At PRMC, we areformalizing our approach to streng<strong>the</strong>ning our culture <strong>of</strong> safetythrough adopting CUSP (Comprehensive Unit Based SafetyProcess). CUSP was developed through Johns HopkinsUniversity & Medical Center to help reduce blood streaminfections in <strong>the</strong>ir ICUs. At our Medical Center, we haveimplemented, so far, CUSP training & methods in our ICUs and4 West patient care area with o<strong>the</strong>r units to follow. CUSPincorporates evidence–based patient safety practices thatsystematically equip staff to improve safety & quality. Throughraising awareness <strong>of</strong> safety issues, transforming current careprocesses and improving caregiver to caregivercommunications, <strong>the</strong> potential for unintended harm is greatlyreduced. Additionally our Emergency Department and Labor &Delivery Department have initiated formal team strategies &tools to enhance performance & patient Safety (TeamSTEPPS).The combination <strong>of</strong> CUSP & TeamSTEPPS provides afoundation for organization-wide improvements in patientsafety and quality outcomes.To ensure we are able to support safety in every aspect <strong>of</strong>our care delivery processes, we have initiated <strong>the</strong> following:• Team based physician & nurse rounding on patientcare units• Executive & manager rounding on patient care units• Bedside reporting• Use <strong>of</strong> helpful technologies like patient andmedication barcoding & scanning; ComputerizedPhysician Order Entry, & Robotic dispensing from<strong>the</strong> pharmacy• Compliance with: Patient identification, medicationadministration, patient hand-<strong>of</strong>f communicationsbetween shifts, infection prevention measures, fallsprevention measures, medication reconciliation,universal protocol for surgery & interventionsand actively involving patients and families in <strong>the</strong>ircareHere’s a look at how <strong>the</strong> use <strong>of</strong> technology is assisting<strong>Peninsula</strong> <strong>Regional</strong>’s healthcare team to continue improving onour Culture <strong>of</strong> Safety and create <strong>the</strong> safest environment possible.Baxa DoseEdgeDoseEdge, introduced at our Medical Center in 2008, issimilar to <strong>the</strong> bar code system <strong>Peninsula</strong> <strong>Regional</strong> uses forscanning medications used at <strong>the</strong> bedside to ensure accuracy,except this system is utilized during <strong>the</strong> preparation <strong>of</strong> sterileproducts (e.g., intravenous products) within <strong>the</strong> Richard A.Henson Cancer Institute pharmacy department. <strong>Peninsula</strong><strong>Regional</strong> implemented this system due to <strong>the</strong> volume <strong>of</strong> highrisk products prepared daily.Through <strong>the</strong> use <strong>of</strong> bar code verification, DoseEdgeensures we are using <strong>the</strong> correct products and preparationprocedure when compounding sterile products. The systemalso captures pictures during <strong>the</strong> preparation process forremote viewing by a pharmacist, and also allows for postprocedurereview, if required.Due to its success, <strong>the</strong> Medical Center plans to implementDoseEdge in our central pharmacy early this year.Baxa CompounderAno<strong>the</strong>r Baxa product, <strong>the</strong> Exacta-Mix 2400 Compounder,is a current fundraising project <strong>of</strong> <strong>the</strong> <strong>Peninsula</strong> <strong>Regional</strong>Medical Center Foundation. It’s an automated system that isused to safely mix IV solutions for pediatric and neonatalpatients. Anyone wishing to donate to that effort may contact<strong>the</strong> Foundation at 410-543-7140.HIMSS Stage 6When it comes to capturing, tracking and utilizing patientcare data through a paperless, electronic medical recordsystem, <strong>Peninsula</strong> <strong>Regional</strong> is more advanced than nearly 98%<strong>of</strong> all United States hospitals.The Medical Center has been designated a Stage 6 facilityon <strong>the</strong> Health Information and Management Systems Society(HIMSS) Analytics model for Electronic Medical Record (EMR)adoption. Stage 6 places <strong>the</strong> Medical Center among an elitegroup <strong>of</strong> just 83 American institutions, or 1.6% <strong>of</strong> all UShospitals, who have reached this level <strong>of</strong> informationtechnology sophistication.<strong>Peninsula</strong> <strong>Regional</strong> is <strong>the</strong> only hospital on <strong>the</strong> Delmarva<strong>Peninsula</strong> to achieve this status and one <strong>of</strong> just six hospitals inMaryland currently at Stage 6.CPOE Success Award<strong>Peninsula</strong><strong>Regional</strong> wasrecognized in 2010 asa top-performinginpatient organizationand awarded <strong>the</strong>McKessonCorporation’sSimona Eng, DO checks patient“Celebrating CPOEcharts using CPOE technology.Success Award.”CPOE, or Computerized Physician Order Entry, is currentlybeing used by 85% <strong>of</strong> PRMC’s physicians and o<strong>the</strong>r clinical staff,which helped <strong>Peninsula</strong> <strong>Regional</strong> to earn <strong>the</strong> national award.While most U.S. hospitals still rely on handwrittenphysician instructions for <strong>the</strong>ir patients’ care, <strong>Peninsula</strong><strong>Regional</strong>’s CPOE system provides <strong>the</strong> ability to electronicallysubmit patient orders for pharmacy, nursing, laboratory andradiology, eliminating errors related to poor handwriting. Thesystem also alerts caregivers to patient allergies or potentialadverse drug reactions. In addition, it updates patient data in asingle, secure electronic health record that is readily available toauthorized members <strong>of</strong> <strong>the</strong> care team.Most Wired-Most ImprovedIn 2010 <strong>Peninsula</strong> <strong>Regional</strong> Medical Center was alsonamed one <strong>of</strong> America’s Most Wired-Most Improved Hospitalsby <strong>the</strong> American Hospital Association’s (AHA) “Hospitals &Health Networks” magazine. <strong>Peninsula</strong> <strong>Regional</strong> was one <strong>of</strong> justthree Maryland hospitals to be honored with a Most Wireddesignation and <strong>the</strong> only designee on <strong>the</strong> Delmarva <strong>Peninsula</strong>.The Most Wired-Most Improved Award was presented tojust 25 <strong>of</strong> <strong>the</strong> 1,280 hospitals from across <strong>the</strong> United States.Those honored demonstrated a significant level <strong>of</strong> progress in<strong>the</strong>ir use <strong>of</strong> Information Technology (IT) to improve care across<strong>the</strong> continuum, clinical quality and safety, infrastructure andbusiness and administrative management. That was until October 10 <strong>of</strong> last year.“The financial burden was getting absolutely ridiculous,” said Debbie in alate October interview. “I finally found <strong>the</strong> willpower and <strong>the</strong> courage totake that first step for myself and my children, and now I want to help andencourage o<strong>the</strong>rs to quit as well.” Debbie credits <strong>Peninsula</strong> <strong>Regional</strong>’ssmoking cessation program, a ten-week class she completed, for hersuccess.That class and many o<strong>the</strong>rs are all part <strong>of</strong> <strong>Peninsula</strong> <strong>Regional</strong> MedicalCenter’s commitment to “Healthiest Maryland.” We’ve united with over 70o<strong>the</strong>r businesses in Maryland to promote healthy practices and workplacewellness for all employees. A Message From The ChairmanIt has been a privilege to serve asChairman <strong>of</strong> <strong>the</strong> Medical Center’sBoard <strong>of</strong> Trustees for <strong>the</strong> past year.We knew it would be a challengingyear, with <strong>the</strong> issues <strong>of</strong> ratereductions, healthcare reform and <strong>the</strong>impact <strong>of</strong> <strong>the</strong> economic downturn. At<strong>the</strong> same time we committed tosupporting <strong>the</strong> Medical Center in implementing <strong>the</strong> newstrategic plan and focusing on patient safety and qualitycare, as we detail in this edition <strong>of</strong> <strong>HealthFocus</strong>.In my first meeting as Chair, I commented on how <strong>the</strong>Board needs to continue to focus on patient safety andquality care. Far too many Boards focus on <strong>the</strong> financialbottom line, and while that is important and it is easy formost <strong>of</strong> us to relate to, we have a fiduciary responsibility toour community to focus on patient safety and quality <strong>of</strong> care.We have a great deal to be proud <strong>of</strong> with <strong>the</strong> continuingrecognitions that <strong>the</strong> Medical Center has received fromHealthGrades and Premier and <strong>the</strong> designation <strong>of</strong> Centers <strong>of</strong>Distinction by o<strong>the</strong>r companies like Blue Cross/Blue Shield.Two major initiatives that make PRMC one <strong>of</strong> <strong>the</strong> safestmedical centers you can experience are <strong>the</strong> eICU andComputerized Physician Order Entry (CPOE). eICU orteleICU monitoring provides an additional level <strong>of</strong> clinicalsupport for overnight monitoring <strong>of</strong> critically ill patients.This helps our nurses and physicians, but most importantly,serves our patients.The CPOE adoption rate has achieved 85% and <strong>the</strong>Medical Center received <strong>the</strong> National CPOE Success Awardfrom McKesson, ranking PRMC among <strong>the</strong> nation’s best.That’s important because CPOE is a process <strong>of</strong>electronic entry <strong>of</strong> <strong>the</strong> medical practitioner’s instructions for<strong>the</strong> treatment <strong>of</strong> patients in <strong>the</strong>ir care. The orders arecommunicated electronically to <strong>the</strong> medical staff or to <strong>the</strong>departments that are responsible for fulfilling <strong>the</strong> orders,such as, pharmacy, laboratory or radiology. CPOE decreasesdelay in order completion, reduces errors related tohandwriting or transcription, allows order entry at <strong>the</strong> point<strong>of</strong>-care,provides error-checking for duplicate or possiblyincorrect doses or tests, and simplifies inventory and posting<strong>of</strong> appropriate charges. Personally, I wouldn’t want to beadmitted to a hospital that doesn’t use CPOE or be attendedto by a physician who isn’t using this tool.I attend a lot <strong>of</strong> meetings with <strong>the</strong> medical staff andadministration. In every meeting, <strong>the</strong> predominantconversations center on patient safety and quality. There istruly a culture <strong>of</strong> patient-centered care and safety within thisorganization. There is also a true sense <strong>of</strong> teamwork. A Message From The PresidentExceptional Healthcare. Exceptional People.I’ve had <strong>the</strong> personal pleasure <strong>of</strong> working in healthcare for over 30 years atboth academic and community hospitals. I have yet to find any organizationthat could provide its patients and <strong>the</strong>ir families an exceptional healthcareexperience without exceptional people at its core.When we recently adopted <strong>the</strong> above phrase as our Medical Center’spositioning statement or “tagline,” it brought with it, quite honestly, a sense <strong>of</strong>urgency and responsibility. Urgency in <strong>the</strong> fact that -today and forever-we can’tbe anything less than exceptional with all <strong>the</strong> o<strong>the</strong>r healthcare options you haveliterally a “click” away. And responsibility? It means we must provide you <strong>the</strong>healthcare experience you expect and deserve by recruiting and retaining employees who foster aculture <strong>of</strong> stewardship and ownership by putting our patients’ needs first.In treating each patient and customer as if <strong>the</strong>y are our most beloved relative, we strive to be apositive role model for o<strong>the</strong>rs including those who work around us. We don’t support <strong>the</strong> use <strong>of</strong>terms like “it isn’t my job” or “I don’t know” as a way <strong>of</strong> passing <strong>the</strong> ball and not helping our patientsand families.I’d like to recommend an exceptional book written by Joe Tye called “The Florence Prescription:From Accountability to Ownership.” It’s a must read for any manager wanting to improve customersatisfaction, workplace productivity and employee loyalty. While <strong>the</strong> novel is based in a hospital, itsmessage resonates across all pr<strong>of</strong>essions. Tye speaks <strong>of</strong> “invisible architecture” where you build acohesive team that puts patients or customers first. It’s <strong>the</strong> soul <strong>of</strong> your organization much like <strong>the</strong>physical architecture is its body. People who are committed and engaged can and do pr<strong>of</strong>oundly andpositively affect productivity, customer service and employee morale.From that invisible architecture is built a strong commitment to teamwork, as employees acceptpersonal responsibility and accountability for <strong>the</strong>ir assignments. We know that our attitude and <strong>the</strong>way we interact with each o<strong>the</strong>r strongly impacts our patients’ perception <strong>of</strong> <strong>the</strong> Medical Center. Asmuch as we want our staff focused on <strong>the</strong> patients, we also insist that <strong>the</strong>y be kind to one ano<strong>the</strong>r,take a personal interest in one ano<strong>the</strong>r and <strong>of</strong>fer to help each o<strong>the</strong>r in whatever way <strong>the</strong>y can. Thesame applies to us, as leaders, as we too must address more than just <strong>the</strong> workplace needs <strong>of</strong> ourteam.At <strong>Peninsula</strong> <strong>Regional</strong>, we’re not perfect, but we are all striving each day to get as close to perfectas we can be. In this edition, we meet CindyLunsford, Executive Vice President/Chief Operating Officer.Cindy Lunsford, an Augusta, Georgianative, along with o<strong>the</strong>r senior executives, isresponsible for <strong>the</strong> overall day-to-dayoperation <strong>of</strong> <strong>Peninsula</strong> <strong>Regional</strong>, includingoversight <strong>of</strong> <strong>the</strong> clinical and supportfunctions <strong>of</strong> <strong>the</strong> Medical Center.All it took was one summer working ina VA hospital for Lunsford to know that acareer in hospital-based medicine wouldbecome her passion.
