<strong>and</strong> learning from SAI<br />
reports. The trust has used<br />
QINMAC to assist the<br />

measuring outcomes <strong>and</strong> no<br />
systematic programme for<br />
measurement <strong>of</strong> clinical <strong>and</strong><br />
social care governance.<br />
The trust undertakes regular<br />
audits every year. However<br />

clinical audit taking<br />
account <strong>of</strong> six service<br />
st<strong>and</strong>ards developed by<br />
the Western Trust.<br />
The review team<br />
identified some evidence<br />
<strong>of</strong> scrutiny <strong>of</strong> information,<br />
but found that a<br />
systematic programme<br />
would greatly strengthen<br />
the approach. It was<br />
reported that recent<br />
audits have been<br />
completed by students<br />
working within the trust.<br />
The review team noted<br />
that measurement <strong>of</strong><br />
clinical <strong>and</strong> social care<br />
governance is reliant on<br />
the complaints procedure<br />
which may not be the<br />
