The Community Partnership Award recognizes the efforts <strong>of</strong> a <strong>Care</strong> <strong>Providers</strong> <strong>of</strong> <strong>Minnesota</strong> member to create a<br />
partnership with a community group or organization that is typically outside <strong>of</strong> the care industry. The partnership<br />
improves the quality <strong>of</strong> life for residents/tenants/clients, staff, and/or volunteers.<br />
The creation <strong>of</strong> community bonds may take the form <strong>of</strong> educational activities, cooperative ventures, creative<br />
collaborations with a vendor, or other initiatives.<br />

different in their delivery or provision <strong>of</strong> a program or service. It recognizes those who have tried something a<br />
little out-<strong>of</strong>-the-ordinary and as a result, enhanced the lives <strong>of</strong> residents/tenants/clients, staff, or volunteers at a<br />
member organization.<br />

Excellence in Assisted Living Award<br />
An opportunity to focus on all the things you do right!<br />
This esteemed award recognizes a housing with services/assisted living establishment for exemplary dedication<br />
and commitment to quality. Nominations are judged on criteria which is based on the National Center for<br />
Assisted Living’s (NCAL) guiding principles for quality in assisted living: person-centered care, ethical practices,<br />
mission and vision statements, quality improvements, and workforce issues.<br />
There are two phases to the award: 1) a written application which consists <strong>of</strong> no more than 25 single-sided<br />
pages, and 2) a tour <strong>of</strong> the assisted living establishment IF applicant achieves a minimum score <strong>of</strong> 85% percent<br />
on their written application.<br />
Eligible Nominees: All housing members are eligible.<br />
Written Presentation: Submit written materials addressing the award criteria. Use Arial font [10 point] or Times<br />
Roman [12 point] on application; maximum amount <strong>of</strong> pages: 25 single-sided, double-spaced pages.<br />
Entry Deadlines:<br />
Presentations must be received by the Association or postmarked by 11:59 p.m. on May 3, <strong>2013</strong>.<br />
Procedures<br />
Finalists are selected based upon a numerical point distribution assigned to each award criterion. If applicant<br />
achieves a score <strong>of</strong> 85% or better, they will proceed to the next step: a tour.<br />
The scores <strong>of</strong> the written portion and the tour will be combined to determine the award recipient. The applicant<br />
with the highest total point score will be named the <strong>2013</strong> Excellence in Assisted Living Award recipient and will<br />
be announced at the <strong>2013</strong> Assisted Living & Senior Housing Summit, August 21-22. <strong>2013</strong>.<br />

Excellence in Assisted Living Award—Nomination Criteria<br />
The written presentation criteria: 120 points possible. Assisted living establishments must achieve a score <strong>of</strong> 85%<br />
(a score <strong>of</strong> at least 102 points) to proceed to community tour.<br />
1. GUIDING PRINCIPLE #1: PERSON-DIRECTED CARE (25 points)<br />
Person-directed care focuses on meeting an individual’s needs and is based on the concept that employees and<br />
management know each tenant—their history, needs, preferences, and expectations. With that in mind, please<br />
describe the following:<br />
How person-directed care occurs in your community.<br />
How management and employees have assisted the tenant and tenant’s family members transition into life at<br />
the community.<br />
Include two recent (within the last 12 months) testimonials from a tenant and/or family member.<br />
2. GUIDING PRINCIPLE #2: ETHICAL PRACTICES AND FINANCIAL STEWARDSHIP (25 points)<br />
NCAL and <strong>Care</strong> <strong>Providers</strong> <strong>of</strong> <strong>Minnesota</strong> believe that all providers should operate their assisted living communities<br />
based on a foundation <strong>of</strong> trust. <strong>Providers</strong> will operate their communities and provide service with integrity.<br />
Based on the statement above, please describe how services are provided in your community.<br />

James B. Swanson was a facility owner, corporate administrator, consultant, state and<br />
national industry leader, and activist. An innovative and inspiring pr<strong>of</strong>essional, he encouraged<br />
his peers and colleagues to do their very best. The Service <strong>of</strong> Excellence: James B.<br />
Swanson Award is named in his honor, in recognition <strong>of</strong> his passionate commitment to the<br />
elderly and our pr<strong>of</strong>ession.<br />

Nursing facilities applying for the <strong>2013</strong> Service <strong>of</strong> Excellence: James B. Swanson Award<br />

Objectives<br />
The Service <strong>of</strong> Excellence: James B. Swanson Award is designed to recognize <strong>Minnesota</strong>’s best nursing facilities.<br />
The award honors the contributions and commitment <strong>of</strong> all nursing facility employees and the important services they<br />
provide as they strive for excellence in the performance <strong>of</strong> their duties. Participating members are judged on criteria<br />
such as leadership, client satisfaction, person-centered care, and employee engagement. The competition provides<br />
the opportunity to educate the public about our pr<strong>of</strong>ession and the high quality <strong>of</strong> the services we provide. It is intended<br />
to promote the long-term care industry’s efforts toward excellence to public <strong>of</strong>ficials, to the general public, and<br />
within the pr<strong>of</strong>ession itself.<br />
Procedures<br />
Finalists are selected based upon a numerical point distribution assigned to each award criterion. Finalists who<br />
achieve a score <strong>of</strong> at least 85% will proceed to the next step, a facility tour. The scores <strong>of</strong> the written portion <strong>of</strong> the<br />
submission and the tour will be combined to determine the award recipient. The finalist with the highest total<br />
point score will be named the <strong>2013</strong> Service <strong>of</strong> Excellence: James B. Swanson Award recipient. The name <strong>of</strong> the<br />
award recipient will be kept in strict confidence until it is announced during the award presentation event. (In the<br />
event there is only one finalist, the facility still needs to reach a certain score on the tour in order to receive the award.)<br />
