23 | Creating viable business models for inclusive growth through the National Optical Fiber Network • Revenue stream: The VLE charges patients per consultation. The CSC and the private provider get their pre-determined percentage shares per consultation • Government support: Same as model 1 • Benefits (over and above those in model 1): –– Government: Strengthening of local entrepreneurship, rural employment generation –– VLE: Expansion of business –– Private healthcare service provider: Additional revenue • Critical success factors (over and above those in model 1): –– Demand generation for the other services sold through CSCs –– VLE’s trust-level with the local populace Apart from the telemedicine models described above, healthcare delivery can be improved by giving the rural population access to a number of services through applications that can run on low-cost feature phones. Such applications, developed in vernacular languages, can help the rural population access relevant content and also periodic health-related alerts from the Government. Recommendations • Government to provide soft support: –– Frame telemedicine laws and regulations –– Incentivize VLEs and CSAs to offer telemedicine by providing special concessions in the CSC scheme – e.g. free/subsidized NOFN connectivity for the CSC –– Ensure acceptability of prescriptions issued by the private healthcare providers through telemedicine at Government pharmacies • Viability gap funding by government. Channeling funds from existing public health schemes could be considered. Integrated planning for the various government healthcare schemes could be explored to ensure optimal allocation of funds towards telemedicine • Involvement of the Panchayats and local leaders: Acceptance of distance treatment is a key success factor in telemedicine. Rural patients are often hesitant in receiving medical advice from a doctor whom they can see only on a screen. This hesitation may be overcome by reassurances from opinion leaders such as Panchayat workers and local leaders • Private healthcare providers could collaborate with the insurance sector. Customized products could be designed around telemedicine minimizing the cost for patients Source: KPMG analysis

Creating viable business models for inclusive growth through the National Optical Fiber Network | 24 Business Models Banking Nearly 70 percent of the Indian population resides in rural areas 35 ; however, approximately 61 percent of the rural population remains unbanked 36 . The banking network in the country has not yet been able to penetrate into many parts of rural India. While close to 31 percent of villages (about 200,000 villages) have at least one branch of a commercial bank 37 , ~46 percent of the rural households still do not avail of banking services 38 . Financial inclusion is a major policy objective for the Indian Government. For achieving this, the Reserve Bank of India (RBI) has adopted a bank-led model that seeks to leverage technology. Some concrete steps in this direction include: • In 2006, banks were permitted to utilize the services of Non-Governmental Organizations (NGOs), micro-finance institutions (other than Non-Banking Financial Companies) and other civil society organizations as intermediaries in providing financial and banking services through the use of business facilitator and business correspondent (BC) models. The BC model allows banks to do ‘cash in - cash out’ transactions at a location much closer to rural habitation, thus addressing the last mile problem associated with branch banking. • In 2009, banks were advised to prepare a roadmap for providing banking services in every village with a population of more than 2,000 by March 2012. Banks could include ICT-enabled models (including deploying BCs) along with brick and mortar structures 39 .
