REGISTRATION DOCUMENTS – If the office has notified your family regarding missing documents for
your child(ren)’s registration, please forward them to the office immediately. Registration is not
complete unless we have all of the required documentation mandated by the Diocese of San Diego.
Your cooperation is very much appreciated.
EXTENDED CARE EMERGENCY FORMS: You will receive extended care information, sign-up forms,
and Extended Care Emergency forms in your family envelope on Tuesday, August 28. This
information will be available to new families at the New Parent Meeting on Wednesday, August 15th.
It is strongly recommended that you read the Extended Care booklet and fill out this Emergency
Information and return it promptly. Even if you do not intend on using Extended Care, it is prudent
to return this completed form. You never know when car trouble, flat tires, traffic jams, or prolonged
doctor appointments will interfere with your plans. Quite often we have been called by frantic
parents saying they are detained by events beyond their control.
SPECIAL MEDICATION INFORMATION: No medications of any kind can be administered by school
personnel without proper authorization. If your child needs special medication to be administered,
please bring it to the school office and fill out the Medical Administration Form authorizing the school
personnel to administer it. This applies to any prescribed and over-the-counter medication, including
aspirin, cough drops, etc. There is a sample copy of this form in the Parent Handbook. According to
our Diocesan Handbook #371: "Medications shall not be furnished to students by the school. No
medications of any kind can be administered by school personnel without written permission from
the parent/guardian stating the student's name, name of medication and dosage. If a pupil must have
medication during the school day, the following procedure is to be followed:
1. release from doctor or parent stating the nature of the medication;
2. the day's dosage must be sealed, labeled and have pupil's name attached and is to be kept in
the nurse's or school office;
3. the pupil is to come to the office for medication;
4. a pupil's medication is to be self-administered."
NON-RETURNING FAMILIES: If, for some reason, your child will not be returning to Our Lady of
Grace School in August, please notify the school office (466-0055) as soon as possible. You may leave
this message on our message machine if the office is not open. It is very important that we know the
status of our enrollment so we can notify the families on our waiting list if an opening does occur.
YEARLY CALENDAR: Please refer to the 2012/2013 school calendar which is on the school website
and is available in hard copy, upon request. Family trips are strongly discouraged during school days.
Refer to information concerning Mass days, holidays, vacations, and minimum days by subscribing to
weekly updates (see below).
WEEKLY COMMUNICATIONS / DAILY CALENDAR: Most happenings will be posted on the school online
calendar and on the website. Every Tuesday a family envelope with time-sensitive information
