Parent Survey<br />
Your school is one of six Ohio schools taking part in a special project to improve schools.<br />
Parent involvement is a very important part of successful schools. Your school would like to<br />
know how they can best help families, and how they can work together with parents to help all<br />
children succeed in school. If you are a grandparent, foster parent or guardian, we consider you a<br />
parent and value your ideas along with mothers and fathers.<br />
Please answer the following questions by filling in the circle <strong>for</strong> one choice <strong>for</strong> each question.<br />
