Overview<br />
While most <strong>St</strong>. <strong>Patrick</strong> <strong>Center</strong> clients come to the Partnership <strong>Center</strong> to seek help, many<br />
chronically homeless people remain on the streets. The mental health department employs two<br />
full-time Mobile Outreach counselors who go out into the community in a van or on bikes to<br />
develop relationships with people who may need our services, while respecting the right of<br />
individuals to make their own choices.<br />
There are no eligibility criteria for Mobile Outreach. Mobile Outreach attempts to individualize its<br />
services to each client, and consequently, the program does not offer special services to a specific<br />
population. Services provided by Mobile Outreach include outreach, referral to service providers,<br />
emergency food and clothing and transportation to shelters or hospitals in an emergency. Mobile<br />
Outreach counselors may also act as a liaison between service providers and hard-to-reach<br />
clients.<br />
<strong>St</strong>. <strong>Patrick</strong> <strong>Center</strong>’s Mobile Outreach Team, other agencies’ mobile outreach teams and various <strong>St</strong>.<br />
Louis social workers meet monthly to discuss clients. These partners include Youth in Need,<br />
Covenant House, Horizon Club (a division of DD Resources), Catholic Charities, <strong>St</strong>. John’s Mercy,<br />
BJC, New Life Evangelic <strong>Center</strong> and The Bridge. Together, these providers discuss and evaluate<br />
the best means to handle clients who are resistant to accessing services.<br />
The major funding streams for Mobile Outreach come from a Missouri Department of Mental <strong>Health</strong><br />
PATH grant, in-kind donations and private donations.<br />
Related <strong>Program</strong>s<br />
Mobile Outreach clients may use the resources provided by our Shamrock Club day program,<br />
which offers all the physical needs homeless persons may require, including a washer and dryer,<br />
showers, hot meals, a mailing address and respite from the elements. If clients choose to enroll in<br />
Intensive Case Management, they can access the Shamrock Club’s individual counseling, group<br />
education, group therapy, community resources, housing support, job training opportunities, and<br />
referrals to other <strong>St</strong>. <strong>Patrick</strong> <strong>Center</strong> programs and partners.<br />

At <strong>St</strong>. <strong>Patrick</strong> <strong>Center</strong>, we’re Missouri’s largest provider of<br />
housing, employment and health opportunities for people<br />
who are homeless or at risk of becoming homeless. We take<br />
an action-based approach to service and offer nationally<br />
recognized programs that assist more than 9,000 people<br />
each year.<br />
Our mission, at its core, is to help individuals and families<br />
move from homelessness to independence in dignified,<br />
cost-effective and permanent ways. This approach transforms<br />
our clients as well as our entire community.<br />

Funding for the program comes from various sources including from the <strong>St</strong>ate of Missouri from<br />
general revenue, PATH funding, and various grant funders that vary from year to year.<br />
Providers<br />
<strong>St</strong>. <strong>Patrick</strong> <strong>Center</strong> has many service provider partners in the building. Logan Chiropractic College<br />
offers free services to clients. Grace Hill <strong>Health</strong> <strong>Center</strong>s, Inc. offer dental services to clients once<br />
per month. Several times per year, clients are offered free eye care through a university-sponsored<br />
mobile eye clinic. Clients are also offered health literacy classes with a nurse who is contracted<br />
through <strong>St</strong>aff Link.<br />
<strong>Program</strong> Outcomes<br />
Two full-time counselors, using bikes or vans, developed trusting relationships with 104 people<br />
who are homeless, mentally ill and living on the streets in fiscal year 2012. We engaged 63<br />
individuals in agency programs and services.<br />
