(c) Annual Schedule of Scholarship Grant Processing of Application Form beginning SY: 2008-2009 (4yrs of secondary education) DATE ACTIVITY RECIPIENT June to Aug., Distribution of Scholarship 2007 Application Form for June to Aug., 2008 June to Aug., 2009 SY: 2008-2009 to 2012 (4yrs) Distribution of Scholarship Application Form for SY: 2009-2010 to 2013 (4yrs) Distribution of Scholarship Application Form for SY: 2010-2011 to 2014 (4yrs) 2 nd Awardee, 1 st Cycle of Phase1: Benjamin-Victoria Manlulu HS Scholarship Grant 3 rd Awardee, 1 st Cycle of Phase1: Benjamin-Victoria Manlulu HS Scholarship Grant 4 th Awardee, 1 st Cycle of Phase1: Benjamin-Victoria Manlulu HS Scholarship Grant (d) When Phase 1 Scholar shall have graduated from high school, s/he has to renew his/her CONTRACT before moving on to the Phase 2: Benjamin-Victoria Manlulu College Scholarship Grant. (e) When all 4 Awardees (of the 1 st Cycle) shall have finished their Phases 1 & 2 education in 8 - 11 years, the FIRST CYCLE of the Scholarship Program shall have been then completed. (f) Simultaneously, SECOND CYCLE of the Scholarship Program shall phase in and begins another CYCLE as the 2 nd , 3 rd and so on… with the 1 st Awardee (2 nd Cycle of the Scholarship Program) who shall have to follow the flow of the 1 st Cycle of the Scholarship Program Implementation. This can be graphically viewed better by looking at the Projected Needs of Scholars 4 of those in Calantas. 4) Scholarship Benefits a) Benefits shall be assessed based on the actual needs and cost per Art. 2, s3, ss3.1b of the CYDFI Handbook on Beneficiary and Benefits of the Scholarship Program being implemented in Calantas which describes, among other things, the following: “Pending availability of funds, recipients of full scholarship will be the 6th grade First Honor student from the graduating class. The APPDHSSG only guarantees the high school education of the recipient which includes a budget on: (1) books and school supplies (2) sets of school uniforms & shoes (3) transportation and personal monthly allowance (4) Payment of Tuition Fee, if the Scholar couldn’t maintain academic standing to continuously qualify him for a free tuition fee in the school, provided s/he meets the Criteria for Maintaining Scholarship. (5) Incentives for “unqualified” school and community service 4 Please, see attached Projected Needs Model of those Scholars in Calantas on page 8 4

work which means that a scholar voluntarily participates in school and community works beyond his normal daily routine of doing things. 5) Criteria for Maintaining the CYDFI-Bo. Benedicto Scholarship Program shall be based on the CYDFI Handbook per Art. 3, s2. ss2.1a. NO grade below 80 in both academic and non-academic subjects to maintain an average grade of 80. ss2.1b. Maintain good behavior during the scholarship period, meaning: “NO Alcohol, Tobacco and Other Drugs (No ATOD Use/Abuse)” ss2.1c. Engage in community work in Bo. Benedicto with school children during SUMMER BREAK and create activities to encourage social awareness and responsibilities to self, others, family and community. ss2.1d. Phase 1 High School Scholars will meet with the CEO/President every after periodic grades are released for updates and evaluation of academic plans and submission of grades’ copy. ss2.1e. Phase 2 College Scholars will meet with CEO/President after semestral grades are released for updates and evaluation of academic plans and submission of grades’ copy. ss2.1f. In addition, CYDFI Scholars, together with one of the respective parents and/or guardian or adult family member, shall attend the Annual Board Meeting in Calantas. 6) Termination of Scholarship. Per Art. 3, s4 of the CYDFI Handbook, Scholarship can be terminated in two ways: a) Voluntary Termination: (1) A Scholar has the right to withdraw from the Scholarship by writing to the CEO/President and the latter has to convene the Executive Committee to conduct a Hearing to determine the CAUSE of why a “voluntary termination” is being sought. (2) Once the “hearing” is over and a final written recommendation has been noted by the Chairman and approved by the CYDFI Founders, the scholar would lose effectively and immediately all the rights and privileges to be in the Scholarship Program EXCEPT to do some volunteer service works at the Foundation. b) Involuntary Termination. For Phases 1 & 2 Scholars: (1) A Scholar will be terminated at once from the CYDFISP if both sections of Art. 3, ss2.1a and ss2.1b of CYDFI Handbook are violated and if either section violated is a year probation. 5
