8. Provide a staff person to accept same day trip cancellations and provide<br />

other operational information to the <strong>County</strong> and customers. This person shall<br />

be available at least one hour before the start of each service day.<br />

9. Assure that there are a sufficient number of road supervisors active and on<br />

call at all times during <strong>ParaTransit</strong> operations to properly monitor<br />

operations.<br />

9.10 Reporting of Accidents<br />

The following events are considered reportable accidents regardless of severity:<br />

• Personal injury, regardless of severity.<br />

• A motor vehicle collision when two or more vehicles are involved.<br />

• A motor vehicle accident when there is any destruction of public property.<br />

• A motor vehicle accident involving a pedestrian.<br />

Report these situations or emergencies according to the following procedures:<br />

• Before, during or after regular business hours, the vendor must<br />

immediately contact the <strong>ParaTransit</strong> Supervisor via cell phone to report<br />

any and all accidents. The contact phone number is 914-879-8526<br />

• Prepare and submit to the WCOFD within 24 hours of the accident a<br />

written report which describes the events surrounding the accident, lists<br />

the names of the passengers onboard the vehicle, identify which - if any -<br />

has been injured, estimates the cost of repair of physical damage, and<br />

includes photographs that show the damage to the vehicle.<br />

• The police report shall also be sent to OFD as soon as it is received.<br />

9.11 Reporting Incidents/Service Disruptions<br />

The following events are considered incidents and must be reported in writing to<br />

the OFD within 24 hours of their occurrence:<br />

• Any incident on board that results in physical contact, whether between two<br />

passengers or between a passenger and the driver must be reported.<br />

• Any incident where a passenger falls, whether or not injury .is reported or<br />

denied by passenger.<br />
