Rationale for VMMC • Sub-Saharan Africa continues to be the most heavily affected region accounting for approximately 68% and 90% of the world’s adult and childhood HIV prevalent cases, respectively (UNAIDS, 2010). Despite HIV prevention efforts, prevalence rates in the region remain high with an estimated 11.3 million adults and children living with HIV in 2010 (UNAIDS, 2011). • Three randomised controlled trials (RCTs) were conducted in Africa to determine whether medically-performed circumcision of adult males would reduce their risk of HIV acquisition.
• Those who had been representatively assigned to the circumcision group had a 60% (South Africa), 53% (Kenya), and 51% (Uganda) lower incidence of HIV infection compared to men assigned to the control group. • Women indirectly benefit through the reduction in HIV prevalence of male partners • Unclear whether VMMC has positive impact on reduction in HIV transmission via anal sex.- MSM, transgender • Does not protect partners of HIV positive men who are circumcised.
