13. In course of vigilance investigation<br />

into a transportation case certain<br />

system improvement have been<br />

suggested on formulating a uniform<br />

guideline regarding methodology for<br />

measurement of route distance<br />

required to finalize a transport<br />

contract.<br />

14. In course of vigilance investigation<br />

regarding unauthorized occupation of<br />

company’s quarter, Director (P) has<br />

been requested to take action for<br />

vacation of unauthorized occupation<br />

of quarters in the areas of <strong>CCL</strong>.<br />

15. In course of a vigilance investigation<br />

regarding fire at Coal Stock, it has<br />

been suggested that measures be<br />

taken up for quick liquidation of coal<br />

stock to ward-off the danger of<br />

spontaneous heating and possible<br />

deterioration in the quality of stacked<br />

coal.<br />

16. In course of vigilance investigation<br />

into the construction of light vehicle<br />

repair shed it was observed that the<br />

said construction was done as per PR<br />

available in the project which was not<br />

an approved one. The matter has<br />

been brought to the notice of D(T/O)<br />

for wide circulation so that this type<br />

of oversight does not occur while<br />

undertaking construction of a capital<br />

nature.<br />

17. While investigating a case of<br />

irregularity in tender proceedings of<br />

MM Department at HQ., following<br />

serious lacunae were detected.<br />

The TC had accepted the offer<br />

of one of the firm even when it<br />

was not fulfilling the eligibility<br />

criteria.<br />

Deficient work order was issued<br />

from MM Department, which<br />

was not carrying any time limit<br />

for submission of security<br />

deposit.<br />

Change was made in the<br />

existing standard eligibility<br />

criteria which had the approval<br />

of CMD.<br />

It was advised to take remedial action<br />

on the above indicated lacunae in<br />

order to eliminate recurrence of such<br />

type of irregularities / deviations in<br />

future.<br />

18. During the course of a Vigilance<br />

Investigation relating to alleged<br />

irregularities into the award of work<br />

in one of the Areas, certain lacunae<br />

in the prevailing system of volume<br />

estimation for quantum of outsourcing<br />

work, framing a proposal, preparation<br />

of tender documents and initiation of<br />

a note sheet for a specific purpose<br />

was observed. D(T/O) was advised<br />

to take certain system improvement<br />

on the lacuna and to issue appropriate<br />

guideline/ circular in the matter.<br />

19. While investigating a case of theft of<br />

diesel at a Colliery of <strong>CCL</strong>, following<br />

lacunae were detected which are<br />

listed below:<br />

Overwriting in the numerical<br />

ledger for maintaining the<br />

records of the book stock of<br />

diesel was found and the same<br />

was not regularly signed by the<br />

Store Personnel.<br />

Key of the panel for controlling<br />

flow of the diesel through flow<br />

meter was found in the<br />

possession of the POL issuer,<br />

which was found to be<br />

improper. The said key is a very<br />

important item and should<br />

always be in the safe custody<br />

of Depot Officer or other higher<br />

official.<br />

Flouting of directives of the<br />

D(T/O) conveyed vide Circular<br />

dated 19.1.2004 in as much as<br />

the authorized person did not<br />

examine, analyse and review
