PB Healthy Rewards Incentive Program<br />
PB Healthy Rewards pays you incentives for taking healthy steps. The program runs<br />
from November 1, 2012, through October 31, 2013.<br />
How It Works<br />
To participate in the program, you must complete these steps:<br />
• Take the online health assessment.<br />
• Get a biometric screening and submit results to your medical plan.<br />
• Achieve target values or complete a health coaching program.<br />
Once you have completed these steps, you can earn points by completing health<br />
actions. Every health action has a point value, and every point equals $1.<br />
Earn the Most Points!<br />
The more health actions you complete, the better for your health and the more points<br />
you earn. Here’s how to earn the most points and a suggested timeline to ensure you<br />
have enough time to complete the steps.<br />

*Employees who are first eligible for benefits on or after January 1, 2013 are not eligible for the<br />
2013 health assessment credit, but you still need to complete the assessment through your<br />
medical plan after you enroll in coverage to earn incentives in 2013.<br />
How Incentives are Paid<br />
Your medical plan determines how your incentives are paid:<br />
