REGISTRATION INSTRUCTIONS ON BACK

REGISTRATION AND PAYMENT OPTIONS

• Register and pay online at www.queenofpeacehs.org.

Campers must bring the signed permission slip the first day of camp.

List T-Shirt size in comment area when processing payment online.

• Call (708) 496-4728 to pay by credit card over the phone.

Campers must bring the signed permission slip the first day of camp.

• Mail enclosed form and permission slip with your check or

money order (payable to Queen of Peace High School)

To guarantee enrollment, register at least three weeks prior

to the start of camp. Registrations received after this date

are considered if space is available. Cancelled checks/online

receipts serve as enrollment confirmation. Refunds issued

with two week prior notice or extenuating circumstances.

CAMP FEATURES

• Individual and team skill development and conditioning

• Limited class size to maximize individual instruction

• Introduction of game rules and strategies for sports

• Individual and group contests

• Basketball, soccer and volleyball campers will receive a ball

FIRST DAY AT CAMP

• Enter north doors (gym)

• Kiddie Campers report to Queen of Peace Café

• Sport campers report to the Peace gym dressed for activity

• Tennis athletes bring a racket, softball players a glove
