IN SAFETY<br />

<strong>Trinitas</strong> <strong>Hospital</strong> and its employees are committed to<br />

be your partners in safety. At <strong>Trinitas</strong> <strong>Hospital</strong>, we have<br />

improved patient safety with our “Catch a Falling Star”<br />

fall prevention program, pressure ulcer prevention,<br />

infection control practices and other safety procedures.<br />

Here are some safety tips to make sure that<br />

your care is as safe as possible.


ACTIVELY PARTICIPATE IN YOUR CARE<br />

Keep an up-to-date list of the following with you at all<br />

times:<br />

• Your medical problems, immunizations and previous<br />

hospitalizations<br />

• The name and phone number of your doctor and<br />

pharmacy<br />

• Any allergies you have to medicine, food or anything<br />

else<br />

Learn about your illness and how to stay healthy.<br />

Ask a family member or friend to go with you to the doctor<br />

to help you ask questions or get further information.<br />

Answer questions about your illness to help you understand<br />

it and the plan for your care.<br />

Let your doctor know if you are not feeling well or if you<br />

experience any change in your condition.<br />

While in the hospital:<br />

• Ask about the use of call lights, bed controls and<br />

medical equipment used in your care.<br />

• Always wear your ID bracelet.<br />

• Wash your hands with<br />

soap and water, especially<br />

after using the restroom<br />

and before eating.<br />

• Ask everyone who comes<br />

in contact with you, staff<br />

members or visitors, to<br />

wash their hands to<br />

reduce your exposure<br />

to germs.


USE MEDICINES SAFELY<br />

Keep an up-to-date list of the medicine you are taking<br />

including prescriptions, vitamins, herbals, food supplements<br />

or any other drugs.<br />

Bring your list with you every time you visit your doctor<br />

or go to the hospital.<br />

Make sure you know the names of all your medicine<br />

and why you are taking them.<br />

When a doctor gives you a new medicine, ask if it will<br />

interfere with or replace what you are already taking.<br />

• Ask questions about any<br />

instructions or information<br />

given that are confusing or<br />

unclear.<br />

• If you are concerned about<br />

any safety issues, please speak with the unit manager or<br />

any member of the hospital staff. We’ll do our best to<br />

resolve the problem.<br />

• Do not accept medication from anyone if they have not<br />

checked your ID bracelet.<br />

• If you are not sure about the medicine the nurse is giving<br />

you, ask him or her to check your chart before you take it.<br />

• Tell your nurse or doctor if you do not feel well after<br />

receiving a medicine.


PREVENT A FALL<br />

According to the Center for Disease Control and Prevention,<br />

falls are the most common cause of injuries to older adults.<br />

Make your home safer<br />

• Remove things you can trip over from stairs and<br />

places where you walk.<br />

• Remove small throw rugs or use double sided tape to<br />

keep the rugs from slipping.<br />

• Use non-slip mats in the bathtub and on shower floors.<br />

Improve the lighting in your home.<br />

Have handrails put in on all staircases.<br />

Wear slippers or shoes that will not slide both inside<br />

and outside the house.<br />

While in the hospital:<br />

• Keep your glasses, cane or walker within reach.<br />

• Wear slippers or shoes that will not slide.<br />

• If something spills on the floor, call us immediately to<br />

wipe it up.<br />

• Call for help when you want to get out of bed or<br />

want to use the bathroom.<br />

• If you get dizzy or<br />

your legs feel weak:<br />

STOP!<br />

SHOUT for HELP!<br />

EASE YOURSELF<br />

SLOWLY to the FLOOR!<br />
