WE ARE YOUR PARTNERS
IN SAFETY
Trinitas Hospital and its employees are committed to
be your partners in safety. At Trinitas Hospital, we have
improved patient safety with our “Catch a Falling Star”
fall prevention program, pressure ulcer prevention,
infection control practices and other safety procedures.
Here are some safety tips to make sure that
your care is as safe as possible.
ACTIVELY PARTICIPATE IN YOUR CARE
Keep an up-to-date list of the following with you at all
times:
• Your medical problems, immunizations and previous
hospitalizations
• The name and phone number of your doctor and
pharmacy
• Any allergies you have to medicine, food or anything
else
Learn about your illness and how to stay healthy.
Ask a family member or friend to go with you to the doctor
to help you ask questions or get further information.
Answer questions about your illness to help you understand
it and the plan for your care.
Let your doctor know if you are not feeling well or if you
experience any change in your condition.
While in the hospital:
• Ask about the use of call lights, bed controls and
medical equipment used in your care.
• Always wear your ID bracelet.
• Wash your hands with
soap and water, especially
after using the restroom
and before eating.
• Ask everyone who comes
in contact with you, staff
members or visitors, to
wash their hands to
reduce your exposure
to germs.
USE MEDICINES SAFELY
Keep an up-to-date list of the medicine you are taking
including prescriptions, vitamins, herbals, food supplements
or any other drugs.
Bring your list with you every time you visit your doctor
or go to the hospital.
Make sure you know the names of all your medicine
and why you are taking them.
When a doctor gives you a new medicine, ask if it will
interfere with or replace what you are already taking.
• Ask questions about any
instructions or information
given that are confusing or
unclear.
• If you are concerned about
any safety issues, please speak with the unit manager or
any member of the hospital staff. We’ll do our best to
resolve the problem.
• Do not accept medication from anyone if they have not
checked your ID bracelet.
• If you are not sure about the medicine the nurse is giving
you, ask him or her to check your chart before you take it.
• Tell your nurse or doctor if you do not feel well after
receiving a medicine.
PREVENT A FALL
According to the Center for Disease Control and Prevention,
falls are the most common cause of injuries to older adults.
Make your home safer
• Remove things you can trip over from stairs and
places where you walk.
• Remove small throw rugs or use double sided tape to
keep the rugs from slipping.
• Use non-slip mats in the bathtub and on shower floors.
Improve the lighting in your home.
Have handrails put in on all staircases.
Wear slippers or shoes that will not slide both inside
and outside the house.
While in the hospital:
• Keep your glasses, cane or walker within reach.
• Wear slippers or shoes that will not slide.
• If something spills on the floor, call us immediately to
wipe it up.
• Call for help when you want to get out of bed or
want to use the bathroom.
