A Cochrane review about the orthodontic treatment for deep bite and retrocl<strong>in</strong>ed<br />

upper front teeth <strong>in</strong> children, the so called Class II,2 malocclusion accord<strong>in</strong>g to Angle’s<br />

classification, was publ<strong>is</strong>hed <strong>in</strong> 2006.<br />

In patients <strong>with</strong> a Class II, 2 malocclusion often due to the deep bite a palatal or lower<br />

labial g<strong>in</strong>gival trauma <strong>is</strong> detected 18 ;furthermore there ex<strong>is</strong>ts an aesthetic impairment. It<br />

seems to be likely that a greater percentage of upper permanent can<strong>in</strong>es are impacted<br />

<strong>in</strong> patients <strong>with</strong> a Class II, 2 malocclusion.<br />

Different treatments may be used to correct th<strong>is</strong> malocclusion. It <strong>is</strong> not possible to<br />

provide any evidence-based guidance to cl<strong>in</strong>icians <strong>with</strong> respect to a Class II, 2<br />

malocclusion due to the fact that no random<strong>is</strong>ed controlled trial or controlled cl<strong>in</strong>ical<br />

trial was identified. 18 Although no cl<strong>in</strong>ical evidence was found on th<strong>is</strong> topic, it <strong>is</strong> clear<br />

that from a dental health po<strong>in</strong>t of view g<strong>in</strong>gival trauma should be avoided.<br />

No randomized or controlled cl<strong>in</strong>ical trials were found concern<strong>in</strong>g the treatment of<br />

Class II,2 treatment and no recommendation can be made.<br />

CLASS III MALOCCLUSION<br />

In a Class III malocclusion, the lower molars and can<strong>in</strong>es occlude too far mesially<br />

relative to the upper molars: th<strong>is</strong> type of occlusion <strong>is</strong> also called a mesio-occlusion<br />

(Figure 5). Besides the deviation <strong>in</strong> the sagittal occlusion, all <strong>in</strong>tra-arch deviations can<br />

also be superimposed on Cl III malocclusions.<br />

Only a protocol for a Cochrane systematic review ex<strong>is</strong>ts. <strong>The</strong>refore the treatment of<br />

Class III malocclusions <strong>is</strong> d<strong>is</strong>cussed by means of other orig<strong>in</strong>al articles.<br />

In a meta-analys<strong>is</strong> Kim et al. 19 evaluated the effectiveness of maxillary protaction <strong>with</strong><br />

orthopaedic appliances for the treatment of Class III patients. <strong>The</strong> conclusion drawn<br />

from th<strong>is</strong> meta-analys<strong>is</strong> was that protraction face mask therapy <strong>is</strong> effective <strong>in</strong> grow<strong>in</strong>g<br />

patients, but to a lesser degree <strong>in</strong> patients older than 10 years, and that protraction<br />

comb<strong>in</strong>ed <strong>with</strong> an <strong>in</strong>itial period of expansion might provide more significant skeletal<br />

effects.
