Step 5 — Establish a system to monitor the indicators over time
During this step, a system for monitoring the previously selected indicators should be
established. This helps to ensure that continuous health status monitoring occurs and
establishes baseline data upon which future trends can be identified. This system will
also be instrumental in identifying the results of the MAPP process and evaluating the
success of MAPP activities.

• The need to modify or add indicators.
• Methods for maintaining the data systems.
