2. <strong>HQIP</strong>’s <strong>Data</strong> <strong>Access</strong> <strong>Request</strong> Form (DARF)<br />

<strong>Data</strong> Sharing Agreement (DSA) – this formalises <strong>the</strong> terms and conditions under which any data will be shared<br />

by <strong>HQIP</strong>. The DSA must be signed by all relevant parties prior to <strong>the</strong> <strong>Data</strong> <strong>Access</strong> Application Form (DARF)<br />

being considered.<br />

<strong>Data</strong> <strong>Access</strong> <strong>Request</strong> From (DARF) – <strong>the</strong> DARF captures <strong>the</strong> specific data being requested and <strong>the</strong> proposed.<br />

The DARF also confirms that <strong>the</strong> necessary approvals from o<strong>the</strong>r <strong>Data</strong> Controllers have been gained. NCAPOP<br />

Suppliers can provide <strong>the</strong>ir recommendations regarding <strong>the</strong> data access request on this form.<br />

<strong>Data</strong> <strong>Access</strong> <strong>Request</strong> Group (DARG) – is <strong>the</strong> group who decides if <strong>the</strong> data can be released as requested. The<br />

DARG will only consider requests accompanied by a signed DSA and completed DARF. The DARG consists of<br />

<strong>the</strong> following members: <strong>HQIP</strong>, Information Governance Manager, MRIS, Project Manager (project specific),<br />

Academic expert.
