In September of 1976, the Congress and the President of the
United States enacted the Indian Health Care Improvement Act
(IHCIA; [P.L. 94-437]), which declared that “it is the policy of
this Nation, in fulfillment of its special responsibilities and legal
obligation to the American Indian people, to meet the national
goal of providing the highest possible health status to Indians
and to provide existing Indian health services with all resources
necessary to effect that policy.”
In 1977, the Indian Health Service awarded their first scholarship
laying the educational foundation for American Indian and Alaska
Native students to train as health professionals serving within Indian
health communities. Today, the IHS Scholarship Program has grown to
support, educate and place health care professionals within medically
underserved Indian health programs throughout the continental
United States and Alaska.
in the need to decrease your course load from full-time to
part-time enrollment. Refer to the Required Documentation
Student Information: Maintaining Your Scholarship Eligibility
Required Documentation
Scholarship recipients are required to provide two pieces
of documentation every academic term documenting their
academic progress. You must submit this documentation
(as described below) to maintain your scholarship eligibility
and continue to receive financial aid.
Recipient’s Initial Program Progress Report
You must submit a Recipient’s Initial Program Progress Report
(IHS-856-8) form to your IHS Scholarship Program Analyst within
30 days of the beginning of every academic term. Your school
advisor or the registrar’s office must sign this form to verify that
your enrollment status matches your approved status when
accepting your scholarship.
Official Transcripts
You must submit an official transcript to your IHS Scholarship
Program Analyst within 30 days of the end of every academic
term. Official transcripts must arrive in a sealed envelope with
the institutional seal and/or the signature of the registrar. Copies
of official transcripts or electronically produced grade reports
are not accepted. Do not remove official transcripts from your
institution’s original mailing envelope. Transcripts that appear to
have been handled and/or altered in any way are not accepted.
Your financial aid will be suspended if official transcripts or other
approved documentation are not submitted within 30 days.
If an official transcript cannot be made available within
30 days, the scholarship branch office will accept an official
grade report, signed by your instructors and your advisor.
You are required to provide an official transcript once it is
made available by your college/university.
Additional Documentation
In addition to your Recipient Initial Program Progress Report
(IHS-856-8) form and official transcript, the IHS Scholarship
Program provides the following forms to update your Program
Analyst should you encounter academic troubles, need to change
your status or provide the program with a change of name,
address or permit credit validation.
Notification of Academic Problem
You must submit a Notification of Academic Problem
(IHS-856-9) form to your Program Analyst if you experience
an academic problem that could affect your enrollment status.
Examples of situations when you should use this form include:
»»
Reduction in credit hours.
»»
Cancelled course.
»»
Academic probation.
»»
Withdrawal from school.
»»
Dismissal from school.
Under no circumstances should you withdraw from a course
or school without notifying your Program Analyst. You will
be placed on probation and jeopardize your financial aid
if you make a decision that affects your graduation date
without prior approval from the program.
Scholarship recipients who are dismissed from school will
be in breach of their agreement/contract and liable for
repayment of all financial aid. Go to Section E: Breach of
Contract and Default for more information.
12
Student Information: Maintaining Your Scholarship Eligibility
Change of Status
You must submit a form if you are requesting approval for a:
»»
School transfer or dual enrollment.
»»
Change in graduation date.
»»
Leave of absence (LOA) from the program.
The following information provides additional explanation
for each of these situations.
Transfer/Dual Enrollment Request
The IHS Scholarship Program requires immediate notification
(no later than 60 days) prior to the time of transfer or dual
enrollment at a new school.
This request is not available to scholarship recipients in their
first year receiving financial aid.
Valid Reasons
The IHS Scholarship Program will only approve a transfer/dual
enrollment request during the current academic year if you are:
1. Transferring to a school with an accredited program in your
chosen health profession.
2. Seeking dual enrollment at a school that offers course(s)
required for your health profession degree track or to
complete preparatory or pre-graduate requirements.
3. Experiencing a personal and/or family hardship. Each case is
considered on an individual basis.
Additional Documentation
You must submit the following documentation, in addition to the
Change of Status (IHS-856-10) form, in support of your transfer/
dual enrollment request:
»»
A letter from the school where you are transferring/seeking
dual enrollment that:
• Verifies your acceptance into an accredited program
associated with your scholarship.
• Details your curriculum.
• Lists the hours and courses earned at the previous school
that the new school will accept.
»»
Two Faculty/Advisor Evaluation (IHS-856-24) forms —
You must submit two hard copy evaluation forms if you are
transferring during the current academic year. You must
document all transfers that occur during the next academic
year as part of your application to request continued
scholarship support.
The scholarship branch office will notify you of its decision
within 10 business days of receipt of your request.
Transferring schools without prior approval from the IHS
Scholarship Program will result in termination from the
program. Health Professions scholarship recipients will be
in breach of contract and placed in default.
Change in Graduation Date
The program requires immediate notification if you anticipate a
change in your graduation date or completion of post-graduate
clinical training. You must also submit documentation, signed by
a school official, supporting this change.
The IHS Scholarship Program determines all requests on a
case-by-case basis. If your request is denied, the program will
discontinue your financial aid, grant you a leave of absence (LOA)
or find you in breach of your contract and place you in default.
13
Student Information: Maintaining Your Scholarship Eligibility
Change in Degree Program
Preparatory and Pre-Graduate scholarship recipients must apply
as new applicants and compete against all other applicants when
changing degree programs or progressing from their current
degree program to a Health Professions scholarship degree
program (for example, pre-medicine to medicine). You will
receive priority consideration for a Health Professions scholarship
if your application score is equal to applicants who have not
received a scholarship in the past.
Leave of Absence (LOA)
Scholarship recipients must submit a Change of Status
(IHS-856-10) form to request a leave of absence (LOA).
The program considers LOA requests on a case-by-case basis.
First-year recipients are not eligible. If you require a leave
of absence during your first year as a recipient, the program
requires you to pay back the financial aid provided to you and
to reapply and compete against new applicants if you want to
reenter the program.
LOA REQUESTS
The program will approve an LOA for one of two reasons:
» Poor performance on required courses.
» Medical or family issues.
The program will not approve an LOA for:
» Voluntary withdrawal from school (not taking classes
without prior IHS Scholarship Program approval for
medical or family issues).
» Involuntary removal from school (suspension or removal
from a professional school or academic program).
» Requests for time away from a normal degree track
or a delayed graduation date to complete:
• A joint degree program (for example, an MD/MPH
and PharmD/MBA).
• Additional majors or minors.
Name Change
Legal documentation (for example, a marriage certificate) must
accompany a Change of Name or Address (IHS-856-22) form
before the IHS Scholarship Program will update your profile.
Mailing Address
You are required to have an official mailing address for IHS
Scholarship Program correspondence during the length of your
scholarship award. A change in your mailing address may cause a
delay or loss of correspondence. If your address does change, you
must submit a Change of Name or Address (IHS-856-22) form.
Address changes received after the tenth day of each month will
not take effect until the following month. Previous scholarship
recipients have encountered delays of up to eight weeks in
receiving their correspondence when changing their address and
failing to notify the IHS Scholarship Program.
Note: The IHS Scholarship Program includes compensation for a post office box
in your August financial aid payment.
Request for Credit Validation
You are required to submit a Request for Credit Validation
(IHS-856-23) form to your Program Analyst to permit the release
of pertinent information from your file to those with whom you
have applied for credit (for example, credit card company, bank,
department store, property manager, etc.).
Approvals
The IHS Scholarship Program will discontinue any financial
aid during the approved period of your LOA. The program
will keep you in good standing until you are ready to submit
a continuation application and reenter the program. As long
as your Program Analyst is kept informed of your academic
status, you will remain in the active-non-pay status.
Current policy limits an LOA to a maximum of two consecutive
years, with annual requests for continued LOA approval.
14
Student Information: Scholarship Extension Application
Online Application
Scholarship recipients who are continuing their studies during
the next academic year under the same degree program and
meet program eligibility requirements are eligible to apply for
an extension of scholarship support.
You can access the scholarship extension application through
the Apply Now page of the program website. Accessing the
scholarship extension application will allow you to update your
contact information, graduation date and school information
(if necessary), as well as provide your Program Analyst with
your plans for the upcoming year, including:
»»
Requesting an extension of scholarship support.
»»
Applying for a Health Professions scholarship.
»»
Seeking a leave of absence.
»»
Leaving the program.
CHANGE IN DEGREE PROGRAM
Preparatory and Pre-Graduate scholarship recipients who
elect to change their degree program (for example predietetics
to pre-nursing or pre-medicine to pre-physical
therapy) must apply as a new applicant and compete against
other applicants seeking a scholarship in that program.
If you have completed your coursework or received a degree
and would like to continue in the program as a Health
Professions scholarship recipient (for example, pre-medicine
to medicine) you must apply as a new applicant and compete
against other applicants seeking scholarship support in that
degree program.
Health Professions scholarship recipients cannot change
their degree program (for example, pharmacy to medicine)
without prior approval from the Director, DHPS. If you receive
approval, the IHS Scholarship Program will update your
recipient profile with your new degree program so that you
can submit your extension application and continue receiving
scholarship support.
Contact your Program Analyst before changing your degree
program to discuss how doing so will affect your status as an
IHS scholarship recipient.
See the IHS Scholarship Comparison Chart
for a list of eligible degree programs.
Delinquent Federal Debt
The Delinquent Federal Debt (IHS-856-5) form provides you
with an opportunity to identify any delinquent federal debt past
due on your scheduled payments, which includes federal income
taxes, guaranteed or direct student loans, FHA loans and other
miscellaneous administrative debts. Delinquency is defined as
being more than 31 days past due on a scheduled payment for
direct and guaranteed loans. IHS does not consider deferred loans
to be delinquent.
You must complete and sign this form and include it with your
application. Fill in your name, career category, Social Security
number, email address and the IHS office through which you
are applying. Answer “YES” or “NO” to the question: “Are you
delinquent on the repayment of any federal debt(s)?”
If you answer “NO,” sign and date the form at the bottom and
submit it with your application.
If you are delinquent on the repayment of any federal debt,
check “YES.” Write an explanation of your delinquent debt in the
space provided. Include the name of the federal agency that you
owe, the type of debt (such as student loan or HUD mortgage,
for example), the name and phone number of a contact person
handling your debt and the account number.
Additionally, you are required to provide a notarized power
of attorney document (in some cases, the federal agency
may require you to use its own power of attorney document)
authorizing the release of information to the IHS Division of Grants
Operations to inquire about your debt. IHS will not consider your
extension application if you do not include this authorization. If
you have any questions regarding the power of attorney, contact
the IHS Division of Grants Operations. An example is provided on
Course Curriculum Verification
Your current advisor or counselor must verify and sign the
Course Curriculum Verification (IHS-856-6) form, which is used
to confirm that the courses you plan to take during the coming
academic year are consistent with the curriculum for major you
submitted when you first applied for a scholarship.
You are required to list the courses in which you have enrolled or
plan to enroll based on your school’s academic term (semester,
quarter or trimester). Include the course number, credit hours and
course title when completing the form. In the space to the right of
each section, fill in the total number of credit hours for each term.
Provide the completed form to your advisor or counselor for
verification, reminding him or her to sign, date and provide his
title and phone number.
Your Area Scholarship Coordinator can serve as a proxy advisor if
you are transferring to a new school or cannot get in contact with
your advisor. You must request that he reviews and signs this
document for submission with your application.
Be advised that courses not required for your degree
program will not count toward determining your full-time or
part-time enrollment status.
Faculty/Advisor Evaluation
For recipients transferring schools or seeking dual
enrollment: The following form is only required with
your application packet if a faculty/advisor evaluation(s)
cannot be completed through your online application.
Your Faculty/Advisor Evaluations forms will be submitted as part
of the online application if you indicate that you are transferring
or seeking dual enrollment at another college/university.
If your selected evaluators do not have email access or are unable
to access the forms, you are required to submit two completed
Faculty/Advisor Evaluation (IHS-856-24) forms with original
signatures. A letter of recommendation without this form is
not acceptable.
If you use the hard copy forms, make sure the forms are signed
by the evaluator, including the Statement of Conflict of Interest
(bottom of form) certifying that the evaluator is not related to
you by blood or marriage and can attest that the evaluation
provided is accurate. Any false representation is sufficient cause
for rejection of your application.
Instructions: Additional Documentation
Official Transcripts
You must submit an official transcript from your current college/
university with your application packet if you did not submit
your transcript within 30 days of the end of the most recent
academic term.
Curriculum for Major
You will need to submit a copy of your course curriculum for
your major if your curriculum has changed since submitting this
documentation with your initial application for an IHS scholarship.
Examples of changes include a change in minor or a schoolinitiated
change to your degree program requirements.
You must send your transcript in an official, sealed envelope with
the college/university’s institutional seal and/or the signature
of the registrar. The scholarship branch office will not accept
transcript copies; nor will it accept transcripts that show signs of
tampering. The IHS Scholarship Program branch office will make
the necessary copies of your official transcript(s) to fulfill the
application requirements.
20
Student Information: Financial Aid
The IHS Scholarship Program provides financial aid covering
tuition, required fees and educational and living expenses for all
scholarship recipients.
Read Potential Conflicts with Your IHS Scholarship for information
on how accepting an IHS scholarship can affect your acceptance of
other scholarships, grants and fee waivers.
Tuition and Required Fees
The IHS Scholarship Program pays for tuition and fees (calculated
by the educational institution) directly applicable to your approved
curriculum and scholarship requirements. Your scholarship award
is dependent on your maintaining your approved course load and
chosen degree program. Any substantial differences between
your official transcripts and the Course Curriculum Verification
(IHS-856-6) form you submitted in support of your scholarship
application can result in the loss of your financial aid.
Changes in Coursework
As part of the application process, you submitted a Course
Curriculum Verification (IHS-856-6) form and your curriculum for
major detailing the courses you will take for your chosen major
in the coming academic year, as well as those required to obtain
your degree. The program uses this information to determine
whether you are following your required curriculum and verifies
that you are on pace to graduate on time.
If unavoidable changes should occur (the school makes changes
to the curriculum or classes, has cancelled classes or the classes
are filled), you are required to submit a Notification of Academic
Problem (IHS-856-9) form to your Program Analyst immediately
documenting the changes that have occurred. Your Program
Analyst will work with you to determine the best course of action
to take and how the changes will affect your financial aid.
You must submit a copy of your scholarship award letter to your
school’s business office and financial aid office once it is received
from the Division of Grants Operations. This letter includes invoice
and payment instructions (allowing your school to bill IHS directly)
and officially notifies your school of your participation in the IHS
Scholarship Program.
Summer school is not included as part of your tuition and fees.
Scholarship support for summer courses must be requested
annually, no later than April 22. Refer to the Summer School
section for more information.
UNAPPROVED COURSEWORK
Academic work not covered under your financial aid
package include:
» Dual degree (for example, a Master of Public Health in
addition to the degree in your health profession or a
second major in a degree track unrelated to the core
health curriculum, such as business administration).
» Courses that must be repeated due to poor academic
performance. IHS will only assist with paying for these
courses if they are taken during summer school.
» Courses unrelated to your approved degree program.
» Audit courses.
Student Information: Financial Aid
Monthly Stipend
Scholarship recipients will receive a monthly stipend at the end of
each month via direct deposit to assist with their education and
living expenses. The amount of your stipend will be documented
in your scholarship award letter. Preparatory and Pre-Graduate
scholarship recipients will receive funding for a 10-month period
(August 1 – May 31). Health Professions scholarship recipients will
receive funding for a 12-month period (August 1 through July 31).
You must submit a new Direct Deposit Sign-Up Form (Standard
Form 1199A) if your banking information has changed since you
first accepted your scholarship.
Health Professions scholarship recipients scheduled to
graduate mid-year will receive two additional stipend
payments after graduation to assist with their transition
to a career as an Indian health professional.
August Payment
You will receive an annual lump sum in your August payment
labeled Other Related Costs (ORC) to cover books and travel,
as well as other pre-approved educational expenses. The IHS
Scholarship Program suggests that you budget your ORC payment
appropriately to cover your expected expenses over the course
of the academic year. IHS will not approve requests for additional
funds unless you provide documentation showing the ORC funding
did not sufficiently cover the approved education expenses.
Lost Stipend Payment
Your stipend payments will be deposited into your account during
the last three days of each month. Notify your Grants Scholarship
Coordinator immediately if you have not received your stipend
payment. You must also submit a Lost Stipend Payment
(IHS-856-19) form after the seventh day of the month following
the month that your payment was not received, so that the US
Department of Treasury can be authorized to reissue payment.
For example, if you do not receive your stipend at the end of
November, you should submit this form no earlier than December 7.
documentation from your Tribe or IHS facility that you are eligible
for health care and/or contract health care through an Indian
health program. If you find that the availability of health care
services is inconvenient, you will be responsible for a separate
health insurance policy (group or individual) while in school.
received your stipend before incurring any costs that you
cannot cover until the program deposits your stipend.
Summer School
All scholarship recipients are eligible to receive financial aid for
summer school per program policies.
Preparatory and Pre-Graduate Scholarship Recipients
The IHS Scholarship Program will pay up to $700 for
full-time students or $350 for part-time students for
summer school tuition and fees as billed by your school.
You are able to enroll in as many credit hours as you
choose; however, you must pay the cost of summer school
beyond what the program provides. No additional funds
are available for books or other miscellaneous expenses.
Health Professions Scholarship Recipients
The IHS Scholarship Program will cover all summer school
tuition and fees as billed by your school. No additional funds
are available for books or other miscellaneous expenses.
All recipients must submit a Summer School Request (IHS-856-21)
form to your Program Analyst by April 22 if you are attending
summer school. Your school advisor must sign the completed form
and attach your curriculum for major and documentation of tuition
and fees. You must only take the required courses for your degree
program, including retaking failed courses. IHS will not approve
summer school requests to take optional courses unrelated to
your degree program.
You are required to submit an official transcript to your Program
Analyst upon completion of your courses. Please ensure these
are ordered early to guarantee prompt delivery once the summer
term is completed.
Tutorial Assistance
The IHS would like to ensure that you are getting the maximum
benefit from your education. We encourage you to use tutorial
services to improve your grades (even if they are satisfactory)
and/or to address weaknesses in other courses, such as in
English or math, which may affect your overall academic
performance. Financial assistance for tutorial services can also
be used for licensure and certification examination preparatory
courses if the course is taken prior to completion of your degree.
Taxes
IHS scholarship stipends are subject to federal income tax
and possibly state and local taxes. IHS withholds only Federal
Insurance Contributions Act (FICA) taxes from your stipend checks.
Please contact the IRS or a tax professional to inquire about any
other tax liabilities associated with your financial aid package.
Student Information: Financial Aid
Suspension of Financial Aid
THE IHS SCHOLARSHIP PROGRAM WILL SUSPEND TUITION
AND FEE PAYMENTS FOR:
»»
The period of time that the IHS Scholarship Program has
approved a recipient’s leave of absence (LOA).
»»
Any repeat course work during the academic year.
Suspended tuition and fees, based on the approval of your LOA
and available funds, will not resume until you have notified
your Program Analyst that you are prepared to resume your
participation in the program. In addition, you are required to
submit the necessary supporting documentation from your school.
If repeat coursework does not delay graduation, but is taken in
addition to your normal full-time or part-time course load, the IHS
Scholarship Program will only pay tuition for the non-repeated
courses. This will not affect payment of your stipend payment.
Your scholarship award could be rescinded if your enrollment
status is affected by repeat course work.
THE IHS SCHOLARSHIP PROGRAM WILL SUSPEND THE STIPEND
PAYMENTS WHEN:
»»
You receive approval for an LOA.
»»
You fail to submit your Recipient’s Initial Program Progress
Report (IHS-856-8) form within 30 days of the beginning of
the academic term.
»»
You fail to submit your official transcripts within 30 days of
the end of the academic term.
The IHS Scholarship Program will not reinstate suspended stipends
until the above-mentioned reports/transcripts are received. It will
not issue payment until the next automated stipend cycle.
Extended delays in providing these required documents could
result in the program rescinding your scholarship.
Potential Conflicts with Your IHS Scholarship
Multiple Scholarships, Grants and Fee Waivers
All scholarship recipients must report their IHS Scholarship Program
award to both their school’s business office and financial aid office in
order to avoid unlawful duplication of federal funding. The applicant’s
school must list any scholarship, grants and fee waivers accepted
from sources other than the IHS Scholarship Program on its invoicing.
The IHS Scholarship Program will deduct other sources of financial aid
from the school invoice charges for tuition and fees before approving
final payment. Student loans are not included in this policy since the
student will repay the loans following his or her graduation.
The IHS Scholarship Program recommends that you decline all
other sources of outside funding so you can take advantage of
your full IHS scholarship award.
Other Public Health Service Benefits
If you are currently receiving scholarship funds or have a service
commitment under the National Health Service Corps (NHSC)
Scholarship Program (Section 751 of the Public Health Service Act)
or the Scholarship Program for First-Year Students of Exceptional
Financial Need (EFN) (Section 758 of the Public Health Service Act),
you are not eligible to participate in the IHS Scholarship Program
during the school year(s) for which you received those awards.
Note: Recipients of the Quentin N. Burdick American Indians
into Nursing Program or the American Indians into Psychology
Program may apply for an IHS scholarship; however, recipients
cannot participate in both programs at one time. Upon
completion of your degree, your IHS Scholarship Program
service commitment will take precedence over all
other service commitments. No other service commitment
can be served concurrently with the IHS commitment.
Veterans Benefits
You may continue to receive education benefits from the US
Department of Veteran Affairs (GI Bill) along with IHS scholarship
funds since VA benefits were earned by prior active duty in a
uniformed service.
Benefits from State, Local and Other
Federal Sources
If you owe a service commitment for professional practice to a state
or other entity under an agreement made before applying for an
IHS scholarship, you are ineligible for an award unless the state or
entity submits a written statement to the IHS Scholarship Program
Branch Chief indicating that:
»»
There is no potential conflict in fulfilling your service commitment
to the state or entity and the IHS Scholarship Program.
»»
You will fulfill the IHS Scholarship Program service commitment
before or concurrently (if applicable) with the service
commitment for professional practice owed to the state or entity.
(JRCOSTEP) candidates serve as an Ensign (O-1).
Employment Options
The IHS Extern Program offers three paths for employment:
»»
Civil Service — The federal civil service personnel system.
»»
Tribal/Urban Indian Program
»»
Commissioned Officer Student Training and Extern Program
(COSTEP) — A program of the US Public Health Service (USPHS)
Commissioned Corps.
The Area Scholarship Coordinator (ASC) serving the IHS Areas
identified as preferred destinations will forward resumes found
on USAJOBS to Tribal and Urban Indian program facilities in search
of externs in their Area. Externs hired through either of these
programs will be brought on and paid as federal civil servants.
Salary
Externs receive a salary based on experience and years of
academic training that is comparable to industry standards. IHS
waives the salary if the externship fulfills a required academic
field placement or an internship, in which case it will only pay the
required tuition and fees (scholarship students only).
Travel
You must submit a Request for Extern Travel Reimbursement
(IHS-856-18) form prior to travel to request reimbursement
for one round trip to the externship site. IHS authorizes travel
reimbursement based on federal travel and transportation
allowance regulations. If a travel advance is required, contact
your ASC and the IHS Area office, Service Unit or health clinic
where you are assigned.
Do not, under any circumstances, travel without authorized
travel orders.
Housing
You are responsible for finding your own housing. However,
information may be available from the ASC serving the Area
where your site is located or the local site. A minimal allowance
is available for transportation of goods, but authorization is
required on your travel orders.
Please stay in touch with your ASC and Extern Coordinator
to verify all of your arrangements before traveling to the
externship site.
Section B: Transition to
an Indian Health Career
Overview
This section of the handbook details the information
related to recipient requirements in preparation for
graduation, pursuit of post-graduate clinical training,
securing an IHS-approved position at an Indian health
facility and updating the program on your employment
status as an Indian health professional.
27
Transition to an Indian Health Career
Graduation
If you are on schedule to begin your Indian health career directly
after graduation, you should read the Fulfilling Your Service
Commitment section to familiarize yourself with the job search
process and employment requirements. The Post-Graduate
Clinical Training section provides information for those who are
pursuing a required or optional residency or training program for
board certification or licensure. You should familiarize yourself
with all service commitment requirements when you enter your
final year of training.
In addition to your service commitment or post-graduate training
preparations, all graduating recipients are required to submit the
following documentation during their final term of school:
Notice of Impending Graduation
You must send a Notice of Impending Graduation (IHS-856-13)
form to your IHS Scholarship Program Analyst at the beginning of
your final academic term.
Other Documentation
You must submit the following documentation after graduation:
»»
A copy of your diploma.
»»
An official transcript documenting the degree awarded.
»»
A copy of your license or board certification, if applicable.
28
Transition: Post-Graduate Clinical Training
The IHS Scholarship Program allows specific health profession
students an opportunity to request the deferment of their service
commitment in order to complete an IHS-approved post-graduate
clinical training program that fulfills board certification or licensure
requirements and has been approved by the appropriate certifying
boards, as determined by the Secretary of HHS.
Physicians, social workers, clinical psychologists and dietitians
are required to complete additional post-graduate training and
licensure before they can begin their service commitment.
Pharmacists, optometrists, nurses and dentists can elect to
complete one year of post-graduate training upon receiving
IHS Scholarship Program Branch Chief or DHPS Director approval.
The following information provides guidance on IHS-approved
programs for each eligible health profession.
Service Commitment Policy
You must begin your Indian health career within 90 days of
completion of your post-graduate training, engaging in full-time
clinical practice in the health profession for which you were
awarded a Health Professions scholarship. Failure to secure a
position could result in a breach of contract.
Read the Fulfilling Your Service Commitment section for
information on program policies and the job search process.
PRE-TRAINING REQUIRED DOCUMENTATION
Request for Approval of Deferment (IHS-856-11)
You are required to submit a Request for Approval of
Deferment (IHS-856-11) form to your Program Analyst at the
beginning of your final year of academic training documenting
the post-graduate clinical training programs that you are
considering after graduation. Your Analyst will review your
request and provide written approval of all programs that
meet IHS Scholarship Program policies.
Notification of Deferment Program (IHS-856-14)
You are required to submit a Notification of Deferment
Program (IHS-856-14) form to your Program Analyst by
May 31 of your final year of school, notifying the program of
the post-graduate clinical training program you will attend.
The program and specialty (if applicable) that you identify
on the Notification of Deferment Program (IHS-856-14)
must be the same program for which you were approved
when you submitted your Request for Approval of Deferment
(IHS-856-11). If you do not submit this form, you may be
found in breach of contract and placed in default.
Once you begin an IHS-approved post-graduate clinical training
program, you cannot transfer to another program without prior
approval from IHS Scholarship Program Branch Chief.
most up-to-date list.
Clinical Psychologists
IHS provides Health Professions scholarships to train clinical
psychology students to practice as independent, licensed
clinical psychologists. State licensing boards require that
graduate psychologists practice under the supervision of a
licensed clinical psychologist for a specified number of hours
(usually 2,000) before they are eligible to take the state exam.
Contact the State Psychology Board or visit the Association of
State and Provincial Psychology Boards for details.
You are required to complete a PhD or PsyD program and
become an independent, licensed clinical psychologist before
any service will count toward your service commitment.
You must complete all supervised work and licensure
requirements within the two-year deferment period.
Social Workers
IHS provides Health Professions scholarships to train social work
students to practice as independent, licensed clinical social
workers (title varies by state). State licensing boards require
graduate social workers to practice under the supervision of a
licensed clinical social worker for a specified number of hours
(2,000 – 3,000) before they are eligible to take the state exam.
Contact your State Social Work Board or visit the Association of
Social Work Boards for details.
You are required to complete a master’s degree program and
become an independent, licensed clinical social worker before
any service will count toward your service commitment.
You must complete all supervised work and licensure
requirements within the three-year deferment period.
Dietitians
IHS provides Health Professions scholarships to dietetics students
to train to practice as registered dietitians (RD). The American
Dietetic Association (ADA) requires dietitians to complete a
900-hour ADA-approved internship under the supervision of
an RD before you are able to take the ADA registration exam.
Go to the Commission on Dietetic Registration website for
information on the internship requirements and exam. Some
schools incorporate this internship into their educational instruction.
If you do not have the opportunity to perform an ADA-approved
internship during school, you are expected to secure your
internship (inside or outside the Indian health system) within
90 days of graduation. IHS requires you to complete a bachelor’s
degree program and become a registered dietitian before any
service will count toward your service commitment. You must
complete all supervised work and become a registered dietitian
within the one-year deferment period.
Transition: Post-Graduate Clinical Training
Optional Training
Pharmacists
Pharmacists can request a one-year deferment to complete a
one-year American Society of Health-System Pharmacists (ASHP)
or American Pharmacists Association (APhA) accredited Post-
Graduate Year One (PGY 1) Pharmacy Residency Program (Hospital,
Community or Managed Care only). Information about pharmacy
residencies is available at the American Society of Health-System
Pharmacists and the American Pharmacists Association.
Deferment requests to complete Post-Graduate Year Two (PGY 2)
residencies or fellowships will not be approved.
Optometrists
Optometrists can request a one-year deferment to complete a
one-year post-graduate Residency Program. Approved residencies
are for the Ocular Disease/Pathology and Primary Care
Optometry residency programs. Information is available from
Optometry Residency Match (ORMatch) or the Association of
Schools and Colleges of Optometry. You should consult the
IHS Discipline Chief for Optometry prior to applying to a
residency program.
Dentists
Dentists have the option to complete a one-year
Advanced Education Program in General Dentistry (AEGD)
or General Practice Residency (GPR). Only programs that
are fully operational with the ADA CODA accreditation status
are permitted.
Nurses
Nurses have the option to complete a one-year training program
with an emphasis on clinical outpatient (OPD), inpatient (IPD)
and/or emergency (ERD) nursing skills (for example, the
Montana American Indian Nurse Internship program).
In-Training Requirements
You must comply with the following requirements to continue
your participation in your post-graduate clinical training program:
» Remain in good standing with your residency or
training program.
» Submit an Annual Status Report (IHS-856-16) and
letter of good standing from your program director or
coordinator to your Program Analyst at the end of each
year of post-graduate clinical training.
» Make no changes in the period, place of training or type
of training without prior approval from the IHS Scholarship
Program Branch Chief.
Potential Problems
Below are problems that can arise when applying for or
participating in a post-graduate clinical training program.
Unsuccessful Match
You must notify your Program Analyst immediately and discuss further
options if you are unsuccessful in securing a training program.
Incomplete Residency
Physicians who elect to withdraw from their residency program
and apply as a General Practitioner might encounter difficulty in
finding employment based on the hiring standards for physicians.
Physicians are significantly less likely to obtain positions in
target IHS Areas if they are not board eligible or certified.
This can result in a breach of contract based on the inability
to fulfill your service commitment.
Failure to Advance in Training
Notify your IHS Scholarship Program Analyst in writing if you did
not pass your PGY 1 level of training and/or did not advance to
the PGY 2 level. Your program director should also send a letter
to your IHS Scholarship Program Analyst.
Suspension of Deferment
Individuals requiring a suspension of their deferment due to
medical or family issues should contact their IHS Scholarship
Program Analyst immediately. IHS will approve extensions on a
case-by-case basis and may require additional documentation.
31
Transition: Fulfilling Your Service Commitment
You must fulfill your service commitment immediately upon
graduation or completion of your post-graduate clinical training.
The IHS Scholarship Program strongly recommends that you
begin the job search process, including filing the appropriate
documentation, as early as possible during your final year of
academic training to ensure you find a position that meets
program policies.
Pre-Employment Documentation
The following forms are due to your Program Analyst during
your final year of academic training.
Preferred Placement (IHS-856-12)
You must submit a Preferred Placement (IHS-856-12) form
to your Program Analyst at the beginning of your final year
of academic/post-graduate clinical training (for example,
August/September for spring graduates) indicating your
preferred IHS Areas and Indian health facilities to fulfill
your service commitment.
Placement Update (IHS-856-15)
You must send a Placement Update (IHS-856-15) form
to your Program Analyst if you are unable to secure
employment prior to completion of your academic or
post-graduate clinical training. You also must attach
documentation outlining your attempts to secure
employment (for example, your letter of application receipt,
interview dates, denial letters, etc.). This form may also be
used to express any dissatisfaction or note any problems
encountered while seeking a position.
This form and accompanying documentation are due 30 days
from the time you complete your training until you either
accept a position or reach the 90-day limit. If you have
not secured employment following 90 days, you will be
subject to involuntary placement according to the needs
and priorities of the IHS and Tribes.
Position Start Date
You must begin your Indian health career within 90 days of
graduation or completion of post-graduate training engaging in
full-time clinical practice in the health profession for which you
were awarded a Health Professions scholarship, regardless of your
facility’s policies.
If you have not secured an assignment within 90 days, you will
be subject to involuntary placement. The IHS Director, acting on
behalf of the Secretary of HHS, can exercise direct placement
authority to fill unmet health care provider needs identified by
IHS, Tribal and/or Urban (I/T/U) Indian health care facilities
across the United States. If you are unable or unwilling to secure a
position, you will be in breach of contract and placed into default.
Full-time Clinical Practice
The IHS Scholarship Program defines full-time clinical practice as
working a minimum of 40 hours per week. The 40 hours cannot
be worked in less than four days and no more than 12 hours of
work can be performed in any 24-hour period. Time spent in
on-call status will not count toward the 40 hours. Overtime
cannot be applied to another workweek.
You must spend at least 32 hours providing direct inpatient or
outpatient care. These services must be conducted during normally
scheduled hospital or clinic hours. The remaining eight hours may
be spent providing additional patient care and/or performing
practice-related administrative activities, with administrative
activities not to exceed eight hours per each two-week period.
Teaching, attending staff meetings, supervising other health
care professionals, making court appearances and other nontreatment
related activities pertaining to the health professional’s
approved position are considered practice-related administrative
activities. Supervision of other health professionals is defined as an
administrative activity if the health professional being supervised is
treating or providing health care services to a patient and billing or
documenting such treatment in his name. If the supervising health
professional is treating the patient and billing or documenting such
treatment under his name, this activity would be counted as direct
clinical services for the supervising health professional.
No more than five weeks, or 35 workdays, per service year can
be spent away from the practice for vacation, holidays, continuing
professional education, illness or any other reason. Absences
greater than seven weeks in a service year or any absent without
leave period will not count toward the service commitment. You
are required to notify the IHS Scholarship Program immediately
if you anticipate absences greater than five weeks so that your
service end-date may be recalculated.
32
Transition: Fulfilling Your Service Commitment
Job Search
The IHS Scholarship Program recommends that you begin
researching career opportunities at the beginning of your final
year of academic/post-graduate clinical training (for example,
July/August for spring/summer graduates or February/
March for fall/winter graduates). This research should include
familiarizing yourself with the Indian Health Service and a career
in Indian health; contacting an IHS recruiter or Tribal Human
Resources (HR) department to assist in identifying openings
in your preferred IHS Area(s); gaining an understanding of the
different career paths available at Indian health facilities; and,
finally, determining if your preferred position meets the program
requirements to fulfill your service commitment.
Completing these steps will assist you in securing a position that
fits your preferred location and career needs.
Step One — Familiarize Yourself with
IHS and an Indian Health Career
As you enter your final year of academic or post-graduated
clinical training, it’s important that you familiarize yourself with
the Indian Health Service, what a career in Indian health entails
and the IHS Areas and facilities where you would like to begin
your career.
Step Two — Contact an IHS Recruiter or
Indian Health Facility
IHS Recruiters are available to assist you with your search and
identify facilities with openings available in your discipline. You
are also encouraged to search IHS Jobs and USAJOBS to find
federal or Tribal facilities with openings in specific IHS Areas.
Step Three — Research Career Paths
A career in Indian health offers three exciting options to begin
your journey as an Indian health professional. Each career path
offers competitive salaries and benefits for health professionals
who practice as part of an interdisciplinary team and share
similar values in health care and life.
Whether you’re looking to work at a federal facility as a civil
servant, as a USPHS Commissioned Corps officer or you’re
interested in working as a direct Tribal or Urban Indian program
hire, we recommend that you research the location, facility and
position for each opportunity that interests you to ensure it is
the right fit for your personal and professional needs.
Civil Service
Health professionals who enter IHS as federal civil service
employees can expect a comprehensive package of
compensation and benefits that includes:
»»
Competitive starting salary and benefits.
»»
13 – 26 days paid vacation, depending on experience.
»»
10 paid federal holidays.
»»
Up to 13 days sick leave per year.
»»
Comprehensive health insurance.
»»
No-cost malpractice coverage.
»»
Comprehensive retirement program.
»»
Thrift savings (contributory) retirement plan with
government match.
»»
Annual continuing education where available.
Tribal/Urban Indian Programs
An alternative to a federal employment career path is to accept
employment directly from one of the more than 560 federally
recognized Tribes in 35 states. Employment benefits for those
who choose to work directly for a Tribe vary depending on the
Tribe and position, but benefits are comparable to those offered
by federal personnel systems.
USPHS Commissioned Corps
Health professionals who enter IHS through the US Public Health
Service Commissioned Corps may receive:
»»
An accession bonus (sign-up bonus) —
varies by specialty and requirements.
»»
30 days paid vacation.
»»
10 paid federal holidays.
»»
Sick leave as needed.
»»
Comprehensive health care.
»»
Tax-free subsistence and housing allowance.
»»
No-cost malpractice coverage.
»»
Non-contributory retirement.
»»
Thrift savings (contributory) retirement.
»»
Annual continuing education where available.
»»
Space-available flights on military aircraft.
33
Transition: Fulfilling Your Service Commitment
Employment as a commissioned officer requires a dual application
process that includes application to the Commissions Corps in
addition to employment. These tasks should be undertaken at
the same time so that when you are offered a position, your
Commissioned Corps paperwork is complete and you are eligible
to be commissioned into your new position. Visit the USPHS
for a complete list of commissionable health professions and
application information.
Step Four — Determine Eligibility of Facility
and Position
Once you’ve selected the facility(s) and position(s) that best fits
your needs, it is your responsibility to ensure that they meet the
eligibility requirements to fulfill your service commitment.
Your service commitment can be fulfilled at a/an:
1. IHS facility.
2. Tribal facility (contracted under the Indian Self-Determination
Act [P.L. 93-638]).
3. Urban Indian program (assisted under Title V, Health Services
for Urban Indians, of the IHCIA [P.L. 94-437]).
4. Private practice: For physicians, dentists and clinical psychologists
only. Private practice is an employment option only available
when there are no identified openings in your chosen health
profession. The facility must be located in a designated
Health Profession Shortage Area (HPSA) in which at least
75 percent of the patient base from that geographic area
are documented members or descendents of federally or
state-recognized Tribes. You cannot pursue private practice as
a means to provide contract or locum tenens services to IHS,
Tribes, Tribal organizations or Urban Indian programs. Final
approval of this option is at the discretion of the IHS Director.
You must submit the position description or vacancy
announcement (a brief description of the position, the site
and duties) to your Program Analyst for verification that
the facility and position meet program requirements.
SPECIAL CASES
Research
The purpose of the IHS Scholarship Program is to train health
professionals to provide clinical health care services to Indian
people. While research — especially research related to
American Indians and Alaska Natives — is beneficial to Indian
people, it does not meet the requirements of the IHCIA. IHS
will not approve requests for service in research positions or
clinical positions containing a substantial research component
to fulfill a scholarship recipient’s service commitment.
Epidemiology
Epidemiology is not considered a clinical health service for
most health professions. However, individuals receiving an IHS
scholarship for a Master of Public Health (MPH) degree with
emphasis in epidemiology may be approved to fulfill their
service commitment in an epidemiology position or clinical
position containing a substantial epidemiology component.
Step Five — Apply for Openings
You must submit all applications for federal positions (civil service
or Commissioned Corps) through USAJOBS. You can get started by:
»»
Creating an account.
»»
Creating a searchable resume. Uploaded resumes are not
searchable.
»»
Researching the definition (open continuous, full-time
permanent, etc.) of each job posting.
»»
Uploading up to 10 documents (license, diploma, transcripts, etc.).
»»
Searching for jobs by discipline. This will save your search and
you’ll be alerted to new jobs posted.
You must submit applications for Tribal or Urban Indian
program positions to the local Human Resources department
serving that facility. Reference the Find Health Care page of the
IHS website to locate Tribal and Urban Indian program facility
contact information.
Note: Hiring priority is given to IHS scholarship recipients. Please
indicate when you apply that you are to receive this priority.
Follow up with your IHS recruiter or Program Analyst monthly
regarding available openings.
34
Transition: Fulfilling Your Service Commitment
Step Six — Submit Verification of Employment
When you have accepted an IHS-approved position, you
must send Verification of Employment documentation that
includes your anticipated or actual start date. The following
documentation will satisfy this requirement:
»»
Federal civil service — Notification of Personnel Action
(SF-50) form. The program will also accept a hire letter
from the facility indicating your start date, position title
and position description.
»»
Tribal/Urban Indian program — A hire letter from the
facility indicating your start date, position title and
position description.
»»
USPHS Commissioned Corps — Call-To-Active-Duty (CAD) notice.
»»
Private practice — A contract, work agreement or hire letter
from the facility indicating your start date, position title,
position description and terms of employment and benefits.
Deadline: The appropriate documentation, including your
diploma, final transcript and licensure/board certification, is
required 90 days from the completion of your academic or
post-graduate clinical training. Failure to submit the verification
of employment document will result in breach of contract and
placement in default.
Service Commitment Documentation
Your reporting responsibilities do not end once you have begun
your career as a health professional. The IHS Scholarship Program
requires the following documentation from its former recipients
currently fulfilling their service commitment:
Annual Status Report
You must submit an Annual Status Report (IHS-856-16) form
annually along with a leave report summary, provided by your
local Human Resources Department, to your Program Analyst
until your service commitment is complete. This is required to
monitor your employment activity and document that you have
worked in an approved full-time clinical position in the health
profession for which you received funding while not exceeding
the maximum five weeks or 35 days of leave.
PRIVATE PRACTICE REPORTING
If you pursue the private practice option, you must also
provide quarterly reports documenting:
» The total number of patients seen.
» The total number of American Indian or Alaska Native
patients seen.
Transfer Requests
Your approved service commitment is site and position specific
— you will not be able to transfer to another site or position
without prior approval from the IHS Scholarship Program branch
office. Failure to obtain approval before transferring to another
site may result in breach of contract and placement in default.
If you want to transfer to a different facility, you must notify your
Program Analyst in writing, providing a position description and
vacancy announcement detailing the facility and position where
you would like to relocate. The program also requires three
letters with your transfer request (listed below). An appropriate
facility representative must sign, on official letterhead, any
letters from your current facility and your prospective transfer
site. These include:
»»
A letter from you stating why you are requesting a transfer.
»»
A letter from your facility administration (HR, CEO, etc.)
stating the impact of your transfer on the facility’s
day-to-day operations.
»»
A letter from the prospective site documenting the position
vacancy, your hire date, a position description and the impact
of your transfer on the facility’s day-to-day operations.
If IHS approves your transfer, you must provide verification
of employment documentation indicating when you will
begin employment at your new facility and the position.
You must also notify the IHS Scholarship Program if there are any
interruptions in employment, such as maternity leave; extended
sick, disability or family-friendly leave; outside training exceeding
10 days per each 12-month period; breaks in service related to a
change of work site; or any other extended time away from work
so the program can recalculate your service end-date.
Transition: Breach of Contract and Default
Failure to Complete Academic Training
You will be liable to the US government for repayment of all
Health Professions scholarship financial aid paid to you and to
your school on your behalf if you are dismissed from school for
academic or disciplinary reasons or voluntarily withdraw from
the program before graduating. Payment must be made within
three years from the date of the breach of contract or such longer
period as specified by the HHS Secretary.
Failure to Begin or Complete
Your Service Commitment
You will be found in breach of your contract and placed in default
by failing to begin or complete your service commitment.
Default Formula
You are liable to repay three times the amount of all scholarship
funding and benefits paid to you and to your school on your
behalf, plus interest, as determined by the formula:
Liability — Sample Calculation
A scholarship recipient received four years of support for
medical school with tuition, fees and books at $30,000 per year
and a stipend of $18,000 per year. The recipient completed a
four-year Internal Medicine residency, but failed to begin fulfilling
his service commitment.
A = 3Z(t-s/t)
»»
Z = (4 x 48,000); t = 48; s = 0
»»
A = 3(4 x 48,000)(48-0/48)
»»
A = 3(192,000)(1)
»»
Amount Owed = $576,000 (plus 10 percent interest per year*)
* Interest rate used is maximum prevailing interest rate as determined by the
US Treasurer. This example uses 10 percent. Interest is compounded based
on federal regulations.
A=3Z(t-s/t)
in which:
A is the amount the United States is entitled to recover.
Z is the sum of the amounts paid to or on behalf of the
applicant and the interest on such amounts that would be
payable, if at the time the amounts were paid, they were
loans bearing interest at the maximum legal prevailing rate
as determined by the Treasurer of the United States.
t is the total number of months in the applicant’s service
commitment period.
s is the number of months of the service commitment period
the participant served.
The amount the United States is entitled to recover will be paid
within one year of the date on which the applicant failed to
begin or complete his or her service commitment or failed to
meet the terms and conditions of deferment or a longer period
beginning on a date specified by the HHS Secretary.
