<strong>APCOM</strong><br />

The Asia Pacific Coalition on Male Sexual Health<br />

<strong>APCOM</strong> was specifically formed as a coalition of community, government, donor sector representatives, along with UN<br />

agencies, to strongly advocate regionally and globally for increased investment in line with the need, scale up of MSM<br />

HIV intervention programming and increased coverage, research to address gaps in knowledge, and the promotion of<br />

individual rights for MSM and transgender people across the Asia Pacific region.<br />

Launched at the 8th ICAAP, Colombo, Sri Lanka in 2007, and arising from the 2006 Asia Pacific consultation meeting<br />

on male sexual health, held in New Delhi, India, <strong>APCOM</strong>, as a community-led institution, represents the forefront of<br />

the challenges and responses to HIV among MSM and transgenders in the Asia Pacific region, strongly advocating for<br />

increased investment, scaling up coverage of comprehensive HIV services, and addressing stigma, discrimination and<br />

social exclusion of MSM and transgenders across the region.<br />

Guiding Principles<br />

The work of the Asia Pacific Coalition on Male Sexual Health will be guided by the following principles, which have been<br />

adapted from the Guiding Principles of the Global Forum on MSM and HIV/AIDS: 1<br />

• Enabling environment. The inter-related co-factors for HIV risk, vulnerability, and disparities in access to care,<br />

treatment and prevention education, coupled with social discrimination and individual rights issues, have a common<br />

basis in the significant lack of services and coverage faced by MSM and transgenders. We believe that an enabling<br />

environment is the key to the health and well-being of MSM, transgenders, and their families.<br />

• Evidence informed: MSM and transgender HIV prevention, treatment, care and support, and programmes and policy<br />

should be guided by a nuanced understanding based on evidenced scientific information.<br />

• Strength-based. Programmatic and policy responses often frame HIV in negative ways and stigmatise MSM and<br />

transgenders; we believe that effective responses to the HIV epidemic directed at MSM and transgenders must<br />

acknowledge and build upon the strengths, competencies, and resources that such persons possess.<br />

• Affirming sexualities and acknowledging sexual behaviours at risk. We believe that the reality of sexualities,<br />

gender expression, and the risks involved in certain sexual behaviours need open acknowledgement. All MSM and<br />

transgenders have the right to healthy and fulfilling sex lives and to access community driven HIV interventions<br />

prevention, treatment, care and support - free from judgement and persecution. There needs to be a healthy and<br />

productive relationship between the communities and governments, based on mutuality of respect, positive involvement<br />

and open flow of information.<br />

• Community driven interventions. Self-organizing and open participation in the provision of appropriate HIV<br />

prevention, treatment, care and support, services for MSM and transgenders is important in our work to end HIV. We<br />

believe in supporting and respecting self-determination and self-initiated HIV programmatic and policy responses.<br />

• Involvement. There must be greater involvement of MSM and transgender, including HIV positive MSM and<br />

transgender people, in programme planning and policy development arenas. There must also be equal support and<br />

participation by governments, funders, and technical experts. We believe that HIV programme and policy responses<br />

are strengthened by ensuring inclusion, parity and representation.<br />

• Resources. There is an urgent need to significantly increase investment, funding, capacity, and technical assistance and<br />

support for HIV programming directed at MSM and transgenders. At a minimum, we believe that funding should be<br />

at a level commensurate with the impact left by HIV on these groups as compared to the general population.<br />

Area of emphasis<br />

The Coalition devotes itself to fostering, supporting and sharing information about:<br />

• Country-level, sub-regional and regional action which raises awareness of MSM and transgender issues, and<br />

ensures that national, sub-regional and regional plans and strategies incorporate activities for MSM and transgenders,<br />

guarantees MSM and transgender representatives having a voice at policy setting forums, and mobilises<br />

expertise and financial resources.<br />

• Evidence-based research and policy development to address critical knowledge gaps and to develop more responsive<br />

programme and policy recommendations.<br />

• Advocacy through the establishment of effective partnerships with civil society organizations, government bodies,<br />

HIV organizations, academia and research organizations, and UN agencies.<br />

INTRODUCTION: The HIV/AIDS epidemic among MSM/TGs<br />

in Asia<br />

Asia has all the ingredients for a full-blown epidemic among men who have sex with men and transgenders. Evidence<br />

points to an increasing prevalence among MSM/TGs in many Asian countries. In many countries, evidence points to a<br />

sexual flip in the mode of transmission, with MSM/TGs becoming the face of the epidemic than heterosexuals. Response<br />

to the infection has been limited, as most countries, if not all, are constrained by poverty, repressive political conditions,<br />

impaired healthcare infrastructures, and obscurantist religions. Available services are therefore a result of external<br />

interventions. 2<br />

The Commission on AIDS in Asia warned that in Asia, a new wave of infections is imminent particularly among<br />

vulnerable groups, and that by 2020, 50% of new infections would come from the MSM/TG population. 3<br />

THE MAPPING PROJECT: Objectives and Methodology<br />

Scaling up response in ISEAN countries necessitates baseline information on the scope of MSM/TG organizing in the<br />

region, as well the range of available HIV/AIDS services for MSM/TGs in said areas,<br />

Produce an organizational database of groups, community organizations, and institutions that provide HIV/AIDS<br />

services for MSM/TGs in their respective countries or territories, from prevention, treatment, care and support to<br />

stigma reduction advocacy work;<br />

Aggregate findings on the state of preventive measures for MSM/TGs that are being implemented in the target<br />

countries and territories and identify the strengths and weaknesses of these responses, and;<br />

Identify MSM/TG-related issues to present a general and qualitative analysis of situation of the community in their<br />

respective countries and cull needs for further action.<br />

Online data-gathering is the main mode of data acquisition for the project. A questionnaire was initially sent to several<br />

organizations to collect data on the following areas: a) basic organizational profile, b) organizational activities/services<br />

on HIV/AIDS, and c) situational factors or conditions affecting the organization.<br />

Key contacts from international or regional and national HIV/AIDS organizations and local academicians working<br />

directly with HIV/AIDS service providers and MSM/TG or lesbian, gay, bisexual and transgender (LGBT) advocacy<br />

groups were likewise tapped as secondary sources of information.<br />

KEY FINDINGS<br />

Of the 99 organizations that were initially contacted for the database-building project, only 20% or 20 responded with<br />

complete information.The lowresponse rate could be attributed to several factors: the language barriers between the<br />

investigator and the target groups, including the inaccessibility of non-English online sources;lack of information of<br />

<strong>APCOM</strong>, Fridae, or the investigator on the state of MSM/TG organizing in some countries, especially in areas where the<br />

society is hostile towards homosexuals or transgenders, and; limitations on resources that could facilitated a more direct<br />

appraisal of the situation.<br />

To augment, alternative channels were explored through key contacts working directly with MSM/TG community<br />

organizations.<br />

A total of40 community organizations, NGOs, centers, advocacy groups that provide a range of HIV/AIDS services<br />

specifically for the MSM/TG community, from exclusively preventive measures or programs to a mix of preventive,<br />

support services and stigma reduction advocacy work,have been included in the database. Government providers of HIV/<br />

AIDS services have been excluded due to questions on their sensitivity to the situation of MSM/TGs, except in the case of<br />

public clinics or hospitals that serve as the main testing centers or treatment hubs for all.<br />

There appears to be no institution, NGO, community-based organization or hospital or clinic in insular Southeast Asia<br />

that can provide a comprehensive response or package of HIV and sexual health services and interventions for MSM/<br />

TGs. 4<br />

Furthermore, there is limited or no information on existing HIV/AIDS services for MSM/TGs in Brunei Darrusalam and<br />

Timor Leste, raising questions on the response being taken to address the issue in these areas.<br />

On Prevention, Treatment, Care and support.<br />

Community response is largely focused on prevention, with peer-led workshops, confidential discussions,online and<br />

on-site or venue-based outreach activities, hotlines and anonymous counseling, and the distribution of IEC materials and<br />

safer sex paraphernalia as the main forms of interventions.<br />

These activities are mostly funded externally, raising questions on sustainability. There are also problems with organizational<br />

capabilities in light of the rise in infection, especially in the area of mobilizing the community and raising<br />

resources.<br />

VCTs and ARVs can mostly be accessed through public hospitals or government-owned social hygiene clinics. While these<br />

facilities provide anonymous testing and are mainly the distribution network for ARVs, it is unclear whether they are<br />

sensitized to the situations of the MSM/TGs.<br />

Preventive activities are oftentimes partnered with affirmative interventions for sexual minorities, especially on the issues<br />

of social acceptance, religion, and human rights.<br />

2.<br />

Governance and participation.<br />

Groups in Malaysia, Indonesia and the Philippines have indicated relatively programmatic interventions on policy<br />

advocacy. It has also been noted that collaboration with governmental bodies, whether in national HIV/AIDS institutions<br />

or local governments, exists, with the community organizations finding such engagements meaningful and effective.<br />

Community organizing is strong in Indonesia and the Philippines,very minimal in Malaysia, and almost non-existent in<br />

Brunei Darussalam and Timor Leste. In all countries, discriminatory policies and practices, abusive law enforcers, and<br />

punitive laws and ordinance hinder community organizing.<br />

In Indonesia and the Philippines, organizations affirm the importance of community organizing to strengthen the<br />

participation and engagement of MSM/TGs.<br />

There is also difficulty in reaching out to vulnerable sub-groups of MSM/TGs, such as male or transgender sex workers,<br />

drug users, and sex workers. Transgender groups in Indonesia and the Philippines highlighted problems in involving the<br />

participation of transgenders.<br />

3.<br />

On stigma reduction.<br />

In all ISEAN countries, political repression, persecution by religious groups, and lack of social acceptance hinder the full<br />

inclusion of MSM/TGs in their respective national response and compromise the effectiveness of an already limited range<br />

of services. Male-to-male sex is prohibited explicitly in Malaysia and Brunei Darussalam, and while legal in Indonesia,<br />

the Philippines and Timor Leste, other laws or ordinances (in the case of Indonesia, Sharia ordinances) are used as tools<br />

for repression against MSM/TGs.<br />

No legal protection for MSM/TGs exists in all ISEAN countries.<br />

Ironically, the national response policies for HIV/AIDS in Indonesia, Malaysia, the Philippines and Timor Leste explicitly<br />

mention MSM/TGs. Correcting the dissonance in HIV/AIDS policies and MSM/TG-punitive laws or enforcement of laws<br />

appears to be a non-priority among ISEAN governments, which compromises their own national response policies and<br />

programs to combat HIV/AIDS. Various MSM/TG groups have identified this failure to harmonize national policies a<br />

major barrier towards prevention.<br />

Faith-based stigma and lack of social acceptance have also been identified as a major issuein all ISEAN countries.<br />
