Effective October 1, 2012, prior versions of the following printed and Internet only forms
will not be accepted unless the form was processed in a prior report of deposit of fees
(RDF), suspense transaction, or was signed and dated prior to September 30, 2012.

For all forms:
• Ensure that current form revisions are being used.
• Destroy all outdated copies.
For Internet only forms:
Photocopies must be made from an original form printed from the Department of Motor
Vehicles (DMV) website, not from a prior photocopy.
Background
Revised forms often contain legally required data or statements which were omitted on
previous versions.

was made via California DMV’s Automated E-mail Alert System in August 2012.
