Benefits Enrollment Guide - City of Knoxville

Your City of Knoxville benefits touch many aspects of your life: your health, your finances, your mental

well‐being and the protecon of your family, just to name a few. The City provides these benefits not only

to support you through important events in your life, but also to enhance your life outside work and help

you prepare for the future. This guide summarizes your benefits for 2014. Check out the important

reminders on page 4, then follow the steps below to make your choices.

Start here for annual enrollment.

If you want to:

Change your medical, dental or vision plan elections for 2014

Participate in the flexible spending accounts (FSAs)

Sell annual leave (formerly Benefit Bucks)

Update your dependents / beneficiaries

Increase your supplemental life coverage (may be subject to

Evidence of Insurability)

Verify or update your address

Waive City medical coverage and receive the $500 Health

Care FSA contribution (see page 18)

From Inside Knoxville, choose the ‘Online Annual Enrollment

2014’ option from the City‐Wide dropdown box.

*If you are enrolling your domestic partner, login to PeopleSoft to

add s/he as a dependent. Follow the required steps and bring the

requested materials to the Employee Benefits Office. See page 4

for dependent eligibility.

Join Sick Leave Bank for the first time Complete the Sick Leave Bank enrollment form (see page 20)

Enroll in or change your coverage in any of the voluntary

insurance plans (see pages 22‐23)

Provide proof of other coverage to the Employee Benefits office in

order to receive the $500 FSA contribution.

Complete the personalized enrollment form

provided by your Benefits Guide if hired since

October 2012; if you’re already enrolled or were previously

eligible and want to enroll now, call 1‐877‐848‐9251

If you don’t enroll, your current benefits will continue in 2014. But remember, FSA and Vacation Sell (formerly Benefit Bucks)

participation does NOT carry over‐‐you must re‐enroll to keep participating. Additionally, if you are enrolled in a Delta buy‐up

option, you will have to elect a new plan.

Start here if you are a newly hired employee enrolling for

the first me.

Elect supplemental life or spouse life coverage

You must:

Log into PeopleSoft, use the Self Service tab and complete the

e‐Benefits online enrollment tool.

Enroll your Spouse or Domestic Partner in City medical

coverage

Join the Sick Leave Bank

Enroll in any of the voluntary insurance plans (see pages 22‐

23)

After adding them as a dependent in People Soft

Complete the Sick Leave Bank enrollment form
