Message from Ken Loving, MDChief Executive OfficerAs we look forward <strong>to</strong> the coming year, one of our key priorities is <strong>to</strong> help our patients navigate changes <strong>to</strong> healthinsurance coverage as a result of the Affordable Care Act. These are uncertain times in health care, and the wayWisconsin will implement the Affordable Care Act offers both opportunities and challenges.For <strong>Access</strong> Community Health Centers, there is an opportunity <strong>to</strong> help more people gain Medicaid (known asBadgerCare in Wisconsin) or private health insurance through the federal Health Insurance Exchange. The challengecomes in the uncertainty many people in our community face as their coverage shifts under the new health care law.We know that a number of patients now covered by BadgerCare will instead become eligible for federal subsidies <strong>to</strong>buy insurance in the emerging health care marketplace. Other patients who have not had health insurance coveragewill be eligible for BadgerCare.As we help our patients adapt <strong>to</strong> these changes, we see future opportunities <strong>to</strong> serve more people in need, andprovide even better care. When people have some form of health insurance coverage, it makes it much easier forthem <strong>to</strong> manage their health more effectively.In addition <strong>to</strong> changes from the Affordable Care Act, we are also making some significant and exciting changes inhow and where <strong>Access</strong> will provide care <strong>to</strong> patients in the future.This <strong>July</strong>, the Wingra Family Medical Center moved <strong>to</strong> a new facility at the corner of South Park Street and FishHatchery Road. Patients receive care there through our partnership with the University of Wisconsin Departmen<strong>to</strong>f Family Medicine. Early next year, our new Joyce & Marshall Erdman Clinic will open at the Village on Parkdevelopment on South Park Street. In this new facility, we will have a dental clinic and more space <strong>to</strong> provide ourcomprehensive services for more of our community members.Two other highlights deserve special mention:Our Celebrate Smiles program will expand significantly next year <strong>to</strong> reach 22 elementary schoolsin Madison and Sun Prairie. Our dental team uses mobile equipment <strong>to</strong> provide screening andres<strong>to</strong>rative services <strong>to</strong> thousands of children who otherwise lack access <strong>to</strong> care.Within our clinics, we are refining the care we provide by working in teams and reaching out <strong>to</strong>patients in new ways. Having attained national recognition as a Patient Centered Medical Home a<strong>to</strong>ur William T. Evjue clinic this year, we know we are on the right track <strong>to</strong> providing patients with anoptimal health care home.As we navigate the challenges and opportunities ahead, it is important <strong>to</strong> recognize thatthere remains an essential need for the services we provide at <strong>Access</strong> Community HealthCenters. <strong>Access</strong> EssentialsAs a community health center, <strong>Access</strong> is organized as a non-profit organizationwith a mission <strong>to</strong> help people who otherwise have no health care available <strong>to</strong>them. Its services are offered regardless of a patient’s ability <strong>to</strong> pay.As with all community health centers, <strong>Access</strong>’ board of direc<strong>to</strong>rs mustinclude equal numbers of community volunteers and patients, so whendecisions are being made, those who have the most at stake – patients –have an equal voice.<strong>Access</strong>’ five different clinics each provide care <strong>to</strong> residents in the nearbyneighborhoods and the broader communities where they are located. Mostimportantly, <strong>Access</strong> provides health care <strong>to</strong> people with very limited resources.Some patients have BadgerCare, some have private insurance and some haveno insurance of any kind. But all of these patients choose <strong>Access</strong> because theyknow it offers affordable, high quality care <strong>to</strong> everyone.Care at <strong>Access</strong> goes beyond curing illness – it also addresses the causesof poor health and gives people a chance <strong>to</strong> prevent illness. Throughimmunizations and patient education, low-cost medications and other services,<strong>Access</strong> enables tens of thousands of people around us <strong>to</strong> be healthier. InitiativesFACILITIESAt <strong>Access</strong>, we are benefitting from some much needed capital improvementsthat will solidify our infrastructure for years <strong>to</strong> come. Our new facilities will helpus provide our unique health care home <strong>to</strong> more people, and ensure that ourcommunity is healthier and stronger.The Wingra Family Medical Center opened in its new clinic at 1102 S. Park St.in <strong>July</strong>. This expanded medical clinic has the capacity <strong>to</strong> serve more patients inneed and builds upon our valued collaboration with the University of WisconsinDepartment of Family Medicine.Groundbreaking, Joyce & MarshallErdman Clinic, May <strong>2013</strong>In the spring, we broke ground for our Joyce & Marshall Erdman Clinic at theVillage on Park development. This facility will replace our South Side Clinic earlynext year, and includes a full-service dental clinic. When completed, <strong>Access</strong> willhave a <strong>to</strong>tal of 47 dental opera<strong>to</strong>ries in four locations. These new facilities willprovide us capacity <strong>to</strong> grow and serve more people far in<strong>to</strong> the future.Joyce & Marshall Erdman ClinicCELEBRATE SMILESAn eye-opening report last year revealed that 71.5 percent of Medicaidenrolledkids in Wisconsin did not see a dentist in 2011. <strong>Access</strong> <strong>to</strong> dental careis a challenge for families in need. Celebrate Smiles, a mobile dental programcurrently serving schools in greater Madison, is making a difference.Last year, this program run by dental staff from <strong>Access</strong> Community HealthCenters, visited 14 schools in Madison and surrounding school districts. Theprogram saw 1,500 children in grades 4K through five, providing them withsealants and fluoride varnish as well as other needed res<strong>to</strong>rative dental treatment.Mobile dental equipment, the same used by the U.S. military, equips thistraveling dental clinic. As part of the program, children become <strong>Access</strong>’ patients,so that if they have a future need, they can be seen at one of our dental clinics.Wingra Family Medical CenterWe have made progress this year, but we know the need persists. So next year,the program will serve children in 22 schools. There are plans <strong>to</strong> expand more
in the future, with the goal of having a broad impact on children by helping them improvetheir oral health. Dental pain is one of the primary reasons for school absenteeism. Evjue medical clinic received the highest level of recognitionfrom the National Committee on Quality Assurance as a Patient Centered Medical Home.This recognition is a direct result of a new approach <strong>to</strong> care that emphasizes team-based servicedelivery that reaches out <strong>to</strong> patients and engages them more in their ongoing health care.As a medical home, patient care is coordinated by provider-directed teams. In addition,patients become partners in their own care, the clinic has more open scheduling, andpatients have access <strong>to</strong> online health management <strong>to</strong>ols, like <strong>Access</strong>’ MyChart.Quality and continuity are emphasized in the Patient Centered Medical Home. Patientsare reminded about recommended preventive care, and surveyed about their experiencereceiving care at <strong>Access</strong>. They work with their care teams <strong>to</strong> set goals for improving theirhealth and fitness, and can work with our behavioral health consultants <strong>to</strong> make beneficialchanges in their daily routines.The Patient Centered Medical Home is one example of the way <strong>Access</strong> implements changethat makes a real difference for patients.<strong>Access</strong> Community Health Centers 5

Medical Care<strong>Access</strong> offers a full range of medical services including prenatal and maternalcare, pediatric care and adult and chronic care. Our goal is <strong>to</strong> become a healthcare home <strong>to</strong> each of our patients. That means providing the best care whenpatients need it, and working as a team in collaboration with patients.Prenatal and maternal services offer prenatal visits as well as integratedbehavioral health services, free childbirth and breastfeeding classes (incollaboration with Public Health Madison & Dane County) and the GroupPrenatal Education and Support Program.Pediatric care provides services <strong>to</strong> children from birth through adolescencethrough regular well-child check-ups offering a preventive examinationalongside education and information for parents. Children receiveimmunizations and regular screenings and an assessment from a BehavioralHeath Consultant. In addition, we treat acute conditions with same day visitsfor those who are ill.For adults, screenings and preventive check-ups are provided by care teamsthat partner with patients <strong>to</strong> find and treat any health issues. Dental CareDental care at <strong>Access</strong> helps patients reach the goal of achievinggood oral health that not only gives strong teeth and gums, but onewhere people are happy with their smile. We consider a wide varietyof cultural and economic fac<strong>to</strong>rs <strong>to</strong> help each patient achieve thesethings at three clinics in Madison, Sun Prairie and Dodgeville. Soon,we will also begin seeing patients at a new dental clinic at our Joyce& Marshall Erdman Clinic in south Madison.<strong>Access</strong>’ dental services take care of patients’ urgent needs forres<strong>to</strong>rative care that preserves their teeth and addresses dentalhealth issues. Behavioral Health CareBehavioral Health Care at <strong>Access</strong> is integrated with medicalcare and provides services <strong>to</strong> patients as soon as the need arises.It’s a part of what makes <strong>Access</strong> a health care home <strong>to</strong> patients.Having this unique integrated approach gives patients greateraccess <strong>to</strong> behavioral health care, better serving those who mostneed these services.This innovative approach gives patients the opportunity <strong>to</strong> meetwith a behavioral health consultant when they see their primarycare provider. Pharmacy Services<strong>Access</strong> not only offers medications that patients can afford, but ittakes extra care <strong>to</strong> provide the right information <strong>to</strong> patients so theyunderstand their medications and how <strong>to</strong> use them. This rangesfrom using show-and-tell practices where patients see medicationsbefore leaving the pharmacy, <strong>to</strong> providing language-appropriateexplanation, and verbal instruction for those who have troubleunderstanding written information.Having direct access <strong>to</strong> a pharmacy helps patients get theprescriptions they need right away.
