疤 痕 手 術 及 治 療
暗 瘡 、 蚊 叮 蟲 咬 、 意 外 創 傷 或 手 術 引 致 的 傷 口 , 會 傷 害 皮
膚 的 完 整 性 。 即 使 傷 口 會 於 一 至 兩 星 期 內 癒 合 , 皮 膚 組 織
仍 有 機 會 出 現 紅 腫 及 凸 起 的 疤 痕 , 一 般 會 於 六 至 九 個 月 內
逐 漸 消 退 。 然 而 , 部 份 人 的 疤 痕 不 單 不 會 消 退 , 皮 膚 組 織
更 會 變 得 又 硬 又 凸 。 嚴 重 者 甚 至 會 擴 散 至 傷 口 以 外 的 皮 膚
組 織 , 又 紅 又 癢 。 過 度 活 躍 的 皮 膚 纖 維 細 胞 會 製 造 大 量 骨
膠 原 , 令 疤 痕 持 續 生 長 兼 凸 起 , 引 致 敏 感 、 痕 癢 、 疼 痛 及
磞 緊 等 反 應 。
明 顯 的 疤 痕 一 般 可 分 為 三 大 類 :
凹 陷 疤 痕
源 於 皮 膚 組 織 受 損 , 真 皮 組 織 無 法 及 時 填 補 。 常 見 於 水 痘
及 暗 瘡 。
增 生 性 疤 痕
來 自 傷 口 癒 合 時 製 造 及 積 聚 過 多 骨 膠 原 , 令 傷 口 凸 起 、 光
滑 及 呈 暗 紅 色 。
疤 痕 疙 瘩
1. 手 術 修 補 法
手 術 除 可 收 細 疤 痕 外 , 亦 會 將 凹 凸 不 平 的 疤 痕 分 成 數
段 , 再 按 皮 膚 紋 理 , 將 疤 痕 變 成 波 浪 或 近 似 皺 紋 的 形
狀 , 令 其 看 來 不 太 明 顯 。
2. 激 光 治 療 法 ( 如 脈 衝 染 料 激 光 及 分 段 雙 層 集 束 激 光 )
利 用 激 光 特 有 的 波 長 , 以 收 緊 微 絲 血 管 或 改 善 增 生 性 疤
痕 。 新 一 代 激 光 系 統 , 能 有 效 改 善 泛 紅 的 疤 痕 。
病 人 需 每 月 接 受 多 次 激 光 治 療 , 方 為 有 效 。
3. 皮 下 幼 針 切 痕 法
在 疤 痕 皮 下 深 處 用 幼 針 放 鬆 纖 維 組 織 , 令 疤 痕 彈 起 , 以
改 善 凹 下 的 疤 痕 。 同 時 可 注 射 透 明 質 酸 填 補 下 陷 位 置 。
透 明 質 酸 一 般 可 維 持 三 至 六 個 月 ; 要 效 果 持 久 , 可 選
擇 將 身 體 其 他 部 位 的 脂 肪 移 植 至 下 陷 的 疤 痕 。
4. 矽 膠 布 及 壓 力 衣
穿 著 為 病 人 度 身 訂 造 的 壓 力 衣 , 可 以 抑 制 凸 起 的 增 生 性
疤 痕 及 疤 痕 疙 瘩 。 坊 間 另 有 原 理 類 近 的 矽 膠 布 , 以 壓 力
改 變 骨 膠 原 的 排 列 方 式 。
5. 注 射 類 固 醇
別 名 瘢 痕 瘤 、 蟹 足 腫 , 其 生 長 超 越 傷 口 範 圍 , 令 傷 口 凸
可 抑 制 骨 膠 原 的 製 造 , 減 輕 痕 癢 及 刺 痛 , 縮 細 疤 痕 。 每
出 、 變 圓 及 變 硬 。 邊 緣 呈 不 規 則 及 爪 狀 , 會 有 痕 癢 及 刺 痛
月 接 受 1 至 2 次 疤 痕 疙 瘩 注 射 , 可 達 致 最 佳 效 果 。

減 退 疤 痕 的 方 法
單 靠 手 術 切 除 疤 痕 疙 瘩 , 手 術 傷 口 會 有 機 會 再 長 出 更 大
的 新 疤 痕 , 復 發 率 極 高 ; 若 配 合 電 療 , 疤 痕 復 發 的 機
會 率 可 降 至 15 至 20%。

Scar Surgery & Therapy
Acnes, mosquito or bug bites, injuries from accidents or
surgical wounds—they all impair skin integrity. Even though
the wound may heal within 1 to 2 weeks, it still leaves red
swellings and protruding scars in the skin tissue. While most
of them usually disappear within 6 to 9 months, some scars
may remain as stiff and elevated as ever with no improvement.
In serious cases, they may even spread beyond the original
site, causing further itchiness and swelling. Massive production
of collagen by hypersensitive skin cells may stimulate scar
growth, heightening skin reaction, itchiness, pain and tightness.
Conspicuous scars are categorized as follows:
Depressed Scar
It is common in chicken pox and acne, mostly caused by the
late replacement of damaged skin tissue.
Hypertrophic Scar
A wound may produce collagen in excess while healing.
Excessive collagen accumulates in the scar, making it look
elevated, smooth and firm red.
Keloid Scar
It spreads beyond the original wound, making the area elevated,
curved and hard. Its fringe area may appear uneven and
claw-like with itchy and stingy sensations.
2. Laser Treatment
(e.g. Pulsed Dye Laser & AFFIRM Fractional laser)
By applying laser of a specific wavelength, the treatment
can reduce micro-capillaries and modify collagen. A new
generation of laser systems can help improve reddish
scars.
Multiple treatments are required every month for optimal
results.
3. Subcision by Needling
A fine needle is inserted into the tissue below the scar to
release and push up the depression. Hyaluronic acid is
also injected to fill up the depressed area. The acid may
last 3 to 6 months. For long-term effects, fat transplanted
from other areas is used as the filling material.
4. Silicone Adhesive & Pressure Garment
Patients can restrain the growth of hypertrophic scar and
keloid scar by wearing tailor-made pressure garment.
One may also use silicone adhesive that applies pressure
to the skin and rearrange the collagen pattern.
5. Steroid Injection
It can inhibit collagen production, relieve itchy, stinging
sensations and shrink the scars. 1 to 2 injections are
required every month for optimal results.
6. Post-Op Radiotherapy
Surgery cannot definitely remove keloid scars. It is highly
probable that a bigger scar may grow again in the wound
unless the surgery is complemented with radiotherapy. It
can reduce the rate of recurrence to 15 to 20%.

can make them less obvious with the help of laser, medication
and surgery.

Excisional surgery can narrow a scar or divide it into
numerous segments. These segments are then
transfigured into wave or wrinkle-like patterns, all in
close resemblance to one’s natural skin color and
texture.
