How can I get and stay healthy?

When it comes to good health, we have programs to help you live healthier.

Personal health support

If you’re dealing with complex health care

needs or living with a chronic condition, such

as diabetes or hypertension, it can be difficult

to sort through all the available information to

make informed decisions.

Personal Health Support can provide you with

access to specialized nurses, as well as other

resources that can help you better manage

your health care needs and improve your

quality of life.

Your eligibility for this program is determined by

UnitedHealthcare when you experience certain

health events that require medical attention, or if

the results of your health assessment reveal that

you might need assistance. If you are eligible

for the program, a Personal Health Support

Nurse assigned to you will contact you by

phone. A Personal Health Support Nurse

will explain the program details to you and

help you enroll in the program.

A Personal Health Support Nurse will:

Provide you with one-on-one health care

information, guidance and support.

Help coordinate your care with

physicians and health care professionals.

Support you in understanding and

following your doctor’s treatment plan.

Treatment Decision Support

If you are considering treatment decisions for a

specific condition, Treatment Decision Support

(TDS) is available to help you make informed

decisions. TDS can help you:

Select the treatment option that best

meets your needs.

Choose a physician and hospital for your

treatment.

Prepare you for your upcoming

treatment and for a successful recovery.

You can call the nurse at 1-888-866-8295 to

talk about treatments for these conditions:

Heart disease (angina)

Musculoskeletal (back pain, hip

replacement, knee replacement)

Men’s health (benign prostate disease,

prostate cancer)

Women’s health (breast cancer,

Where do I go for wellness programs and tools?

When it comes to living well, we can help you stay healthy or get healthy. These services are part

of your benefits available to you at no additional cost.

Health Assessment

Available on myuhc.com, this is a

comprehensive, confidential online health

questionnaire. It analyzes lifestyle behavior and

identifies individual health risks. Participants

receive a personalized health report, along with

preventive steps and recommendations.

Online Health Coach Programs

Based on data from the online Health

Assessment, the Online Health Coach creates

a personalized health improvement plan

that encourages behaviors that can lead to a

healthier lifestyle. Complete with tracking

tools and quizzes, the program offers online

courses on exercise, nutrition, weight

management, smoking cessation, heart

health and more – all on myuhc.com.

HealtheNotes

UnitedHealthcare has a new way we

want to communicate with you.

The messages are called HealtheNotes

(pronounced “healthy notes”), and we designed

them to provide you with personalized care

opportunities to improve your health.

Enjoy discounts that could add up to big savings

One of the best things about a HealtheNote

is you don’t have to do a thing to receive one.

We’ll send them to you automatically when

we have a message or recommendation we

think would benefit you. For example, if you

are a diabetic and our evidence-based medicine

What if I get sick or injured?

Where you go for your care matters. You get the best care when you see your regular health care provider. They know you

and your medical history. For most medical problems, you should try to go to your regular health care provider first. But if

your doctor is not available, you can consider urgent care clinics, convenience care clinics and hospital emergency rooms. Take

a look below at the chart that could help you in choosing the right place for care for your situation.

Doctor’s Office You need routine care or treatment for

a current health issue. Your primary

doctor knows you and your health

history, can access your medical

records, provide preventive and routine

care, manage your medications and

refer you to a specialist, if necessary.

You can’t get to your doctor’s office,

but your condition is not urgent or an

emergency. Convenience care clinics

are typically located in many retail

stores offering services for minor

health conditions.*** Staffed by nurse

practitioners and physician assistants.

You may need care quickly, but it is

not an emergency, and your primary

physician may not be available. Urgent

care centers offer treatment for nonlife

threatening injuries or illnesses.

Staffed by qualified physicians.

You need immediate treatment of

a very serious or critical condition.

The ER is for the treatment of

life-threatening or very serious

conditions that require immediate

medical attention. Do not ignore an

emergency. If a situation seems life

threatening, take action. Call 911
