We provide compassionate and dignified end-of-life care for terminally ill patients and their families, including comprehensive medical care and emotional, social spiritual and grief support. Hospice focuses on caring, not curing, under the premise that each of us is entitled to die pain free and with dignity, surrounded by family and loved ones with the support of caring people who understand the process of dying peacefully. Hospice of the Rapidan is dedicated to helping patients maintain their dignity, comfort and as much quality of life as possible, to helping each patient and their loved ones make the most of the time remaining and to support them as they confront their fears, emotions and grief. Hospice of the Rapidan cares for most patients in the familiar comfort of their hones, but also serves patients in nursing homes and area hospitals. Hospice of the Rapidan is also a leading area resource for caregivers of terminally ill patients and for information about end-of-life issues. Medicare/Medicaid/Private pay accepted. Hospice of the Rapidan will not turn anyone away regardless of their ability to pay. Hospice Support volunteers receive 30 hours of intensive training in order to meet the emotional, spiritual, and practical needs of the patient. If a patient requires skilled medical care, an appropriate referral is made. Hospice Support works collaboratively, as requested, with attending physicians, medical hospices, clergy and other support agencies in order to provide a continuum of patient care. Its mission is to encourage, support and provide options to people with disabilities. IEC helps people to gain access to appropriate community services, exercise their civil rights, participate in all aspects of society and advocate their value to society. Like the other 15 Centers for Independent Living throughout Virginia, IEC offers four core services: Information and referral, advocacy (individual and systemic), peer counseling, and independent living skills 6

training. In addition, IEC facilitates the Medicaid Elderly or Disabled Consumer Directed Medicaid Waiver and is a transition coordination agency for Virginia’s Money Follows the Person Program, which brings people out of nursing homes to live independently in the community. MHAFC currently supports housing subsidies for county residents with mental health issues, and provides direct support to Fauquier County Public Schools to assist at-risk students. MHAFC also works in collaboration with other community groups and providers to advocate for and strengthen the delivery of mental health supports in this area.
