The Spirit of ExcellenceThe results of the December <strong>2012</strong> Nurse Engagement Survey exceeded the nationalbenchmark mean, with a level of engagement of 36.1% as compared tothe national mean of 28.5%. Our commitment to exceedingour goals <strong>and</strong> individual accountability is where our compass is pointed. Ourtravels in 2013 <strong>and</strong> beyond will be guided by an abiding commitment to growpatient satisfaction. Our nursing vision sets the course.• A multidisciplinary team, including administrators, unit <strong>and</strong> departmentmanagers, staff <strong>and</strong> physicians, ascertained <strong>and</strong> prioritized opportunities forimprovement <strong>and</strong> focus. The fundamentals of AIDET <strong>and</strong> rounding, as well asthe ability to connect the dots from staff <strong>and</strong> physician level behaviors topatient perceptions, were determined to be the most immediate needs of theorganization. As a result of these findings, tools, education <strong>and</strong> training werecreated <strong>and</strong> conducted throughout the <strong>Medical</strong> Center.• Previously, inpatient HCAHPS scores fluctuated as much as 25 points ona monthly basis. The Journey ContinuesThe Primary Stroke Program received Disease-Specific recertification by TheJoint Commission in April <strong>2012</strong>. The Stroke Committee continued to monitorperformance measures safeguarding continuing excellence. In seven of tenmeasures, the hospital achieved a compliance score of 95% or better, <strong>and</strong>exceeded the statewide compliance score in six of ten core measures.Two stroke conferences in <strong>2012</strong> provided education to staff nurses <strong>and</strong> thosefrom nearby hospitals. Stroke conferences were held for patient care associatesfor the first time. Nurses in the ER, ICUs, Dean 5 <strong>and</strong> Dean 6 completed annualNIH stroke competency to ensure that their practice stays current <strong>and</strong> complieswith the highest st<strong>and</strong>ards.Opportunity at Every BendThe Outpatient areas continued to represent a wide array of services, in which apatient’s entry can be elective through a scheduled procedure (Infusion Center,Radiation Oncology or Wound Center) or emergent (Cardiac Cath Lab, Endoscopy orInterventional Radiology). For all patients, the goal remained the same: to exceedeach individual’s expectations. The growing Outpatient team, which included newmembers in many areas, ended <strong>2012</strong> excited about the challenges of the future.• The staff continued to participate in quality measures through participation inHCAHPS <strong>and</strong> core measures. Interventional Radiology was an integral part ofthe recent Stroke Center re-accreditation. Cardiac Services experienced rapidexpansion. To meet this challenge, staff collaborated with physiciansin the renovation of the Cardiac Stress <strong>and</strong> Cardiac Cath areas.• In a joint <strong>Nursing</strong> <strong>and</strong> Pharmacy research project, Infusion Centerstaff studied potential drug-to-drug interactions with homemedications in patients receiving chemo/biotherapy.• All Outpatient units have been involved in various CIS development<strong>and</strong> applications since our “Go Live” in November2011. In all council meetings, there were exchanges ofinformation between staff about the many applicationsin use. No two units use the CIS the same way. It hasbeen tremendous to see the sharing of ideas that weregenerated when diverse staffs have the opportunityto share experiences. Many of these have resultedin changes to streamline the CRV documentation.Roads Old <strong>and</strong> NewThe Emergency Department, the regional leader inemergency care, embraced opportunity to reach new heightsfor the people it serves. Nurses traveled many roads, both old <strong>and</strong>new, on the way to excellence.• The team collaborated with the EMS community <strong>and</strong> implemented protocols toimprove patient outcomes from the patient’s door through our doors. In thecare of the acute MI patient, door-to-balloon times decreased with the use ofEKGs in the field, which allows for the mobilization of the Cardiac Cath Labteam prior to the arrival of the patient. In addition, staff continued to workvery closely with the interventional cardiologists <strong>and</strong> the EMS community toensure that we continue to exceed our goals.• The Therapeutic Hypothermia Protocol was exp<strong>and</strong>ed to include the EMScommunity. This protocol is now initiated by emergency responders, whobegin cooling post-cardiac arrest patients prior to their arrival. Currentresuscitation research identifies inducing hyperthermia for a short periodof time will improve neurological function of a patient who survives acardiac arrest.• The Stroke Team was exp<strong>and</strong>ed by adding new members from the Pharmacy,Lab <strong>and</strong> Radiology to the multidisciplinary team, enhancing efficiency, quality<strong>and</strong> patient outcomes. In addition, the processes to improve patient carewere streamlined.• A Korean volunteer program, a multidisciplinary effort, was launched in the EDlobby, exp<strong>and</strong>ing our service to the Korean community.Setting the Pace for Surgical Patients• Transcatheter Aortic Valve Replacement (TAVR), in which a catheter is used toguide an artificial valve to the heart, was introduced for patients with severeaortic stenosis. Physicians <strong>and</strong> <strong>Nursing</strong> staff from the OR, Cardiac Cath Lab <strong>and</strong>CVICU participated in the education.• The hospital celebrated the opening of a hybrid operating room, a surgicaltheater equipped with imaging devices such as fixed fluoroscopy unitsthat enable a surgeon to perform minimally invasive surgery. These imagingdevices provide remarkable visualization of small blood vessels in treating aclogged blood vessel. In one application, circulation can be restored to a limbwithout an open operation.• In <strong>2012</strong>, the hospital received Disease-Specific Certification for Joint Replacement(hips <strong>and</strong> knees) <strong>and</strong> Spinal Fusion by The Joint Commission. As the patient progressed through thesurgical units, the flow sheet was completed. The patient was identified earlyin the hospital stay, <strong>and</strong> an orange campaign was established. Patients weregiven an orange ID b<strong>and</strong> <strong>and</strong> set apart by having their names highlighted inorange. The leadership team posted the appropriate antibiotic selections ineach OR suite <strong>and</strong> ensured that the necessary antibiotics were available tothe surgeons. As the patient progressed through each department, staff alsocompleted the flow sheet, ensuring that each requirement was done on time.The use of sequential compression devices, antibiotic timing <strong>and</strong> VTE prophylaxiswere included in this measurement process. In the postoperative period,the staff was reminded to discontinue antibiotics within 24 hours of surgery,administer VTE prophylaxis within 24 hours of surgery, <strong>and</strong> ensure that theFoley catheter was discontinued one to two days following surgery.
