INSTRUCTIONS
If you:
♦ are a degree-seeking student at Miami University;
♦ plan to attend another university or study abroad program for a semester or
year;
♦ want to apply for federal financial aid to help defray your costs,
academic
follow the steps below to insure timely processing of your financial aid.
STUDENT RESPONSIBILITIES
1. Submit the Free Application for Federal Student Aid (FAFSA) to the federal processor by
February 15.
2. If you will be attending during the summer, submit the Summer Aid to our office by April 1.
Summer applications are available in February.
3. If you will apply for a Federal Direct PLUS Loan, submit the PLUS Loan Request Form to our
office at least 8 weeks prior to the beginning of your program.
4. Complete Section I of the attached Consortium Agreement. Be sure to have your Academic
Dean at Miami complete the Statement of Permission in Section I.
5. Have the financial aid office at the school you will attend complete Section II of the
Consortium Agreement and return it to our office.
6. Provide our office with the address where your Federal Direct Loan or Federal Direct PLUS loan
proceeds are to be mailed. If you will not be easily accessible because of the location of your
program, you should arrange for an appropriate person to have power of attorney to sign your
checks. Make sure your host school knows how to reach you by mail.
7. Contact the Registrar at the school you will attend and arrange to have that office confirm your
registration in writing to our office. We must have this confirmation before we are able to release
any funds to you.
8. After you've completed your program, arrange for the financial aid office at the institution you
attended to send us a Financial Aid Transcript. This is required by federal law.

AD HOC CONSORTIUM AGREEMENT
The student listed above is a degree seeking student at MIAMI UNIVERSITY. However, the student will
be a transient student at during the semester/quarter of
the 19 academic year.
The student wishes to use financial aid to help defray the cost of attendance during this transient term.
To facilitate the registration process, Miami University will consider the student enrolled in an eligible
program of study and will award and disburse financial aid. The host institution agrees to notify Miami
University if the student fails to register or withdraws from classes. Miami University agrees to handle any
refunds and/or repayments to the Title IV programs, resulting from the student's withdrawal from classes,
according to its established policies.
The contents of this agreement are drawn up to comply with all pertinent federal, state and university
regulations, policies and procedures. As a part of this document, both financial aid officers will receive a
written statement from the student's academic dean verifying that the student has permission to register
as a transient student at the host institution and that the student will receive credit toward a degree from
the parent institution.
The financial aid administrators who are officially responsible for the administration of
financial aid at the schools listed must sign below.

♦ want to apply for federal financial aid to help defray your costs,
follow the steps below to insure timely processing of your financial aid.
STUDENT RESPONSIBILITIES
1. Submit the Free Application for Federal Student Aid (FAFSA) to the federal processor at least 10
weeks prior to the beginning of your program at Miami University. Be sure to list Miami
University as a school where you want information sent.
2. Submit the Miami University Summer Aid Application (if applicable) to our office by April 1.
3. If you will apply for the Federal Direct PLUS loan to help defray your summer school expenses,
submit the PLUS Loan Request Form to our office as soon as possible.
3. Complete Section I of the attached Consortium Agreement. Be sure to have your Academic
Dean/Advisor at your home institution complete the Statement of Permission in Section I.
4. Have the financial aid office at your home school complete Section II of the Consortium
Agreement and return it to our office.
5. Have the financial aid office at your home school send us a Financial Aid Transcript. This
is required by federal law.

Miami University will process Pell Grants and applications for Federal Direct Subsidized, Unsubsidized
and PLUS loans for transient students who are charged tuition at Miami University. Miami University
agrees to calculate the student's eligibility for financial aid based on Miami's student expense budget and
enrollment status at this institution. Miami University agrees to handle any refunds and/or repayments to
the Title IV programs, resulting from the student's withdrawal from classes, according to its established
policies.
The contents of this agreement are drawn up to comply with all pertinent federal, state and university
regulations, policies and procedures. In conjunction with this document, both financial aid officers must
