PURPOSE OF YOUR JOB POSITION:

The primary purpose ofyour job position is to plan, organize, develop, and direct the

overall operation ofthe Social Services Department in accordance with current federal,

state, and local standards, guidelines and regulations, our established policies and

procedures, and as may be directed by the Administrator, to assure that the medically

related emotional and social needs ofthe resident are met/maintained on an individual

basis.

DELEGATION OF AUTHORITY:

As Social Services Director/Admissions Coordinator, you are delegated the

administrative authority, responsibility, and accountability necessary for carrying out

your assigned duties.

DUTIES AND RESPONSIBILITIES

SOCIAL SERVICES ADMINISTRATIVE FUNCTIONS:

Plan, develop, organize, implement, evaluate, and direct the social service programs of

this facility. Programs must meet federal, state, and corporate guidelines.

Assist in the development, administering, and coordinating ofdepartment policies and

procedures.

Keep abreast of current federal and state regulations, as well as professional standards,

and make recommendations on changes in policies and procedures to the Administrator.

Review department policies and procedures, at least annually, and participate in making

recommended changes.

Develop and implement policies and procedures for the identification of medically

related social and emotional needs ofthe resident.

Participate in community planning related to the interests ofthe facility and the services

and needs ofthe resident and family.

Will be a part ofthe pre-admission and admission process.

Maintain appropriate social service records, including a social history, social assessment,

discharge plan, and progress notes with follow-up on social service needs identified on

the Plan ofCare. Periodic updates are required for each of the~e records, except the

social history.

Interview resident/families as necessary and in a private setting.

Orient the resident and family to the nursing home environment and assist in the

adjustment ofboth through individual and group programs.

Will actively participate in Interdisciplinary Resident Care Plan Conference. Promote

resident and family participation in Interdisciplinary Resident Care Plan Conferences.

Will identify and promote a supportive family network, including notification (with

resident's permission) to the family when important changes in the resident's health or

well being occur.

Develop a social services plan of care which matches medically related social and

emotional problems and needs with realistic goals and specific actions to be taken.

Assure that resident's personal care needs are met by arranging for shopping and/or

appropriate appointments, (e.g. dental care, eye care, hearing services).

Perform administrative requirements, such as completing necessary forms, reports, etc.,

and submitting such to the Administrator as required.

Refer resident/families to appropriate social service agencies when the facility does not

provide the services or needs ofthe resident.

Provide information to resident/families as to MedicarelMedicaid, and other financial

assistance programs available to the resident. Assist in completing forms and making

arrangements for assistance provided by federal, state, community, and veteran services.

Assist resident in detennining how they would like health care decisions, including the

use of advance directives, ifdesired by the resident. Provide infonnation on DNR orders

to resident and family members.

Provide consultation to members ofour staff, community agencies, etc., in efforts to

solve the needs and problems ofthe resident through the development of social service

programs.

Assist in the review and updating ofdepartmental job descriptions at least annually.

Assume the authority, responsibility, and accountability ofdirecting the social service

department.

Maintain a quality working relationship with the medial profession and other health

related facilities and organizations.

Coordinate social service activities with other departments as necessary.

Work with the facility's consultant as necessary and implement recommended changes as

required.

Delegate authority, responsibility, and accountability to other responsible department

personnel.

Make written and oral reports/recommendations to the Administrator concerning the

operation ofthe social service department.

Interpret the department's policies and procedures to employees, residents, visitors,

government agencies, etc.

Keep abreast of economic conditions/situations and recommend to the Administrator

adjustments in social services that assure the continued ability to provide daily social

services.

Responsible for coordinating family support group. Coordinate all room changes

according to procedure.

Review and develop a plan ofcorrection for social service deficiencies noted during

survey inspections and provide a written copy of such to the Administrator.

Assist the Infection Control Coordinator in identifying, evaluating, and classifying

routine and job related social functions to ensure that tasks involving exposure to blood

or body fluids are properly identified and recorded.

Review departmental complaints and grievances from personnel and make written reports

to the Administrator of action(s) taken. Follow facility's established procedures.

Assist the Quality Assessment & Assurance Committee in developing and implementing

appropriate plans of action to correct identified deficiencies.

Develop, implement, and maintain an ongoing quality assurance program for the social

services department.

Participate in facility surveys (inspections) made by authorized government agencies.

Interview residents, or family members, as necessary, to obtain social history.

Assure that social service progress notes are infonnative and descriptive of the services

provided and ofthe resident's response to the service.

Maintain a reference library ofwritten material, law, etc., necessary for complying with

current standards and regulations, and that will provide assistance in maintaining quality

social service.

Involve the resident/family in planning objectives and goals for the resident.

Meet with administration, medical and nursing staff, as well as other related departments

in planning social service programs and activities.

Maintain high level ofvisibility in the facility; be known to residents and family

members; be available as needed by the facility.

Others as deemed necessary and appropriate, or the Administrator may direct as.

ADMISSIONS ADMINISTRATIVE FUNCTIONS:

Assist in interviewing residents/guardians and obtaining required information and

signatures.

Admit and prepare identification records for residents in accordance with established

policies and procedures.

Maintain an accurate record ofavailable beds.

Admit, transfer, and discharge residents in accordance with established policies and

procedures.

Obtain the resident/guardian's signature on all required permits, releases, authorizations,

etc.

Maintain a current listing ofall residents, their level ofcare, and assigned room numbers.

Provide copies of such listing to nursing service, administrative personnel, government

agencies and personnel, etc. as necessary.

Attach preadmission documentation to admission papers as appropriate.

Collect, assemble, and check admission papers. Forward to appropriate department.

Obtain the resident' assigned room number. Place on all necessary admission papers.

Explain to the resident, and/or guardian, room rates, billing procedures, visitors/guest

privileges, restrictions, etc., as appropriate.

Refer admission problems to proper authority, i.e., office supervisor, credit manager,

nursing service, etc.

Maintain a resident waiting list in accordance with established policies and procedures.

Keep abreast ofcurrent MedicarelMedicaid regulations governing admission/discharge

requirements ofhealth care facilities.

Assume the administrative authority, responsibility, and accountability ofperforming the

assigned duties ofthe Admissions Office.

COMMITTEE FUNCTIONS:

Serve on, participate in, and attend various committees ofthe facility (Le., Infection

Control, Quality Improvement, etc.) as required, and as appointed by the Administrator.

Provide written and/or oral reports ofthe social service programs as required, or as may

be directed by such committee(s).

Participate in regularly scheduled reviews of resident discharge plans.

Evaluate and implement recommendations from established committees as they may

pertain to social services.

Meet with social services personnel, on a regularly scheduled basis, and solicit advice

from inter-department supervisors concerning the operation ofthe social services

department, assist in identifying and correcting problem areas, and/or the improvement of

services.

Attend department head meetings, etc., as scheduled or as may be called.

Schedule and announce departmental meeting times, dates, places, etc.

PERSONNEL FUNCTIONS:

Assist in standardizing the methods in which social services tasks are performed.

Maintain a productive working relationship with other department supervisors and

coordinate social services to assure that daily social services can be performed without

interruption.

Report occupational exposures to blood, bodily fluids, infectious materials, and

hazardous chemicals in accordance with the facility's policies and procedures governing

accidents and incidents.

Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as

well as a calm environment throughout the facility.

STAFF DEVELOPMENT:

Develop and participate in the planning, conducting, scheduling oftimely in-service

training classes that provide instructions on "how to do the job", and ensure well­

educated social services department.

Develop, implement, and maintain an effective orientation program that orients the new

employee to the department, its policies and procedures, and to his/her job position and

duties.

Provide leadership training that includes the administrative and supervisory principles

essential for the social services department.

Attend and participate in continuing educational programs designed to keep you abreast

ofchanges in your profession.

STAFF DEVELOPMENT (continued)

Ensure that all social services personnel attend and participate in annual OSHA and CDC

in-service training programs for hazard communication, TB management, and blood

borne pathogens standard.

Ensure that the department's policy and procedure manual identifies safety precautions

and equipment to use when performing tasks that may result in bodily injury.

Monitor social services personnel to assure that they are following established safety

regulations in the use ofequipment and supplies.

Ensure that social service work areas are maintained in a clean and sanitary manner.

Ensure that all social services personnel participate in and conduct all fire safety and

disaster preparedness drills in a safe and professional manner.

Assist the Infection Control Committee in the development, implementation, and revising

ofwritten aseptic and isolation techniques.

Develop, implement, and maintain a program for monitoring communicable and/or

infectious diseases among the residents and personnel.

Ensure that social services personnel follow established infection control procedures

when isolation precautions become necessary.

Ensure that department personnel follow established procedures governing the use of

labels and MSDSs.

Report missing/illegible labels and MSDSs to the safety officer or other designated

person.

Develop, implement, and maintain a procedure for reporting hazardous conditions or

equipment.

Report any exposure to blood/bodily fluids.

EQUIPMENT AND SUPPLY FUNCTIONS:

Recommend to the Administrator the equipment and supply needs ofthe department.

Place orders for equipment and supplies as necessary or as may be required.

Make periodic rounds to check equipment and to assure that necessary equipment is

available and working properly.

Ensure that all personnel operate social service equipment in a safe manner.

Develop and implement procedures that ensure social service supplies are used in an

efficient manner to avoid waste.

Ensure that MSDSs are on file for hazardous chemicals used in the social services

department.

Develop preliminary and comprehensive assessments ofthe social service needs ofeach

resident.

EQUIPMENT AND SUPPLY FUNCTIONS (continued):

Develop a written plan of care (preliminary and comprehensive) for each resident that

identifies the problems/needs ofthe resident and the goals to be accomplished for each

problem/need identified.

Encourage the resident/family to participate in the development and review of hislher

plan ofcare.

Ensure that all social services personnel are aware ofthe care plan and that care plans are

used in providing daily social services to the resident.

Review social service notes to determine ifthe care plan is being followed. Report

problem areas to the Director ofNursing Service.

Review and revise care plans and assessment as necessary, but at least quarterly.

Develop and maintain a good rapport with all services involved with the care plan to

ensure that a team effort is achieved in developing a comprehensive plan of care.

BUDGET AND PLANNING FUNCTIONS:

Forecast needs ofthe department.

Assist preparing and planning the social services department's budget for equipment,

supplies, and labor and submit to the Administrator for review, recommendations, and

approval.

Make departmental adjustments in order to conform to approved budget, and/or as

dictated by an analysis ofthe monthly operating statement.

RESIDENT RIGHTS:

Will interpret and promote resident's rights and responsibilities by supporting individual

resident needs and preferences, customary routines, concerns and choices under the

guidance and supervision ofthe Administrator.

Maintain the confidentiality of all resident care information.

Knock before entering a resident's room.

Ensure that all social services personnel are knowledgeable ofthe resident's rights and

responsibilities including the right of refusal.

Review complaints and grievances made by the resident and make a written/oral report to

the Administrator indicating what action(s) were taken to resolve the complaint or

grievance. Follow facility's established procedures.

Maintain a written record ofthe resident's complaints and/or grievances that indicates the

action taken to resolve the complaint and the current status ofthe complaint.

Provide residents with information concerning resident rights, living wills, etc. Explain

as necessary.

Participate in resident/group council meetings as requested and provide support services

to such council.

MISCELLANEOUS:

Make quarterly inspections ofall social service functions to assure that quality control

measures are continually maintained.

Be prepared to handle emergencies as they come up (Le. rescheduling work assignments

and work schedules, etc.).

Work with the facility's consultants as necessary and implement recommended changes

as required.

WORKING CONDITIONS:

Work in office areas as well as throughout the facility.

May be required to move intermittently during working hours.

Is subject to frequent interruptions.

Is involved with residents, personnel, visitors, government agencies/personnel, etc., under

all conditions and circumstances.

Is subject to hostile and emotionally upset residents, family members, etc.

Communicates with the medical staff, nursing service, and other department supervisors.

Works beyond normal working hours, on weekends, and in other positions temporarily,

when necessary.

Is subject to callback during emergency conditions (e.g., severe weather, evacuation,

post-disaster, etc.).

Attends and participates in continuing educational programs.

Is subject to injury from falls, burns from equipment, odors, etc., throughout the

workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air

contaminants.

Is subject to exposure to infectious waste, diseases, conditions etc., including TB and the

AIDS and Hepatitis viruses.

Maintains a liaison with other department supervisors to adequately plan for social

services/activities.

May be subject to the handling ofan exposure to hazardous chemicals.

EDUCATION:

Must possess, as a minimum, a high school diploma. We prefer an Associated Degree in

social work or human services field including but not limited to sociology, special

education, rehabilitation, counseling and psychology.

EXPERIENCE:

Must have, as a minimum, one year of supervised social work experience in a health care

setting working directly with individuals.

SPECIFIC REQUIREMENTS:

Must be able to read, write, speak, and understand the English language.

Must possess the ability to make independent decisions when circumstances warrant such

action.

Must possess the ability to deal tactfully with personnel, resident, family members,

visitors, government agencies/personnel and the general public.

Must possess leadership ability and willingness to work harmoniously with and supervise

other personnel.

Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to

handle residents based on whatever maturity level in which they are currently

functioning.

Must possess the ability to seek out new methods and principles and be willing to

incorporate them into existing social services.

Must not pose a threat to the health or safety ofother individuals in the workplace.

PHYSICAL REQUIREMENTS:

(With or Without the Aid of Mechanical Devices)

Must be able to move intermittently throughout the workday.

Must be able to speak and write the English language in an understandable manner.

Must be able to cope with the mental and emotional stress of the position.

Must possess sight/hearing senses or use prosthetics that will enable these senses to

function adequately so that the requirements of this position can be fully met.

Must function independently and have flexibility, personal integrity, and the ability to

work effectively with residents, personnel, and support agencies.

Must meet the general health requirements set forth by the policies ofthis facility, which

include a medical and physical examination.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at

times, hostile people within the facility.

Must be able to push, pull, move, and/or lift a minimum of 50 pounds to a minimum

height of 3 feet and be able to push, pull, move, and/or carry such weight a minimum

distance of3 feet.

May be necessary to assist in the evacuation of residents during emergency situations.
