Annual Report 2008
Dear Friends,
As the Chair of the Board it is my privilege to welcome
you to browse and read through our first annual report.
What a steady pace of growth it’s been since we
opened over seven years ago. From the core group
of our founding partners in 2001 — County of San
Mateo, Lucile Packard Children’s Hospital, Peninsula
Community Foundation*, David & Lucile Packard
Foundation and the City of East Palo Alto — our circle
of support has widened with each year. Now, 38% of our
funding comes from the private sector of foundations,
donor-advised funds and individuals.
“A typical morning in the waiting room.
Three powerful forces — a contracting economy, global warming,
and huge health care costs — have made us all a lot more focused
on prevention. My career in health care began in 1973 when I
became a full time volunteer community health worker in a United
Farmworker’s Union clinic. In the ensuing years, as a public
health nurse and health care leader, I’ve been committed to the
development of community health centers (CHCs) in California
and across the country, designed to meet the needs of medically
underserved and uninsured people.
CHCs have now come of age and offer the most cost effective
system for delivering primary and preventive care in the country.
With rising unemployment, the expansion of the over 65 age group,
and an alarming increase in chronic diseases, the CHC has risen
from “humble” origins to a place of prominence in the nation’s
health care system, averting and significantly reducing costly ER
”
visits and hospitalization and improving the health status of our
most vulnerable populations.
Benjamin Lopez with parents and sister, and the attending physician Dr. Reshma Thadani.
East Palo Alto residents suffer from
disproportionately high rates of asthma.
From 10.92% to 14% of children in our community are affected.
Although asthma can be well controlled, every year children
die needlessly for lack of appropriate medical management and
access to care. Fortunately, Benjamin Lopez was not one of those
children, because Ravenswood Family Health Center (RFHC)
provided an important safety net for him and his family.
Following a severe asthma attack at age 7, Benjamin remained
symptom free for 6 years, until environmental factors in his home
triggered another episode. His mother brought him to RFHC,
where Dr. Elizabeth Baca treated Benjamin. When he required
hospitalization, RFHC benefit coordinators enrolled him in
insurance, because his family has no health insurance.
“It was Dr Baca’s attentiveness that led to my son’s recovery,”
says Benjamin’s mother.
“
Ravenswood Family Health Center is a jewel of
a community resource. I’ve always been grateful
that my family’s health care needs were well
met and I want our neighbors in East Palo Alto,
east Menlo Park and North Fair Oaks to have
the same opportunity. RFHC provides the basic
family health care needs—primary medical,
pre-natal, mental health and dental—that, left
untreated, can lead to more serious problems.
ur patients are all ages, sometimes entire
families: expectant mothers, newborns,
growing children, parents, and seniors
struggling with chronic disease.
In our definition of family we include those who
have no home or are living in transitional housing as
they go through community recovery programs.
Family describes the way we treat our patients—with
respect and genuine concern, speaking their language
whenever possible, always providing culturally
appropriate treatment. Our skilled providers help
clients learn to care for children with asthma, or
grandparents with diabetes. We teach prevention,
as well as cure.
And family describes our staff and volunteers—the
collaborative way our providers work with one
another and with our valued community partners.
In December, Jennifer McPike found
shelter and support in a recovery program.
At RFHC she’s under the care of clinician Brad Hamman,
Physician Assistant, and has benefited from counseling with
Denise Chun, MFT. Both, she said, made her feel they were
committed to her success.
Our Mobile Clinic reaches out to the
homeless every Thursday afternoon.
Luis hobbled in to the mobile clinic with holes cut into his shoes
because his feet were swollen and painful and an infection was
spreading up his legs. His feet were treated, and he was given
strong antibiotics to treat the infection. Most importantly, he
was treated with dignity and compassion.
As Nichole Armijo-Inguito, a student
Palo Alto Medical Foundation is proud to support
RFHC because of its dedication to providing
high-quality health care to those less fortunate
in the community. We have been meeting
regularly with RFHC leadership over the last four
years with the goal of helping them to increase
their capacity to care for the underserved and
target resources for chronic disease prevention
and management. A big part of this is tracking
and reporting patient outcomes, and I would
especially like to recognize PAMF Senior Director
of Quality and Planning Tomas Moran for his
dedicated efforts in this regard. In addition,
PAMF has contributed financially to hire and train
staff at the clinic. Through our relationship with
RFHC we are able to make an impact beyond
their muscles and develop healthy hearts and lungs.
We encourage our clients to reconnect with cultural
and family traditions that promote fitness and vitality.
From the Get Fit collaborative emerged City on the
ways in the community that people stay healthy.
RFHC partnered with Collective Roots Garden Project,
supported by the Tides Foundation, to fund a weekly
Farmer’s Market and outreach to local schools to spread
the message of going green with fresh, organic produce
and local gardens.
Health disparities emerge when people live on the
margin with poorer living conditions and greater
stresses. Diabetes, hypertension, asthma and mental
health issues are common. In 2008, 14% of our patients
were diagnosed with diabetes, 12% with hypertension,
5% with asthma and 8% with depression or anxiety
disorders. RFHC addresses these conditions with a
comprehensive chronic disease management program
and an integrated behavioral health program. Thanks
to grant support from Palo Alto Medical Foundation
and Kaiser Permanente, RFHC has the tools to monitor
key indicators and track patient outcomes, leading to
improved care through a set of best practices.
Nothing is worth more than health. Please share the gift of
health with a generous contribution to Ravenswood Family
Since its establishment, Lucile Packard Children’s Hospital
has partnered with the Ravenswood Family Health Center to
ensure that children receive quality primary care in an accessible
community clinic setting. Packard Children’s provides grant
support and contracts medical providers and other pediatric staff
to Ravenswood. Together our organizations have established a
seamless continuum of care for children and pregnant women.
By attending to primary care needs, Ravenswood has contributed
to a decline in emergency department visits for non-urgent care
by people living in the Ravenswood service area, and has enabled
avenswood Family Health Center is a catalyst for
positive changes. We are fortunate to be supported
by community partners, who provide staff members,
financial support, and services to our patients.
As a community health center, we provide a seamless
continuum of care that includes patient exams, preventive
care, referrals for additional services, and careful follow-up.
In 2008 8,154 patients received services, including prenatal
care, chronic disease care and management, health education,
family planning, behavioral health counselling, dental care,
and referrals for social services and specialty care.
Lucile Packard Children’s Hospital accepts all of our special
needs pediatric cases and San Mateo Medical Center cares
for adults whose treatment is beyond our scope.
We can’t do it all alone. We draw on our community partnerships
to help clients integrate healthy choices in their lives. For
example, we work with Nuestra Casa and Taulama for Tongans,
two community organizations serving immigrant groups.
With our partners, we develop healthy activities that are
both culturally appropriate and workable within the realworld
constraints our patients face.
Put yourself at the center of a circle of caring. Support RFHC and
our partners in keeping our community healthy and thriving.
