each team must submit a team admission fee. No exceptions can be made.
2. Teams must be registered for this World Series event on the national website, www.usssa.com, before
submitting this form.
3. Make check payable to Wicomico County ($50 fee applied to returned checks).
4. Mail check/credit card information and order form to Wicomico County Tourism, ATTN: Linda Lyons,

5. Payment must be received by July 1, 2013. No refunds on team admission fees due to late
