MOM is a non-profit that is leading a community-wide effort to<br />
prevent homelessness and end hunger for people throughout<br />
<strong>Middleton</strong>, West Madison and Cross Plains. Through the support<br />
of business partners, area schools and faith communities, as well as<br />
numerous individuals and service organizations, MOM provides - free<br />
of charge - food, clothing, housing assistance, emergency financial<br />
assistance, and help for seniors.<br />

MOM’s <strong>Legacy</strong> <strong>Society</strong><br />
The <strong>Legacy</strong> <strong>Society</strong> recognizes a special group of donors who have<br />
made a future gift provision in their estate plan for the benefit<br />
of <strong>Middleton</strong> <strong>Outreach</strong> Ministry. <strong>Legacy</strong> <strong>Society</strong> members have<br />
made a commitment to preventing homelessness and ending<br />
hunger by ensuring the future of <strong>Middleton</strong> <strong>Outreach</strong> Ministry<br />
and its programs that provide basic needs such as food, clothing,<br />
housing assistance and other life necessities.<br />
How to Become a Member of<br />
MOM’s <strong>Legacy</strong> <strong>Society</strong><br />
TO JOIN MOM’S LEGACY SOCIETY…<br />
• Make a future gift provision in your will or through a beneficiary<br />
designation on a life insurance policy or retirement plan.<br />
• Notify MOM of the future gift by sending MOM a completed<br />
membership application form.<br />
WHAT ARE THE BENEFITS TO LEGACY SOCIETY MEMBERS?<br />
In addition to knowing that MOM and its program will continue<br />
well into the future, <strong>Legacy</strong> <strong>Society</strong> members will receive…<br />
• Recognition on MOM’s web site and other periodic publications.<br />
• A certificate of membership.<br />
• An acknowledgement from MOM upon notification of an<br />
eligible gift.<br />
Future Gift Options<br />
Typically, <strong>Legacy</strong> <strong>Society</strong> members make future gifts to MOM in<br />
one of three ways…<br />
BEQUESTS. Historically a bequest via a will or revocable<br />
trust is the most popular future gift option used by<br />

donors. Donors like bequests because they are easy to<br />
understand and do not require the donor to part with<br />
assets during his life. This provides donors peace of mind<br />
knowing that their assets are available to satisfy unforeseen<br />
expenses such as medical or nursing home costs.<br />
LIFE INSURANCE. This type of future gift may be<br />
attractive because it affords donors the opportunity<br />

to make a gift of a sizeable face value for a minimal<br />
outlay of cash. A donor may 1) name MOM as the<br />
beneficiary of a life insurance policy or 2) name MOM<br />
as both the owner and beneficiary of a life insurance policy.<br />
Upon a donor’s death, MOM will receive all, or a portion, of<br />
the proceeds from the policy.<br />

RETIREMENT PLANS AND IRAS. This third type of<br />
future gift involves naming an organization as the<br />
beneficiary of a retirement plan or IRA. This option<br />
merely involves obtaining a beneficiary designation<br />
from the retirement plan administrator and naming MOM as<br />
the entire, or partial, beneficiary of the retirement plan assets<br />
upon the owner’s death.<br />

When referring to MOM in a will, beneficiary designation,<br />
etc., please use “<strong>Middleton</strong> <strong>Outreach</strong> Ministry, Inc. or its<br />
successor agency”.<br />
MOM recommends that you consult with your attorney,<br />
financial planner, tax advisor, etc., as you make your estate<br />
plans. MOM does not provide legal, financial or tax advise.
