As the Chair of the Board it is my privilege to welcome<br />

you to browse and read through our first annual report.<br />

What a steady pace of growth it’s been since we<br />

opened over seven years ago. From the core group<br />

of our founding partners in 2001 — County of San<br />

Mateo, Lucile Packard Children’s Hospital, Peninsula<br />

Community Foundation*, David & Lucile Packard<br />

Foundation and the City of East Palo Alto — our circle<br />

of support has widened with each year. Now, 38% of our<br />

funding comes from the private sector of foundations,<br />

donor-advised funds and individuals.<br />

Board of Directors 2009<br />

Our Board of Directors reflects the diversity of our patient population.<br />

Melieni Talakai, Chair Staff Nurse, San Mateo County Mobile Clinic<br />

Melieni has served on our Board from the beginning and has made major<br />

contributions in guiding the first years of the clinic’s development. She<br />

served as President of the Board from 2002 – 2004. Her background in<br />

nursing and public health has given her a breadth of understanding that<br />

underlies her commitment to community health. Her Tongan heritage has<br />

made her an invaluable resource in developing culturally appropriate methods<br />

for addressing chronic disease issues in the Tongan population. In her current<br />

role as a Clinical Case Manager for San Mateo County’s Mobile Clinic, she<br />

oversees follow-up care for homeless patients. Melieni is also the host of a<br />

bi-monthly radio program about health issues that is broadcast in Tongan.<br />

Julio Garcia, Vice-Chair Program Coordinator, One East Palo Alto<br />

Leadership Program Coordinator, Nuestra Casa<br />

Julio has been a board member since 2002. With over 20 years in community<br />

grassroots organizing, he is attuned to the needs of the community, particularly<br />

regarding immigrant issues and community education. Currently, he is the<br />

Program Director of LEARN, a One East Palo Alto initiative that emphasizes<br />

the involvement of parents in their children’s education. He is also working<br />

part-time with Nuestra Casa, a community organization with a mission to help<br />

youth and parents develop their language, communication and leadership skills.<br />

Manuel Arteaga, Treasurer Social Worker, County Child Support Services<br />

After graduating from the University of California, Santa Cruz with a BA in<br />

psychology, Manuel was an elementary school teacher in Redwood City. In 1998<br />

he moved to Barcelona where he taught English and worked with Centre Integral<br />

de Serveis, coordinating group therapy and managing a caseload of patients in<br />

a mental health clinic. He now works with San Mateo County Child Support<br />

Services as a social worker. He has well developed cross-cultural skills and is an<br />

adept and thoughtful communicator.<br />

Gordon Russell, Secretary Former General Partner, Sequoia Capital<br />

Gordon was a General Partner at Sequoia Capital, a venture capital firm, for<br />

over 20 years. He specialized in high tech innovation. He also held management<br />

positions in the biomedical and healthcare industries and has been a director<br />

of private and public companies. He has extensive experience in nonprofit<br />

governance and board leadership. A trustee of the Palo Alto Medical Foundation<br />

from 1986 through 2000, he served as Chairman from 1990 to 1995. From 1992<br />

through 2002, he was on the Board of Peninsula Community Foundation and<br />

was the Chairman from 1998 to 2001. A Dartmouth College graduate, he was<br />

Chairman of the Board of Overseers at Dartmouth Medical School. He is also<br />

a director at Carnegie Foundation for the Advancement of Teaching and Woods<br />

Hole Research <strong>Center</strong>, and is the founder of the Sun Valley Writers Conference.<br />

Senseria Conley Community advocate and leader<br />

As a patient of the clinic, Senseria represents the patient’s perspective. She<br />

was invited to serve on the Board because she is forthright and effective in<br />

communicating the needs of her community. She knows the issues and challenges<br />

of parenting, and her compassion helps keep the clinic in balance.<br />

Marcelline Combs Formerly with Sequoia School District<br />

A resident of East Palo Alto since 1955, Marcelline raised her family in this<br />

community and knows firsthand how important it is to provide access to health<br />

care. She is very aware of the importance of providing culturally sensitive<br />

services. She attended Foothill and Canada Colleges and worked at the Sequoia<br />

Union High School District for many years before retiring.<br />

Charlesetta Fishman Community advocate and leader<br />

Charlesetta has served on the Board since the clinic opened and has contributed<br />

hundreds of volunteer hours on the Board, and for five years has played a lead<br />

role on the Development Committee. She has engaged and cultivated the support<br />

of many through her network of friends.<br />

Jonathan Lindeke Attorney<br />

Jonathan Lindeke is a retired Partner at Foley & Lardner LLP where he was a member<br />

of the firm’s health care industry team and its antitrust practice group. He has over<br />

32 years of experience in the practice of law. For 22, Mr. Lindeke specialized in the<br />

health care industry, providing advice and counsel to clients including hospitals<br />

and hospital systems, physicians and medical groups, academic medical centers,<br />

long-term care providers, HMOs, and public entities. He has extensive experience<br />

with managed health care, including capitated and “global risk” payment<br />

arrangements, as well as health care joint ventures, mergers and acquisitions.<br />

Joseph G. Lopez Territory Account Manager, Tableau Software<br />

Joe is a successful sales and marketing account manager. He recently<br />

joined Tableau Software after working in regional sales with SAP and<br />

Oracle Corporation. He earned a BA in Communications from California<br />

State University, Hayward where he was awarded the Ruben Mehling<br />

Scholarship for Academic Achievement.<br />

Raymond Mills Alcohol & Drug Advocate, Voices of Recovery<br />

Raymond is our newest board member. He brings valuable experience in the field<br />

of recovery. He serves as an Advocate with Voices of Recovery, an organization<br />

sponsored by San Mateo County’s Behavioral <strong>Health</strong> and Recovery Services.<br />

Rev. Elisapeta M. Savusa Pastor, Samoan Assembly of God<br />

Rev. Savusa succeeded her late husband as the pastor of the church he led<br />

until 2004. Rev. Savusa also serves as the church administrator, overseeing<br />

management of personnel and finances, as well as chairing meetings. She has<br />

over ten years experience in customer service and working with the public.<br />

Sherri Sager, Ex Officio Chief Government Relations Officer Lucile Packard<br />

Children’s Hospital<br />

Sherri is an advocate and leader in the public arena, having worked first as<br />

a Legislative Assistant to a member of Congress and later as a consultant to<br />

political candidates and elected officials. She also served as the Assistant<br />

Director of the Valley Medical <strong>Center</strong> Foundation. For the past fifteen years<br />

at LPCH, she organized and manages the government relations program and<br />

monitors proposed law and public policy that may impact the hospital. She<br />

has served on many nonprofit boards and is known for her political acumen,<br />

organizing ability, and knowledge of public policy, particularly in the area<br />

of health care.<br />

“A typical morning in the waiting room.<br />

Three powerful forces — a contracting economy, global warming,<br />

and huge health care costs — have made us all a lot more focused<br />

on prevention. My career in health care began in 1973 when I<br />

became a full time volunteer community health worker in a United<br />

Farmworker’s Union clinic. In the ensuing years, as a public<br />

health nurse and health care leader, I’ve been committed to the<br />

development of community health centers (CHCs) in California<br />

and across the country, designed to meet the needs of medically<br />

underserved and uninsured people.<br />

CHCs have now come of age and offer the most cost effective<br />

system for delivering primary and preventive care in the country.<br />

With rising unemployment, the expansion of the over 65 age group,<br />

and an alarming increase in chronic diseases, the CHC has risen<br />

from “humble” origins to a place of prominence in the nation’s<br />

health care system, averting and significantly reducing costly ER<br />

”<br />

visits and hospitalization and improving the health status of our<br />

most vulnerable populations.<br />

avenswood was born of the joint efforts of<br />

San Mateo County, Lucile Packard Children’s<br />

Hospital and community agencies and<br />

residents to ensure access to the best medical<br />

care for one of the most vibrant and ethnically<br />

diverse areas on the Peninsula.<br />

This year over 9,000 of the area’s residents will<br />

receive health care at <strong>Ravenswood</strong>: over 90% are<br />

Latinos, African Americans and Pacific Islanders;<br />

76% live at or below 100% of the Federal Poverty<br />

guidelines (annual income $22,050 for family of 4)<br />

based on <strong>2008</strong> patient data.<br />

Less than half (43%) of our patients benefit from<br />

programs such as Medi-Cal, Medicare, <strong>Health</strong>y<br />

Families or <strong>Health</strong>y Kids. Most (57%) have no<br />

insurance and pay for their care on a sliding<br />

fee scale with a minimum of $15 a visit.<br />

<strong>Health</strong> Services to put together a new community clinic.<br />

Served 9,150 patients in 32,119 visits, including medical, dental,<br />

health education and enabling services, over 19,000 patients enrolled.<br />

Initiated Mobile mammography in partnership with Community<br />

<strong>Health</strong> Partnership.<br />

Served 8,154 patients in 35,028 visits, including medical, dental,<br />

East Palo Alto residents suffer from<br />

disproportionately high rates of asthma.<br />

From 10.92% to 14% of children in our community are affected.<br />

Although asthma can be well controlled, every year children<br />

die needlessly for lack of appropriate medical management and<br />

access to care. Fortunately, Benjamin Lopez was not one of those<br />

children, because <strong>Ravenswood</strong> <strong>Family</strong> <strong>Health</strong> <strong>Center</strong> (RFHC)<br />

provided an important safety net for him and his family.<br />

Following a severe asthma attack at age 7, Benjamin remained<br />

symptom free for 6 years, until environmental factors in his home<br />

triggered another episode. His mother brought him to RFHC,<br />

where Dr. Elizabeth Baca treated Benjamin. When he required<br />

hospitalization, RFHC benefit coordinators enrolled him in<br />

insurance, because his family has no health insurance.<br />

“It was Dr Baca’s attentiveness that led to my son’s recovery,”<br />

In our definition of family we include those who<br />

have no home or are living in transitional housing as<br />

they go through community recovery programs.<br />

<strong>Family</strong> describes the way we treat our patients—with<br />

respect and genuine concern, speaking their language<br />

whenever possible, always providing culturally<br />

appropriate treatment. Our skilled providers help<br />

clients learn to care for children with asthma, or<br />

grandparents with diabetes. We teach prevention,<br />

as well as cure.<br />

And family describes our staff and volunteers—the<br />

collaborative way our providers work with one<br />

another and with our valued community partners.<br />

Become part of our family of concerned, caring citizens. A gift of $200<br />

supports <strong>Health</strong> Navigator home visits to chronic disease patients;<br />

$300 supports prenatal & infant care education for new mothers.<br />

In December, Jennifer McPike found<br />

shelter and support in a recovery program.<br />

At RFHC she’s under the care of clinician Brad Hamman,<br />

Physician Assistant, and has benefited from counseling with<br />

Denise Chun, MFT. Both, she said, made her feel they were<br />

committed to her success.<br />

Our Mobile Clinic reaches out to the<br />

homeless every Thursday afternoon.<br />

Luis hobbled in to the mobile clinic with holes cut into his shoes<br />

because his feet were swollen and painful and an infection was<br />

spreading up his legs. His feet were treated, and he was given<br />

strong antibiotics to treat the infection. Most importantly, he<br />

was treated with dignity and compassion.<br />

she is attending our 8-session prenatal and infant care<br />

Palo Alto Medical Foundation is proud to support<br />

RFHC because of its dedication to providing<br />

high-quality health care to those less fortunate<br />

in the community. We have been meeting<br />

regularly with RFHC leadership over the last four<br />

years with the goal of helping them to increase<br />

their capacity to care for the underserved and<br />

target resources for chronic disease prevention<br />

and management. A big part of this is tracking<br />

and reporting patient outcomes, and I would<br />

especially like to recognize PAMF Senior Director<br />

of Quality and Planning Tomas Moran for his<br />

dedicated efforts in this regard. In addition,<br />

PAMF has contributed financially to hire and train<br />

staff at the clinic. Through our relationship with<br />

RFHC we are able to make an impact beyond<br />

Fit Collaborative, encouraging <strong>Ravenswood</strong><br />

residents to dance, play sports, garden, or<br />

even walk. Anything to get moving and strengthen<br />

their muscles and develop healthy hearts and lungs.<br />

We encourage our clients to reconnect with cultural<br />

and family traditions that promote fitness and vitality.<br />

From the Get Fit collaborative emerged City on the<br />

Move, a video that captures the diverse and lively<br />

ways in the community that people stay healthy.<br />

To bring fresh produce to a city with no supermarket,<br />

RFHC partnered with Collective Roots Garden Project,<br />

supported by the Tides Foundation, to fund a weekly<br />

Farmer’s Market and outreach to local schools to spread<br />

the message of going green with fresh, organic produce<br />

and local gardens.<br />

<strong>Health</strong> disparities emerge when people live on the<br />

margin with poorer living conditions and greater<br />

stresses. Diabetes, hypertension, asthma and mental<br />

health issues are common. In <strong>2008</strong>, 14% of our patients<br />

were diagnosed with diabetes, 12% with hypertension,<br />

5% with asthma and 8% with depression or anxiety<br />

disorders. RFHC addresses these conditions with a<br />

comprehensive chronic disease management program<br />

and an integrated behavioral health program. Thanks<br />

to grant support from Palo Alto Medical Foundation<br />

and Kaiser Permanente, RFHC has the tools to monitor<br />

key indicators and track patient outcomes, leading to<br />

improved care through a set of best practices.<br />

Since its establishment, Lucile Packard Children’s Hospital<br />

has partnered with the <strong>Ravenswood</strong> <strong>Family</strong> <strong>Health</strong> <strong>Center</strong> to<br />

ensure that children receive quality primary care in an accessible<br />

community clinic setting. Packard Children’s provides grant<br />

support and contracts medical providers and other pediatric staff<br />

to <strong>Ravenswood</strong>. Together our organizations have established a<br />

seamless continuum of care for children and pregnant women.<br />

By attending to primary care needs, <strong>Ravenswood</strong> has contributed<br />

to a decline in emergency department visits for non-urgent care<br />

by people living in the <strong>Ravenswood</strong> service area, and has enabled<br />

pediatric care and high-risk prenatal care..<br />

positive changes. We are fortunate to be supported<br />

by community partners, who provide staff members,<br />

financial support, and services to our patients.<br />

As a community health center, we provide a seamless<br />

continuum of care that includes patient exams, preventive<br />

care, referrals for additional services, and careful follow-up.<br />

In <strong>2008</strong> 8,154 patients received services, including prenatal<br />

care, chronic disease care and management, health education,<br />

family planning, behavioral health counselling, dental care,<br />

and referrals for social services and specialty care.<br />

Lucile Packard Children’s Hospital accepts all of our special<br />

needs pediatric cases and San Mateo Medical <strong>Center</strong> cares<br />

for adults whose treatment is beyond our scope.<br />

We can’t do it all alone. We draw on our community partnerships<br />

to help clients integrate healthy choices in their lives. For<br />

example, we work with Nuestra Casa and Taulama for Tongans,<br />

two community organizations serving immigrant groups.<br />

With our partners, we develop healthy activities that are<br />

both culturally appropriate and workable within the realworld<br />

constraints our patients face.<br />

Financials<br />

Since our incorporation in 2000, the staff of <strong>Ravenswood</strong><br />

<strong>Family</strong> <strong>Health</strong> <strong>Center</strong> has made prudent use of our<br />

available resources. We work with government and private<br />

entities to secure all the assistance our clients deserve.<br />

One example is the Pharmacy Assistance Program,<br />

through which pharmaceutical companies provide free or<br />

low-cost medication to low-income patients. We help our<br />

eligible patients register for this program, and receive the<br />

medicine they need.<br />

We leverage resources whenever we can. We screen every<br />

patient to determine if they are eligible for some type of<br />

insurance, and patients are linked to social services through<br />

an extensive network of agencies. The bottom line is: we<br />

multiply the value of every dollar we receive.<br />

the lifeblood of the clinic, accounting for 62% of the<br />
