<strong>Application</strong> <strong>for</strong> a <strong>Tempo</strong> <strong>100</strong> <strong>Examination</strong><br />

of a public service vehicle PSVC 1<br />

Please read these notes be<strong>for</strong>e you complete this application.<br />

Failure to complete accurately could cause delay.<br />

Form PSVC1 must be used when applying <strong>for</strong> a <strong>Tempo</strong> <strong>100</strong><br />

examination whether or not it is intended to have another<br />

examination carried out at the same time (eg Certificate of Initial<br />

Fitness examination).<br />

When to apply<br />

<strong>Application</strong>s <strong>for</strong> examinations must be sent not less than 1 month<br />

be<strong>for</strong>e the date requested.<br />

When completing this <strong>for</strong>m please remember to write in<br />

BLOCK CAPITALS<br />

Question 1 It is essential to give the identification mark of the<br />

vehicle. If the vehicle has been registered give the registration<br />

mark and chassis number, even if it is not currently licensed. If it<br />

is not registered give the chassis number (See Question 3).<br />

Question 11 <strong>Examination</strong>s will normally be conducted at Goods<br />

Vehicle Testing Stations. It may however be possible to conduct<br />

the examination at another location but only with the agreement<br />

of the local Certifying Officer who may be contacted through the<br />

local Goods Vehicle Testing Station.<br />

All stations except those marked with an asterisk, are open from<br />

08.00-17.00 on Mondays to Thursdays and 08.00-16.30 on Fridays.<br />

Stations marked with an asterisk are smaller and although they are<br />

capable of testing most vehicles are only open on demand.<br />

Questions 12, 13 A lower charge will apply if a <strong>Tempo</strong> <strong>100</strong><br />

examination is carried out at the same time as an examination <strong>for</strong><br />

Certificate of Initial Fitness. Details of current charges are<br />

available on request from your local Goods Vehicle Testing Station,<br />

or the PSV Section, Swansea.<br />

Sending the wrong payment may cause delay in your vehicle<br />

being examined.<br />

Cheques and postal orders should be crossed and made payable<br />

to the ‘VOSA’. Do not under any circumstances send notes, or<br />

coins or Giro transfers in the post.<br />

NOTE. The payment enclosed with this application only covers the<br />

examination. When you receive the documentation from the<br />

Certifying Officer you should send one copy of his report and the<br />

fee <strong>for</strong> the <strong>Tempo</strong> <strong>100</strong> permit and decal to the West German<br />

authorities. The Certifying Officer will be able to give you further<br />

advice at the time of the examination.<br />

Question 14 For limited Companies, partnerships or sole<br />

proprietor, the registered name should be given. If a trading name<br />

is used, it should be given.
