2011, we developed a set of international guidelines for the prevention and management of occupational asthma and incorporated these in the relevant federal policies, accordingly. The ultimate aim is to facilitate a substantial improvement in the prevention and treatment of, and successful early intervention in, workrelated asthma. A substantial decrease in disease severity, morbidity and mortality, as well as costs, should be attainable. The guidelines are intended for all who are concerned with asthmatic disorders. While the ZfAM central executive concentrates on policy-making and international coordination, the Clinical Occupational Medicine and Occupational Epidemiology working groups determine and assemble the evidencebased knowledge from which the recommendations and guidelines are derived. Side projects: Establishment of national and international guidelines in conjunction with a) WHO and ERS, b) DGAUM (directed by the Central Executive) Participation in the selection of published findings and their evaluation (AG Clinical Occupational Medicine and AG Occupational Epidemiology). Evaluation of preventative measures for bakers sensitized to the dust of flour (AG Clinical Occupational Medicine) Isocyanate Asthma Isocyanates are widely used in the industrial production of polyurethane foams, coatings, elastomers, curing agents, adhesives and paints. But, the high chemical reactivity of isocyanates also ensures that they are major causes of asthma and other work-related diseases. The AG Occupational Toxicology and Immunology works with the central executive in the development of new biomonitoring processes and with AG Clinical Occupational Medicine in the establishment of new sensitive methods for improving isocyanate asthma 32
