Spiritual Autobiography Questionnaire<br />

We will be including some of your own special words in your Shabbat<br />

bulletin the day of your <strong>Bar</strong>/<strong>Bat</strong> <strong>Mitzvah</strong>. This is NOT your d’var Torah.<br />

Please fill out the following questions so the worshippers and guests can<br />

learn more about you.<br />

We recommend that you weave the answers into a mini-essay.<br />

Please e-mail Julie the answers/essay by the One-on-One <strong>Meeting</strong>!<br />

(about a month before your <strong>Bar</strong>/<strong>Bat</strong> <strong>Mitzvah</strong>)<br />

I would like to thank all of you for sharing this special day with<br />

my family and me. Thanks to all the people who have made this day<br />

possible. Thanks to the Rabbis, my <strong>Bar</strong> <strong>Mitzvah</strong> tutor, and the Cantor.<br />

Thank you, Mom and Dad, for showing me love, care, and support to<br />

help me reach this milestone in my life.<br />

Becoming a <strong>Bar</strong> <strong>Mitzvah</strong> means that I have a bigger<br />

responsibility in the Jewish community and that I am now considered<br />

a Jewish adult. As I am becoming a <strong>Bar</strong> <strong>Mitzvah</strong>, I will become more<br />

involved in the Jewish Community.<br />
