INSTRUCTIONS<br />
If you:<br />
♦ are a degree-seeking student at <strong>Miami</strong> <strong>University</strong>;<br />
♦ plan to attend another university or study abroad program for a semester or<br />
year;<br />
♦ want to apply for federal financial aid to help defray your costs,<br />
academic<br />
follow the steps below to insure timely processing of your financial aid.<br />
STUDENT RESPONSIBILITIES<br />
1. Submit the Free Application for Federal Student Aid (FAFSA) to the federal processor by<br />
February 15.<br />
2. If you will be attending during the summer, submit the Summer Aid to our office by April 1.<br />
Summer applications are available in February.<br />
3. If you will apply for a Federal Direct PLUS Loan, submit the PLUS Loan Request Form to our<br />
office at least 8 weeks prior to the beginning of your program.<br />
4. Complete Section I of the attached <strong>Consortium</strong> <strong>Agreement</strong>. Be sure to have your Academic<br />
Dean at <strong>Miami</strong> complete the Statement of Permission in Section I.<br />
5. Have the financial aid office at the school you will attend complete Section II of the<br />
<strong>Consortium</strong> <strong>Agreement</strong> and return it to our office.<br />
6. Provide our office with the address where your Federal Direct Loan or Federal Direct PLUS loan<br />
proceeds are to be mailed. If you will not be easily accessible because of the location of your<br />
program, you should arrange for an appropriate person to have power of attorney to sign your<br />
checks. Make sure your host school knows how to reach you by mail.<br />
7. Contact the Registrar at the school you will attend and arrange to have that office confirm your<br />
registration in writing to our office. We must have this confirmation before we are able to release<br />
any funds to you.<br />
8. After you've completed your program, arrange for the financial aid office at the institution you<br />
attended to send us a Financial Aid Transcript. This is required by federal law.<br />

The student listed above is a degree seeking student at MIAMI UNIVERSITY. However, the student will<br />
be a transient student at during the semester/quarter of<br />
the 19 academic year.<br />
The student wishes to use financial aid to help defray the cost of attendance during this transient term.<br />
To facilitate the registration process, <strong>Miami</strong> <strong>University</strong> will consider the student enrolled in an eligible<br />
program of study and will award and disburse financial aid. The host institution agrees to notify <strong>Miami</strong><br />
<strong>University</strong> if the student fails to register or withdraws from classes. <strong>Miami</strong> <strong>University</strong> agrees to handle any<br />
refunds and/or repayments to the Title IV programs, resulting from the student's withdrawal from classes,<br />
according to its established policies.<br />
The contents of this agreement are drawn up to comply with all pertinent federal, state and university<br />
regulations, policies and proc<strong>edu</strong>res. As a part of this document, both financial aid officers will receive a<br />
written statement from the student's academic dean verifying that the student has permission to register<br />
as a transient student at the host institution and that the student will receive credit toward a degree from<br />
the parent institution.<br />
