*($1900/term for Sunday Girls Soccer – a separate form with more details will be sent upon registration).
All registrations received after the dates below will be subject to a $300 late fee.

• Medical waiver and additional information
In the event of an emergency, when contact cannot be made with the parent, I/we authorize HKIS and/or its representative to administer basic first aid and to take my child
to the nearest Hospital if and when emergency care is needed. I/we do not hold HKIS and/or its representatives responsible for authorizing treatment or for any costs and/or
expenses resulting from such treatment. I/we also authorize the Hospital to perform any emergency care deemed necessary for the treatment of my child.
I will not hold Community Sports or their employers responsible for any injury to my child, or damages to property as a result of participation in the course.
