Health Records<br />

Health records help school staff support student needs.<br />

The following are required to enroll a student for the first<br />

time:<br />

n Verification of all state-required immunizations, recorded<br />

on Department of Health and Mental Hygiene (DHMH)<br />

form 896.<br />

n Documentation of a recent physical exam, recorded on<br />

the Maryland <strong>Schools</strong> Record of Physical Examination<br />

form.<br />

n Pre-k and kindergarten enrollment also require a<br />

Maryland DHMH Blood Lead Testing Certificate if the<br />

child lives or has ever lived in an area that the Maryland<br />

Targeting Plan for Childhood Lead Poisoning has designated<br />

"at risk" (see zip code list on the back of the form).<br />

Lack of these records may cause delayed enrollment or<br />

exclusion from school. (See Policy 420 and Regulation 400-64.)<br />

School offices have all required health forms and can<br />

provide further information. Forms are also at www.fcps.org/<br />

forms. For assistance in getting immunizations or a physical<br />

exam, contact the school health room staff or FCPS health<br />

specialist.<br />

FCPS encourages medical and dental exams for students<br />

entering middle school to help identify health concerns that<br />

may affect educational performance. ★<br />

Interscholastic Sports<br />

To participate in interscholastic sports activities, students<br />

must have an annual physical examination between June 1<br />

and the first day of practice (mid-August for fall sports). They<br />

must also have evidence of medical insurance coverage. See<br />

Insurance (below). ★<br />

Insurance<br />

FCPS does not have insurance coverage for students’ personal<br />

property. Students are responsible for their personal<br />

property on the bus, in school and during school activities.<br />

Limited student accident insurance may be purchased<br />

through a company approved to offer it through Maryland<br />

schools. A football accident policy is also available. Visit www.<br />

StudentInsurance-KK.com. Before purchasing this insurance,<br />

you may wish to consult with your insurance agent to see if<br />

policies you already hold provide adequate coverage.<br />

Note that FCPS does not endorse these programs and does<br />

not handle claims or associated problems.<br />

The <strong>Frederick</strong> <strong>County</strong> Health Department’s Health Care<br />

Connection assists with health insurance needs through<br />

resources such as the Maryland Children’s Health Program<br />

for children up to age 19 and pregnant women of any age from<br />
