Please be advised that a number of product codes of <strong>SYLVANIA</strong> <strong>8W</strong> <strong>LED</strong> lamps, are being recalled in<br />
cooperation with the U.S. Consumer Product Safety Commission (see CPSC website for press release<br />
describing recall at CPSC Recall for more information). In cooperation with INSERT OPCO NAME, we<br />
have identified that you have purchased lamps that are potentially affected by this recall. We wish to<br />
extend an offer to you to receive new products if you do in fact have affected types. Below are images,<br />

The affected lamps were manufactured between October 4, 2010 and March 18, 2011. They will bear the<br />
following date codes:<br />

To check the date code on the lamps you have please check the base of the lamp to determine if the<br />
printing on the base include any of the above mentioned codes. Images below show were to find the<br />
code on the lamp.<br />

Please immediately examine any lamps in your facility and determine if this product is in your possession.<br />
If you find lamps with the affected date code, stop using them immediately and contact OSRAM<br />
<strong>SYLVANIA</strong> toll free at 1-877-423-3772. Do not destroy any affected lamps as we are legally obligated to<br />
receive them back and provide proof of destruction to the CPSC.<br />
