4. Radionuclide renography 3 : (Renal scientigraphy)<br />
Proforma<br />
IVP was in the past the primary radiographic study used to define UPJO. In<br />
most institutions, this has been supplanted by radionuclide renography, which<br />
provides differential renal function data and an assessment of washout from the<br />
individual kidney. Several radiopharmaceuticals have been used, each with slightly<br />
different properties.<br />
Diethylenetriamine-penta-aceticacid (DTPA) is eliminated entirely by<br />
glomerular filtration, with an extraction efficiency of 20%. It provides indirect means<br />
of measuring the GFR.<br />
Differential GFR can be measured by comparing the amount of uptake in each<br />
kidney during the first 1 to 3 minutes after intra venous injection (Rowell et al, 1986).<br />
Mercaptoacetyltriglycine (MAG-3) is cleared by the kidneys primarily by<br />
tubular excretion and to a lesser extent by glomerular filtration. Therefore MAG-3 is<br />
an excellent agent for estimating renal plasma flow. Differential renal plasma flow<br />
measured with MAG-3 correlates reasonably well with differential renal function.
