Yukon Home Care Program<br />
The Yukon Home Care Program assists people to<br />
maintain their independence while living at home. The<br />
Home Care Team is made up <strong>of</strong> social workers, nurses,<br />
home support workers, physiotherapists <strong>and</strong> occupational<br />
therapists. <strong>Services</strong> are based on assessed needs <strong>and</strong> are<br />
free <strong>of</strong> charge. Available services include:<br />
• Assistance with bathing <strong>and</strong> dressing, light<br />
housework <strong>and</strong> meal preparation;<br />
• Home adaptations to increase safety <strong>and</strong><br />
accessibility;<br />
• Chronic disease management support <strong>and</strong> health<br />
promotion <strong>and</strong> prevention support<br />
• Assistance with wound care or other medical<br />
treatments;<br />
• Prescription exercise programs, braces <strong>and</strong> mobility<br />
equipment;<br />
• Counseling, advocacy <strong>and</strong> assisting with referrals to<br />
appropriate community programs <strong>and</strong> support;<br />
• Respite care for caregivers;<br />
• Palliative care<br />

Palliative Care Program<br />
Palliative care is an approach to care <strong>of</strong>fered to a person<br />
affected by a life-threatening illness. Its goal is much<br />
more than comfort in dying; palliative care is about<br />
living, through detailed attention to control <strong>of</strong> pain <strong>and</strong><br />
other symptoms, supporting emotional, spiritual <strong>and</strong><br />
cultural needs <strong>and</strong> maximizing functional status.<br />
Palliative care is not a physical location but a philosophy<br />
<strong>of</strong> care at the end-<strong>of</strong>-life.<br />
