The Basics: Eligibility, Whom You Can Cover,
Enrolling and Costs
Eligibility for Benefits
To be eligible for coverage under our benefit plans, you must be:
• A regular full-time employee or a buffer employee or regularly scheduled
to work at least 30 hours per week.
Whom You Can Cover on Your Pitney Bowes Plans
Who you cover
determines
your costs
• Your spouse or eligible domestic partner
• Your children up to age 26, regardless of student or marital status (except for
Life Insurance)
• Your children of any age who are incapable of self-sustaining employment due to
a behavioral or physical disability, as long as the incapacity started when the child
was otherwise eligible for coverage
For complete details on whom you can cover, visit www.pbprojectliving.com >
U.S. Benefits > Using Your Benefits.
To cover your dependents, they must be enrolled in the same medical, dental
and vision plan you choose for yourself. But you don’t need to cover the same
dependents for medical, dental and vision. For example, you may choose to cover
yourself, your spouse and children for medical but only yourself and your children
for dental and vision.
When you enroll your dependents for the first time, you must provide proof that they
are eligible for coverage. See page 26 for more details.
When You Can Enroll
Deadlines apply,
don’t miss them
You can enroll in or make changes to your benefits during these enrollment windows:
During Annual Enrollment each fall. This is the annual time you can change your benefit
plans and add or drop covered dependents.
When your life changes. Certain events such as getting married or having a child
allow you to make changes to your benefits. Make allowable changes within 30 days
of the event. If you miss this deadline, you can’t make changes until the next Annual
Enrollment period. For more information about other actions to consider during a life
change, visit www.pbprojectliving.com > Using Your Benefits > Benefits & Your Life.
4
As a new employee. If you are in PBMS, Mail Services, Level One or certain Customer
Ops, you are eligible for benefits on the first of the month after you reach 90 days
of service. For all other business units, you are eligible on the first of the month
following your date of hire. You must enroll before the 15th of the month in which
you became eligible.
How You and Pitney Bowes Share Costs for Benefits
Pitney Bowes provides some benefits at no cost to you. For others, you and Pitney
Bowes share the cost. And there are some plans where you pay the entire cost. Your
cost will be deducted from your paycheck twice a month during the calendar year.
Take a look at how your paycheck deductions are calculated:
Covering a working spouse or domestic partner? Enrolling a working spouse or
domestic partner in your Pitney Bowes medical and dental coverage when they can
obtain coverage through their own employer means you’ll pay more annually—$1,500
for medical coverage and $150 for dental coverage. This doesn’t apply if you both
work for Pitney Bowes.
Pay less for medical coverage. Complete the short, online health assessment through
your medical plan by November 30, 2012, and pay less for medical coverage in 2013—
$100 (you) or $200 (you + your spouse/domestic partner). This health assessment
credit is part of the PB Healthy Rewards Program.
Employees who are first eligible for benefits on or after January 1, 2013, are not
eligible for the 2013 health assessment credit, but you still need to complete the
assessment through your medical plan after you enroll in coverage to earn incentives
in 2013.
Medical Plan Options
All of our medical plans provide you with preventive care and comprehensive
coverage to protect you in case of a serious accident or illness. All plans include
prescription drug and behavioral health coverage. The key differences are how you
pay for services during the year and the plan administrator. There are three types of
medical plans available.
Medical Plan Options: Which Plan Is Right for You?
Health Fund
Plan = lower
paycheck
deductions +
help paying your
deductible
Health Fund Plan You pay lower paycheck deductions and a higher deductible,
shifting costs to when you actually use care. You can choose the Health
Reimbursement Account or the Health Savings Account to help you pay for health
care expenses. Pitney Bowes contributes money to the account, that you can use to
help pay for the deductible.
PPO Plan You pay a higher fixed cost through paycheck deductions regardless of how
much care you use. You can choose the Health Care Account to help you pay for health
care expenses.
HMO Plan You pay a higher fixed cost through paycheck deductions regardless of
how much care you use. Coverage is limited to inside the HMO network (except in
certain emergencies). The HMO option is available only in certain states.* You can
choose the Health Care Account to help you pay for health care expenses.
*If you live in certain parts of CA, CO, GA, HI, OR, WA or the Mid-Atlantic area, you have an HMO option with Kaiser and/or
Group Health Cooperative.
If you live outside the service area for Cigna, UHC, and an HMO, your medical plan option is the Out-of-Area Plan. You will
receive separate details on the plan or you can visit www.pbprojectliving.com.
Take time to consider your needs, your budget and what’s most important to you—like
monthly paycheck deductions, flexibility in providers and out-of-pocket costs—before
you choose your medical plan.
Health Accounts
A Health Account can be used to cover eligible expenses, such as deductibles,
coinsurance and copayments with tax-free dollars. Your medical plan choice
determines the Health Account(s) that can be paired with it. How the accounts
work and the advantages of each vary, so it’s important you understand the features.
you won’t have to pay anything out of your pocket until the account reaches $0.
How the Health Accounts Work
with Your Medical Plan
When you need care—doctor’s visits, prescriptions, and other medical services like
surgery or a stay in the hospital—this is how your Health Account works with the plan
to help you cover expenses.
PREVENTIVE CARE
In-network preventive care is always 100% covered.
DEDUCTIBLE
Pay health care
costs until you
reach your
deductible.
COST SHARING
After you meet the
deductible, you pay
your share of
in-network medical
and Rx bills. The
plan pays the rest.
FULL COVERAGE
If your expenses
meet your
out-of-pocket
maximum the
plan covers 100%.
HEALTH ACCOUNTS
Each Health Account works differently in how you use the account to pay for your
deductible and your portion of the cost sharing:
Choosing the Health Fund Plan?
• Health Reimbursement Account (HRA) – Pitney Bowes contributes to the account
and funds are available the first day your benefits become effective (new hires)
or January 1 (current employees). This account is linked directly to your medical
plan, and your costs are automatically deducted from the account when your
claim is processed until you reach a $0 balance. You can increase your HRA
balance by earning the full PB Healthy Rewards incentive.
• Health Savings Account (HSA) – Both you and Pitney Bowes contribute to the
account. Your pretax contributions and Pitney Bowes’ contributions are made per
paycheck. Pay for medical expenses with your debit card or pay out of pocket and
reimburse yourself from your account. There are two easy ways you can boost
your HSA balance: earn the full PB Healthy Rewards incentive and contribute
enough to get the full company match.
Choosing the PPO or HMO Plans?
• Health Care Account (HCA) – This account is funded completely by you. Pay for
medical expenses with your YSA debit card or pay out of pocket and submit a
Choosing the Health Savings Account (HSA)?
Choose one of these banks to get the Pitney Bowes match:
• JPMorgan Chase Bank (Cigna)
• OptumHealth Bank (UHC)
• Bank of America (Cigna and UHC)
Don’t miss
this step
If you enroll in an HSA but don’t select a bank, an account will automatically
be opened for you with Bank of America. You can change your bank at any time.
See more details about the features and advantages of each of the banks on
www.pbprojectliving.com.
Want to use your own bank?
You can, but you should be aware that Pitney Bowes will not match your HSA
contributions. You must fund the account with after-tax contributions and get your
tax savings when you file your tax returns. You’ll also pay monthly maintenance fees
(covered by Pitney Bowes if you choose paycheck deductions with one of the three
banks listed above).
Set an annual savings goal
When you enroll in the HSA, you need to set an annual savings goal. If you don’t enter
a goal, one will be set for you—$1,000 (you only coverage) or $2,000 (all other types
of coverage)—so that you receive the full company match. Keep in mind when setting
your goal that you can’t exceed the annual IRS maximum. Any PB Healthy Rewards
incentives you earn and the Pitney Bowes match count toward the maximum, and
your annual savings goal may be adjusted accordingly.
Don’t miss
this step
Watch your balance
You must have adequate funds in your account to pay for expenses. Don’t have
enough? Hang onto your receipts and reimburse yourself when you do.
Limited Use Health Care Account
Interested in additional tax savings? If you enroll in the Health Fund, you may also
be able to enroll in a limited use Health Care Account (HCA) to save additional pretax
dollars for certain eligible expenses after you meet your annual deductible.
To qualify, you can’t be enrolled in your spouse or domestic partner’s flexible
Spending/Health Care Account. Learn more about the limited-use HCA at
www.pbprojectliving.com > U.S Benefits > Financial.
Save tax-free for dependent care expenses too. See page 22 for details >
Life, Accident, Disability and Critical Illness Insurance
If the unthinkable happens—like a sudden illness or death—you can protect yourself
financially with Life, Accident, Disability and Critical Illness Insurance coverage.
FREE
Basic Coverage
for all these
plans is free
Basic Coverage
Your Pitney Bowes Flex$ cover basic coverage under these plans at no cost to you:
Basic Life Insurance
Pays a benefit if you die while employed by Pitney Bowes equal to 1 times your annual
base pay.
Business Travel Accident Insurance
Pays a benefit if you have an accident or die while traveling on approved company
business equal to 10 times your annual earnings, with a minimum benefit of $250,000
and a maximum benefit of $2 million.
Short-Term Disability (STD)
Replaces part of your income if you have to temporarily be away from work due to an
injury or illness. Benefits begin after a set waiting period. Before you can receive a
benefit, you must use some of your own time off first. The specific details of your
STD Plan depend on which business unit you are in: Level One, Call Center, PBMS
and PBGS employees are not eligible for STD benefits during your first year of service;
PBI employees are eligible for STD payments up to 26 days, if approved, during your
first year of service. After 21 weeks, this coverage either ends or your long-term
disability (LTD) coverage begins.
Long-Term Disability (LTD)
Replaces 50% of your income if you are unable to work for a long period of time due
to a non-work-related illness or injury. You determine if you want coverage paid with
pretax or after-tax dollars.
• Coverage paid with pretax dollars? You’ll pay taxes on the amount of your benefit,
reducing the amount you receive.
• Coverage paid with after-tax dollars? You pay taxes on the cost of coverage, which
will be lower than the taxes on your benefit.
Do you need
more than basic
coverage?
20
Supplemental Coverage
For additional coverage, you can purchase the following:
Supplemental Life Insurance
For you: 1–5 times your pay; up to a maximum of $2 million.
For your spouse/domestic partner: Up to $250,000 in $10,000 increments.
For your children: $10,000 or $20,000
Supplemental Accidental Death and Dismemberment (AD&D) Insurance
Accidental Death and Dismemberment (AD&D) Insurance protects your family financially
if you or a covered dependent dies or loses a limb, eye or hearing in an accident.
For you: 1–5 times your pay; up to a maximum of $2 million
For your spouse/domestic partner: Up to $250,000 in $10,000 increments
For your children: $10,000 or $20,000
Choosing Supplemental Life Insurance or AD&D?
You may be required to submit proof of good health before your new coverage
becomes effective. This process can involve urine and blood tests (screenings),
along with a physical exam, to be scheduled by a physician.
Supplemental Long-Term Disability
Increase the 50% income replacement you already receive through basic LTD
to 66 2 /3% of your monthly pay. You can choose to pay for coverage with pretax
or after-tax dollars. Supplemental LTD is not available to Buffer employees.
Critical Illness Insurance
Critical Illness Insurance through MetLife complements your medical and disability
income coverage. If you or a covered family member becomes critically ill, you receive
a lump-sum benefit that you can use for anything—medical expenses, household
expenses, rent/mortgage payments, child care and more. You can elect Critical Illness
Insurance if you’re eligible for benefits, but you do not have to be in a Pitney Bowes
medical plan. There’s no waiting period for coverage to begin.
Coverage is optional and you pay the full cost. You can purchase coverage for yourself,
your spouse or domestic partner and your child(ren) at a level of $10,000 each.
Do you want
financial help
if you or a
family member
becomes
seriously ill?
Benefits are paid to each covered person as a percentage of the amount of coverage
you elect up to the $30,000 maximum. You receive payment for the first incident of an
illness. And for certain conditions (heart attack, coronary artery bypass graft surgery
and cancer), if you face the illness again, you can receive a 50% reoccurrence benefit.
There are also certain conditions such as cystic fibrosis and multiple sclerosis for
which each covered person can receive a one-time benefit of 25%. And, if you receive
a major organ transplant, you will receive 100% of your elected benefit amount.
Critical Illness Insurance benefits are subject to lifetime maximum—$30,000 total
benefit amount plus the $10,000 major organ transplant benefit. Once you reach the
lifetime maximum, your coverage will end.
Guaranteed issue
No proof of good health is required when you purchase coverage for 2013.
See www.pbprojectliving.com > U.S. Benefits > Financial for more details about
Critical Illness Insurance.
important financial decision you make.
help you prepare for a strong financial
future. And, after one year of service,
Pitney Bowes helps you build your
savings with matching contributions.
Annual Enrollment is a good time to take
stock of your retirement savings efforts
and evaluate your long-term goals.
Ask yourself:
Am I saving enough?
The sooner you start saving for
retirement, the more prepared you will
be when you want to stop working!
Can I save a little more?
Take advantage of the auto-increase
feature of your plan.
Is my money invested the way
I’d like it to be?
Depending on your age and your
goals, you may want to change your
investment strategy.
Are my beneficiary
designations up to date?
Be sure that your balance goes where
you want it to if you should die.
24
Highlights:
• You’re eligible to start saving in the 401(k) your first day on the job. You will be automatically
from another company-sponsored plan can be rolled over into the Pitney Bowes plan.
• You choose how much to contribute on a pretax, after-tax, or Roth basis up to the IRS limits.
Use convenient payroll deductions to make your contributions.
Don’t Forget …
Enroll by the deadline. If you don’t
take action, you’ll automatically be
enrolled in “default” coverage—and be
responsible for the costs—that may not
meet your needs and budget.
• If you are already enrolled in
Pitney Bowes benefits coverage and
you do not take action during Annual
Enrollment, you will remain in your
Account (DCA), Group Legal and Flex
Days for 2013.
• If you are newly eligible for benefits
and do not enroll by the deadline,
you will automatically be enrolled
in the Cigna Health Fund Plan
with HRA, Delta Dental Preventive
Plan, Basic Life Insurance and
Basic Long-Term Disability. You
will not participate in any other
benefit programs that require
your enrollment: Health Care
Account (HCA) or Dependent Care
Account (DCA), Supplemental
Life Insurance, Critical Illness
Insurance, HealthAllies Discounts,
Voluntary Vision, Accidental Death
& Dismemberment (AD&D), Group
Legal, or Flex Days.
Verify your dependents. When enrolling,
you must verify that your dependents
are eligible for coverage. If you don’t
provide proof when requested, coverage
for your dependents will not become
effective, and any reimbursement
claims on behalf of that dependent
will be denied. If you need additional
information, please call the Benefits
Center at 1-888-5PB-FLEX. To verify
your dependents, you’ll need:
• A Social Security number for
each dependent
