My Account – Upload Files

The Upload Files link in My Account is intended only for

uploading supporting photos, pictures and instructions for

a particular case, CEREC or implant treatment digital Rx.

If you have 3Shape files, please submit the scan or design

files through 3Shape via the current 3Shape process.

I. Patient Images

Patient Images is used for uploading files for a particular case if a case number is

available, or for a particular patient if a case number is not available. The following

file formats are allowed for upload:

■ .jpeg

■ .tiff

■ .png

■ .doc

■ .pdf

■ .jpg

■ .gif

■ .bmp

■ .docx

■ .txt

Follow the below step to submit files.

1 Login to My Account with the proper user name and password and click on

the Upload Files link. The page will refresh to reveal the Submit Digital Images

page.

2 If a case number is available, click on the Yes radio button. This will enable

the Case Number field to allow case number entry.

3 Enter the desired case number and click the VALIDATE button. My Account

will find the case and populate the patient’s first name and last name.

4 If the case number is unknown, select the No radio button and enter the

patient’s first name and last name.

5 Click on the BROWSE FILES button and a window pops up allowing you

to select and open files.

6 Navigate to the desired path, highlight one or more files and click on the

Open button. The page will now display the files attached.

7 To remove a file, click on the X icon.

8 Click on the Cancel all Uploads button if the attached files are not the

desired files.

9 Once the desired files are attached, enter any comments and click on the

SUBMIT button.

10 The files will begin to upload. You will see a check mark for files that have

already finished upload, and a progress bar and a circular progress icon

next to the file name for the file currently uploading.

11 If the file being uploaded is the wrong file, click on the Cancel button below

the progress bar. The progress bar will close and a warning icon will appear

next to the attached file to indicate the upload was canceled.

12 Once all uploads are finished, you will see a confirmation message. The

Glidewell team will be notified of the upload. Click on the UPLOAD NEXT

PATIENT button to upload files for other cases or patients.

13 The below e-mail will indicate completed uploading of the digital file.

II.CEREC Files

CEREC Upload is intended only for CEREC cases. Only file formats .sdt and .cdt

are allowed for CEREC upload. Information will be collected in the online form

in five steps. Under UPLOAD FILES, select CEREC Files to get to the CEREC

upload page. Below are the steps to upload CEREC files.

1 The first three steps are included on the first screen. Step 1 is patient

information, Step 2 is restoration selection and Step 3 is additional restoration

information.

2 Enter the patient’s first name, last name, gender and age under patient

information.

3 In the restoration selection, select the desired restoration radio button and

then select the desired tooth. If a particular tooth needs to be deselected,

simply click on that tooth again. To deselect all teeth, click on the CLEAR

ALL TEETH button.

4 For each tooth selected, additional restoration information is needed. Make

the appropriate selection(s).

5 If all selections are correct, click on the NEXT button to proceed. If you

need to start over, click on the START OVER button to deselect all options

and blank out all fields.

6 The second page to the CEREC upload contains Steps 4 and 5 of information

gathering. In Step 4, enter any special instructions.

7 Select the party submitting the Rx. If the Rx is submitted by the prescribing

doctor’s office, the signature, license number and delivery date must be

entered to proceed.

If the Rx is being submitted by a laboratory, select the Laboratory radio

button and check the box to proceed.

8 If all entered information is accurate, click on the NEXT button to proceed.

The BACK button can be used to review the settings from the previous

page. To start fresh, click on the START OVER button.

9 The next screen will display the case summary. To review the previous

screens, click on the BACK button. To start from the beginning, click on

the START OVER button. Click on the FINISH button to proceed to the

upload page.

10 The SUBMIT CEREC FILES screen appears. The patient’s first and last

name fields are populated.

11 Click on the BROWSE FILES button and a window opens allowing you to

select and open files. Navigate to the desired path and select the files to

be uploaded. CEREC only allows file formats of .sdt, .cdt and .rdt. Highlight

the desired file(s) and click on the Open button to attach the file(s).

12 The attached file(s) will display on the Upload Images screen. To remove a

file, click on the icon. To remove all files, click on the Cancel all Uploads

button. To begin file upload, click on the SUBMIT button.

13 The files will begin to upload. To cancel a particular file upload, click on

the Cancel button during the upload of the file. If there are more files to

upload, once a file upload is canceled, the next file will start to upload.

14 Once the upload is finished, a message will display noting that files have

been submitted for processing. If you need to upload another patient, click

on the UPLOAD NEXT PATIENT button.

15 An e-mail will be sent to the e-mail address on the login profile and the subject

line will read “Glidewell Laboratories – CEREC Rx Received.”

III. Implant Treatment Digital Rx

1 Select Implant Treatment Digital Rx under UPLOAD FILES to open the Submit

Implant Treatment Case page.

2 Enter the patient’s first and last name. Select either NobelGuide or Sim-

Plant. NobelGuide packages or SimPlant packages will appear based on

the choice of system made.

3 Select the desired package.

4 If a pathology report is needed, check the Report checkbox. Be sure to fill

in the gender, age and race. Click the NEXT button to proceed.

5 On the tooth graphic, select the teeth where you intend to place the

implant(s). Select a system from the drop down menu and fill out the intended

restoration delivery and type.

6 Enter a description of the implant plan in the Treatment Plan text box. Type

name for signature and enter the license number.

7 Decide how the DICOM files will be sent. If From the Radiology Laboratory

is selected, please enter the radiologist contact name and radiologist

phone number. Make a selection and click the NEXT button to proceed.

Click the BACK button to review the previous page. To start from the beginning,

click on the START OVER button.

8 This page provides a summary of the implant case. Review the information

to ensure its accuracy. Click the FINISH button to proceed to upload the file.

Click the BACK button to review previous pages. Click the START OVER

button to start from the beginning.

9 On the DICOM Submission page, look over the pre-populated information

to make sure the area of interest, patient first and last name, gender, age,

and race are correct. Make sure the Case ID is the same as the one on top

of the page.

10 Click on the Choose Folder/File button to open the Choose a file/folder

(Attach limit: 10) window. Navigate to the desired folder, select the intended

files or folder and click on the Attach button.

11 The page now displays the file(s) attached.

12 Click on [remove] next to the file name to remove the attachment.

13 Click on the Send button to start the upload.

14 The progress bar will display. During the upload, if the Pause button is

pressed, the upload for the current file will restart even if close to being done.

15 If the Cancel button is pressed, the below warning will pop up for each attached

file. If No is selected, the current file being uploaded will be deleted.

Once all files are deleted, files will need to be re-attached. If Yes is selected,

the pop up will close and the upload page is displayed with a Resume button.

Resume will restart the file upload.

16 Once the upload is finished, the below message displays:

17 An e-mail will be sent out to inform that the digital Rx was received. The subject

to the e-mail will read “Glidewell Laboratories – Digital Rx Received.”
