25(vii)Authorise ex-gratia lump-sum compensation simultaneously in <strong>the</strong>Pension Payment Order, where payable.SPECIAL FAMILY PENSION OR DEPENDANT PENSION WHEN DEATHOCCURRED AFTER RELEASE/ RETIREMENT/INVALIDMENT35. (a) In case of death of an Officer within 10 years afterrelease/retirement/invalidment, <strong>the</strong> next of kin shall send an intimation of death, to <strong>the</strong>Principal Controller of Defence Accounts (Pensions) and to <strong>the</strong> ArmyHeadquarters/AG(MP-5&6) in case of Service Officers, Medical Personnel ServiceRecord (Officers) in case of Army Medical Corps, Army Dental Corps and MilitaryNursing Service Officers or Territorial Army Directorate in <strong>the</strong> case of Territorial ArmyOfficers.(i) Death certificate issued by competent registering authority.(ii) A death certificate of <strong>the</strong> deceased in prescribed <strong>for</strong>m as at Annexure I toAppendix IX if he was treated by a medical practitioner. orwith <strong>the</strong> following documents-His/her statement duly verified by two reliable witnesses in <strong>the</strong>prescribed <strong>for</strong>m as at Annexure II to Appendix IX if <strong>the</strong> deceased wasnot treated by a medical practitioner. orDeath certificate (AFMSF-93 Part I) in case of death in <strong>the</strong> MilitaryHospital.(iii) Claim <strong>for</strong>m <strong>for</strong> family <strong>pension</strong> duly completed toge<strong>the</strong>r with(1) Certificate that <strong>the</strong> diseased Officer was not re-employed afterrelease/retirement/invalidment as per Annexure-I to Appendix-X.(2) Certificate from <strong>the</strong> Pension Disbursing Authority of <strong>the</strong> deceasedOfficer to <strong>the</strong> effect that arrears of <strong>pension</strong> in respect of <strong>the</strong> deceasedOfficer have been paid indicating <strong>the</strong> date upto and <strong>for</strong> which paidand <strong>the</strong> name of <strong>the</strong> recipient as per Annexure II to Appendix X to<strong>the</strong>se Regulations.(b) The Army Headquarters/AG (MP-5&6) in case of Service Officers, MedicalPersonnel Service Record (Officers) in case of Army Medical Corps, Army Dental Corpsand Military Nursing Service Officers, Territorial Army Directorate in case of TerritorialArmy Officers shall <strong>for</strong>ward within one month, service/medical documents of <strong>the</strong>deceased Officer along with <strong>the</strong> death intimation/certificate, etc. received from <strong>the</strong> nextof kin, to Army Headquarters (G S Branch) <strong>for</strong> <strong>the</strong> adjudication of death of <strong>the</strong> retiredOfficer whe<strong>the</strong>r attributable to or aggravated by military service.(c) The Principal Controller of Defence Accounts (Pensions) shall, on receiptof completed application <strong>for</strong>ms from <strong>the</strong> Next of Kin and o<strong>the</strong>r necessarydocuments, authorise ordinary family Pension, if <strong>the</strong> same was not authorised in<strong>the</strong> Pension Payment Order of <strong>the</strong> deceased Officer. In case death has beenaccepted as attributable to or aggravated by military service. Authorise specialfamily <strong>pension</strong>/dependent <strong>pension</strong> in lieu of <strong>the</strong> ordinary family <strong>pension</strong> alreadygranted.Pension Regulations <strong>for</strong> <strong>the</strong> Army, Part II (2008)
