NCAPOP Supplier wishes to use the
data collected from a project for a
purpose or project which is outside
the scope of their HQIP contract

data contacts either HQIP or the
Project Supplier to discuss proposed

1. HQIP’s Data Sharing Agreement (DSA)
2. HQIP’s Data Access Request Form (DARF)

Data Sharing Agreement (DSA) – this formalises the terms and conditions under which any data will be shared
by HQIP. The DSA must be signed by all relevant parties prior to the Data Access Application Form (DARF)
being considered.
Data Access Request From (DARF) – the DARF captures the specific data being requested and the proposed.
The DARF also confirms that the necessary approvals from other Data Controllers have been gained. NCAPOP
Suppliers can provide their recommendations regarding the data access request on this form.
Data Access Request Group (DARG) – is the group who decides if the data can be released as requested. The
DARG will only consider requests accompanied by a signed DSA and completed DARF. The DARG consists of
the following members: HQIP, Information Governance Manager, MRIS, Project Manager (project specific),
Academic expert.
