A, There are midshaft comminuted fractures of the radius <strong>and</strong> ulna.<br />

Linear fissure fractures extend both proximal <strong>and</strong> distal to the major<br />

fracture. There is caudal displacement of the distal fragment. The<br />

slight irregularity between the radius <strong>and</strong> ulna in their proximal<br />

thirds is the site of normal interosseous muscle attachment. The longitudinal<br />

fissure fractures both proximal <strong>and</strong> distal to the fracture<br />

site are of great importance <strong>and</strong> should be evaluated carefully in a<br />

fracture of this type. B, Postoperative radiographs of the right radius<br />

<strong>and</strong> ulna reveal that the fracture has been repaired by means of a<br />

Kirschner-Ehmer device. Alignment <strong>and</strong> apposition are good. The<br />

pins are well placed in the proximal <strong>and</strong> distal fragments. C,<br />

Radiographs were obtained 5 weeks after surgical repair. The<br />

Kirschner-Ehmer device has been removed. There is extensive callus,<br />

which bridges the fracture completely. A radiolucent line is still evident<br />

at the fracture site. There is an increase in bony density within<br />

the proximal radius <strong>and</strong> ulna. A similar area is in the distal radius at<br />

the site from which the pins were removed. Diagnosis: Healing fracture<br />

with slight malalignment of the fracture fragments. The bony<br />

response seen at the site of pin placement is normal <strong>and</strong> there is no<br />

evidence of infection.<br />
