your eyes or vision, including x-rays, MRI
and CT scans of your eye, ophthalmic
diagnostic test results, ophthalmic surgical
notes and summary letters from doctors
❑ Insurance card or proof of insurance coverage
❑ Authorization or referral forms if required
by your insurance company
WHAT TO EXPECT
ON YOUR VISIT
As our patient, you will receive the finest
ophthalmic care available. You will benefit from
having all necessary examinations, diagnostic
tests and medical procedures in one location.
Because we aim to perform all necessary tests
on the day of your appointment to avoid the
need for multiple visits, please be aware that
the appointment may take three to four hours.
Questions for your doctor:
You may want to bring reading materials and
a sweater as the building is generally cool.
If you require something to eat or drink during
your appointment, you are welcome to bring
a snack. For your convenience, several restaurants
are within walking distance of Bascom
Palmer and vending machines are located in
the adjacent Bascom Palmer Surgery Center.

co-payments and deductibles, is collected at
the time of your appointment unless prior
arrangements are made.
For your convenience, payment may be made
with cash, personal check, MasterCard, Visa,

Drive, Palm Beach Gardens, Florida. Bascom
Palmer’s new Palm Beach Gardens campus
opened in the summer of 2006. It is located
at the corner of PGA Boulevard and Fairway
Drive across from the entrance to the
Florida Turnpike.

Your pupils may be dilated during your visit
making them sensitive to light and causing
blurred vision for several hours. Therefore,
we ask that you bring someone with you to
drive you home because driving with dilated
pupils is not safe or advisable.
Free parking is available adjacent to the building.
If you are unable to find someone to drive
you to your appointment or you require
hotel accommodations during your visit
to Bascom Palmer, the Patient Services
Department can arrange for transportation
services and hotel reservations.
