Deforest-Windsor Clinic Opens<br />

The brand-new <strong>UW</strong> <strong>Health</strong> DeForest-Windsor<br />

Clinic opened on Monday, June 13, 2011,<br />

consolidating two older clinics in DeForest and<br />

Waunakee.<br />

Six family physicians, one longterm<br />

float, and three certified<br />

physician assistants staff the<br />

new 22,000 square-foot, LEEDcertified<br />

facility. The clinic also<br />

offers laboratory, x-ray, and<br />

mammography services, plus<br />

health and nutrition education<br />

classes.<br />

Eau Claire and Augusta Clinics Recognized as Medical Homes<br />

The DFM’s Eau Claire and Augusta <strong>Family</strong> <strong>Medicine</strong> Clinics<br />

were among the first clinics in the <strong>UW</strong> <strong>Health</strong> system to receive<br />

national recognition as medical homes.<br />

The clinics were awarded Level I and Level II recognition,<br />

respectively, from the National Committee for Quality<br />

Assurance (NCQA) Physician Practice Connections® – Patient<br />

Centered Medical Home (PPC-PCMH) program.<br />
