Mail or bring your completed registration and payment to the City of Powell Municipal Building, 47 Hall St. • Registration cannot be accepted over the phone, via e-mail or fax. • Class fees must be paid in full at the time of registration. Inperson registration payment may be made by cash, check or credit/debit card (VISA/Mastercard). Please make checks payable to “City of Powell.” • Please complete all information on the registration form. Incomplete registration forms, incorrect payment and/or failure to sign the liability waiver will result in your application being delayed or returned. • Mail-in registrations are processed in the order received AFTER in-person registrations. www.cityofpowell.us
General Information/Fall 2012 5 Photo Release By registering for or attending any City of Powell Parks and Recreation Department program, class or event, you are agreeing to allow the publication of any photos taken of you and/or your child at any program, class, event or facility of the City of Powell Parks and Recreation Department. Class Cancellation The City of Powell Parks & Recreation Department reserves the right to cancel classes due to low enrollment and/or other unforeseen circumstances. In the case of program/class cancellation, you will be notified and issued a full refund in the form of a check. There are no cash refunds. Refunds take approximately four weeks to process. When a program or class is cancelled due to low enrollment, every effort will be made to notify you 48 hours in advance of the first class. Please prepare to attend each class unless you are notified by a City of Powell employee or the instructor of your class. Refunds Participant-requested refunds are subject to a $5 processing fee. Refunds will be granted only when requested no later than 24 hours prior to the first class meeting. Since decisions to maintain classes are based on the enrollment at the first session, no refunds are granted after the first session. In the case of maximum class enrollment or class cancellation, you will be granted a full check refund. Please allow four weeks for processing all refunds. Anticipate to participate. Register early . . . Waiting until the last minute could mean the program you are interested in will be cancelled. In order for a class to survive, we must have at least the minimum number of registrants 48 hours prior to the start of the first class. Attempting to enroll on the day classes are set to begin will not save the class. PLEASE REGISTER EARLY. You can share your talents . . . The City of Powell Parks & Recreation Department is continually seeking potential instructors with the desire and ability to teach. If you have a special skill, practical knowledge or experience in an area, talent, craft, hobby or activity that you would like to share, we would like to work with you to set up a program. Please contact Silas Bowers at 614.885.9820, ext. 2 to take advantage of this great experience and opportunity. Future Brochure Mailings This Parks & Recreation Program Guide is delivered at no charge to all residents within the City of Powell municipal boundary. Recreation Program Guides are also available at the Village Green Municipal Building and the Delaware County Library - Powell Branch. Anyone who wishes to receive the Program Guide who currently does not may subscribe at a cost of $5 for the entire year. Simply enter Program Number 2012B on the registration form and return it with the $5 fee. Please include your e-mail address on the registration form to be made aware of future programming.
