Patient safety is our top priority at <strong>Columbia</strong> Memorial, and over<br />
<strong>the</strong> past three years we have made a very significant investment<br />

The hospital recently joined <strong>the</strong> NY State Partnership for<br />
Patients (NYSPFP), part <strong>of</strong> a nationwide initiative to reduce<br />
hospital-acquired conditions and preventable re-admissions.<br />

assistance, training and programming at no additional expense,<br />
enabling us to intensify our efforts.<br />
A recognized regional leader in patient safety, we recently<br />
hosted a seminar for staff from Seton Healthcare, Ellis <strong>Hospital</strong>,<br />
Albany Memorial, Samaritan <strong>Hospital</strong>, Albany Vascular Group,<br />
Empire State College Nursing Program, Questar, nursing<br />
students from <strong>Columbia</strong>-<strong>Greene</strong> Community College and <strong>the</strong><br />
hospital’s own staff.<br />
Norm Chapin, MD, <strong>Columbia</strong> Memorial’s Medical Director,<br />
in talking about safety measures <strong>the</strong> hospital has taken, said that<br />
“cross-departmental communication is an essential component<br />
<strong>of</strong> how <strong>Columbia</strong> Memorial has changed <strong>the</strong> culture and<br />
improved our safety process,” a stated goal <strong>of</strong> NYSPFP.<br />
“The model must be patient-centric, always,” he said. “Our<br />
Medical Executive Committee mandates safety training for all<br />
physicians at <strong>the</strong> hospital. Safety training is also a requirement<br />
for employment for all staff. By providing safety training to<br />
staff and physicians toge<strong>the</strong>r, we have enhanced communication<br />
and collegiality. For example, a group training session may<br />
include doctors, lab techs, housekeeping staff, security, and<br />
nurses.”<br />
Teamwork in <strong>the</strong> surgical suite, in <strong>the</strong> emergency<br />
department, and at <strong>the</strong> bedside is crucial to preventing<br />
errors. Everyone – <strong>the</strong> housekeeper, <strong>the</strong> surgeon, <strong>the</strong> lab<br />
technician, <strong>the</strong> nurse – is part <strong>of</strong> <strong>the</strong> team and has <strong>the</strong><br />
responsibility to speak up to keep you safe.<br />

a“card carrying hipster ”<br />
A grateful patient is on <strong>the</strong> road to a new life<br />
Ellyn Polay says she is a “card carrying hipster”<br />
but not in <strong>the</strong> way you normally refer to that<br />
term. When she walks through airport screening<br />
devices, she pulls out a laminated card that<br />
shows that two <strong>of</strong> her hips are titanium. If it<br />
wasn’t for <strong>the</strong> collaboration between <strong>Columbia</strong><br />
Memorial doctors Tomasz Andrejuk, Ca<strong>the</strong>rine<br />
Shin and Louis DiGiovanni, Polay would not<br />
be walking anywhere.<br />
Polay grew up in Manhattan and worked<br />
for years in Florida as a real estate pr<strong>of</strong>essional.<br />
She has a home in Spencertown, New York<br />
where she paints and illustrates children’s<br />
books.<br />
“Fate brought me up here to meet Dr.<br />
DiGiovanni and have hip surgery.”<br />
Excruciating pain<br />
About two years ago in Florida, Polay started<br />
feeling lots <strong>of</strong> pain that she thought was in her<br />
back.<br />
“It was excruciating just getting in and out<br />
<strong>of</strong> my car. It was so bad that I could only do one<br />
thing a day. I never trusted doctors, especially<br />
<strong>the</strong> ones in Florida,” said Polay. “Moving back<br />
here was <strong>the</strong> road to my new life in more ways<br />
than one,” she said.<br />
Polay, who got her first degree black belt in<br />
Tae Kwan Do at <strong>the</strong> age <strong>of</strong> 50, thought that her<br />
back problem was <strong>the</strong> result <strong>of</strong> all <strong>the</strong> kicking<br />
and athletic moves <strong>of</strong> <strong>the</strong> self defense sport.<br />
She also thought <strong>the</strong> cold up here in <strong>Columbia</strong><br />
County was a contributing cause.<br />
From Pain Management to Bone & Joint<br />
Polay turned to <strong>Columbia</strong> Memorial’s Pain<br />
Management Program to get relief. She<br />
figured that a Cortisone shot would do <strong>the</strong><br />
trick. However, when Dr. Andrejuk looked at<br />
her workups, he saw something more serious.<br />
He sent <strong>the</strong> x-rays on to Dr. Shin at <strong>the</strong> hospital’s<br />
Bone & Joint Center.<br />
“Dr. Shin came in after examining me and<br />
<strong>the</strong> x-rays and said ‘I’ve got some good news<br />
and some bad news,’” said Polay. “‘You do not<br />
have a back problem, but both <strong>of</strong> your hips are<br />
degenerating.’”<br />
“<br />
It was <strong>the</strong>n that Polay knew she had to start<br />
trusting doctors and do something about <strong>the</strong><br />
pain.<br />
Passion for his work<br />
“I was introduced to Dr. DiGiovanni and I<br />
learned that both his dad and my dad were<br />
dentists in <strong>Columbia</strong> County back in <strong>the</strong> ’60s.<br />
Dr. DiGiovanni was enthusiastic, full <strong>of</strong> life,<br />
comforting and obviously passionate about his<br />
work. That made my decision easier. He is just<br />
a nice guy, intelligent,” said Polay.<br />
Dr. DiGiovanni <strong>the</strong>n asked me, “‘Which hip<br />
do you want to do first?’And I said, both!”<br />
“I call it a miracle”<br />
“I asked Dr. DiGiovanni what I might expect<br />
after <strong>the</strong> surgery,” said Polay. “He told me I<br />
wouldn’t jump up and be walking. I would be<br />
nine days in <strong>the</strong> hospital followed by weeks <strong>of</strong><br />
physical <strong>the</strong>rapy. Dr. DiGiovanni visited me in<br />
<strong>the</strong> hospital and again during rehab.”<br />
“It took at least eight weeks before I could<br />
drive and I almost had to learn to walk again.<br />
But my life has improved enormously. I call it<br />
a miracle. It is truly amazing what <strong>the</strong>y can do<br />
to knees, hips, and shoulders to restore a person’s<br />
functionality.”<br />
“My stay at <strong>Columbia</strong> Memorial was a great<br />
experience. The nurses were so pleasant. I like<br />
<strong>the</strong> hospital’s size – you are not a number, you<br />
are a patient with needs.”<br />
Bionic woman<br />
“My daughter Samantha calls me <strong>the</strong> bionic<br />
woman. I do feel strong, thanks to Dr.<br />
DiGiovanni.”<br />
Polay was so grateful for <strong>the</strong> experience<br />
that she had a painting from famed Abstract<br />
Expressionist Gino Hollander donated to <strong>the</strong><br />
hospital. It now hangs in <strong>the</strong> lobby <strong>of</strong> <strong>the</strong><br />
Medical Office Building. �<br />

Spinning isn’t just for athletes<br />
Non-surgical help for injured tendons<br />
Things had gotten to <strong>the</strong> point with Lonny<br />
Kalfus’s left knee that he couldn’t tolerate<br />
walking a few city blocks. Kalfus, now 66, tore<br />
up his knee ligaments when he was 40, but<br />
“I didn’t have it rebuilt because that would<br />
have involved a complicated recovery period,”<br />
he said.<br />
“I work out a lot,” continued <strong>the</strong> long-time<br />
commercial photographer who concentrates<br />
<strong>the</strong>se days on fine art photography. “I would<br />
experience swelling and things like that with<br />
my knee. Then, a few years ago, I started running<br />
again. That was a mistake. The knee<br />
swelled, got painful, and stayed that way.<br />
We’d be in Hudson walking, but I couldn’t<br />
make it more than two or three city blocks.<br />
Then, an old friend from <strong>the</strong> city told me<br />
about <strong>the</strong> platelet rich plasma (PRP) injection.<br />
He said he’d had remarkable results.”<br />
Spinning for healing platelets<br />
The PRP procedure, which takes about 30 minutes<br />
in a doctor’s <strong>of</strong>fice, involves drawing a<br />
small amount <strong>of</strong> blood from <strong>the</strong> patient, spinning<br />
it in a centrifuge, drawing <strong>of</strong>f <strong>the</strong> healing<br />
platelets, and injecting those platelets into <strong>the</strong><br />
affected area.<br />
Kalfus began looking for a doctor close<br />
to his Copake home who could perform <strong>the</strong><br />
procedure.<br />
“When I called <strong>Columbia</strong> Memorial Bone &<br />
Joint Center, <strong>the</strong>y told me Dr. Pregont could<br />
do it,” he said.<br />
Lonny Kalfus was “on <strong>the</strong> verge <strong>of</strong> getting a<br />
knee replacement,” said Dr. Scott Pregont.<br />
“Here was a man who couldn’t walk two city<br />
blocks without experiencing severe pain. He<br />
was at end stage knee arthritis.”<br />
How it works<br />
When a tendon is chronically injured, at some<br />
point <strong>the</strong> body stops trying to heal it and<br />
patients live with chronic pain.<br />
According to Pregont, “PRP incites a repair<br />
response within <strong>the</strong> body. Those platelets are<br />
chemical messengers that say ‘heal here.’ The<br />
injection reorganizes <strong>the</strong> tendon fibers into<br />
normal alignment, which can permanently fix<br />
<strong>the</strong> problem.”<br />
Kalfus said that, “Over time, I noticed all<br />
<strong>the</strong> things that used to give me pain no longer<br />
did. Now I walk two to three miles every day,<br />
and briskly. I no longer have to wear a bandage<br />
support. I can play golf. It’s been great.”<br />
The cost <strong>of</strong> a PRP procedure at <strong>Columbia</strong><br />
Memorial Bone & Joint is around $750.<br />
Pregont notes that although <strong>the</strong> procedure<br />
is not covered under most medical insurance<br />
policies, he feels that in time companies will<br />
<strong>of</strong>fer coverage since <strong>the</strong> cost <strong>of</strong> a knee replacement<br />
ranges into <strong>the</strong> thousands <strong>of</strong> dollars.<br />
“Amazing” results<br />
“PRP induces <strong>the</strong> body to heal itself, and <strong>the</strong><br />
results we are seeing have been from dramatic<br />
improvement to complete recovery,” says<br />
Pregont. He describes <strong>the</strong> results he has seen<br />
thus far as “amazing.”<br />
“It’s ano<strong>the</strong>r tool in our arsenal. The results<br />
have been very satisfying. I’m very pleased with<br />
Mr. Kalfus’s outcome.” �<br />

“We haven’t even had time to take a breath,”<br />
said <strong>Columbia</strong> Memorial <strong>Hospital</strong> CEO Jane<br />
Ehrlich. “In our goal to be better, we have experienced<br />
one <strong>of</strong> <strong>the</strong> fastest growth spurts in <strong>the</strong><br />
hospital’s history, yet one <strong>of</strong> <strong>the</strong> most rewarding<br />
in terms <strong>of</strong> national and state rankings and outcomes<br />
for our patients. As we move ahead,<br />
<strong>the</strong>re will be more <strong>of</strong> <strong>the</strong> same.”<br />
It didn’t happen by accident. Ehrlich and<br />
her management team drew up a blueprint for<br />
growth five years ago that would ensure<br />
absolute safety, provide optimum care, and<br />
implement new surgical techniques.<br />
“<strong>Columbia</strong> Memorial is on <strong>the</strong> leadingedge<br />
in <strong>the</strong> healthcare environment. We<br />
have a system-wide focus on recruiting <strong>the</strong> best<br />
physicians, installing state-<strong>of</strong>-<strong>the</strong>-art equipment,<br />
and creating processes that ensure patient<br />
safety, superior surgical outcomes, familycentered<br />
births, and expert emergency care.<br />
We have committed more than $25 million<br />
dollars in <strong>the</strong> last five years ($60 million in a<br />
decade) and thousands <strong>of</strong> hours <strong>of</strong> training<br />
to achieve this position,” she said.<br />
The list <strong>of</strong> what <strong>the</strong> hospital is doing is long.<br />

<strong>Columbia</strong> Memorial’s Rapid Care Center<br />
will serve walk-in patients <strong>of</strong> all ages who have<br />
minor medical needs that don’t require a trip<br />
to <strong>the</strong> Emergency Department. Services include<br />
x-rays, treatment <strong>of</strong> broken bones and sprains,<br />
suturing <strong>of</strong> simple cuts, and diagnosis and<br />
treatment <strong>of</strong> o<strong>the</strong>r illnesses, for example. Open<br />
9 am – 7 pm, Monday through Friday, and<br />
weekends from 9am - 9pm. Located at <strong>the</strong><br />
Ocean State Job Lots Plaza on Route 9 in<br />
Valatie.<br />
Kinderhook Medical Care and Valatie<br />
Family Care have moved to new, expanded<br />
medical <strong>of</strong>fices, in <strong>the</strong> Ocean State Job Lots<br />
Plaza in Valatie.<br />
Twin County Cardiology, with <strong>of</strong>fices in<br />
Catskill and Hudson, has joined <strong>the</strong> <strong>Columbia</strong><br />
Memorial family <strong>of</strong> specialty practices as has<br />
<strong>Greene</strong> Medical Imaging in Jefferson Heights<br />
in Catskill and Valatie Medical Imaging in<br />
Valatie.<br />
The <strong>Columbia</strong> Memorial Pediatric Center is<br />
now open in bright, cheerful medical facilities<br />
at 813 Warren Street, Hudson.<br />
The hospital’s 5th and 6th floors are<br />
undergoing significant renovation. When<br />
complete, <strong>the</strong> 6th floor will be 18 new private<br />
patient rooms and hospice, and <strong>the</strong> 5th floor<br />
will house a fully renovated unit for mental<br />
health patients.<br />
Kaaterskill Manor <strong>of</strong>fers affordable housing<br />
for <strong>the</strong> elderly and options for “aging in place”<br />
close to centralized medical services in <strong>Greene</strong><br />
County. <strong>Columbia</strong> Memorial <strong>Hospital</strong> helped<br />
build <strong>the</strong> new 21 unit senior housing complex<br />
in Catskill next to Kaaterskill Care Nursing and<br />
Rehabilitation. “The Manor is part <strong>of</strong> a continuum<br />
<strong>of</strong> care campus to meet <strong>the</strong> needs <strong>of</strong> <strong>the</strong><br />
elderly at all levels,” said Todd Zbytniewski, Vice<br />
President <strong>of</strong> Operations <strong>of</strong> Kaaterskill Care, an<br />
affiliate <strong>of</strong> <strong>Columbia</strong> Memorial. “This is a way<br />
to centralize services for <strong>the</strong> elderly close to<br />
medical services such as dialysis and rehabilitation.”<br />
New initiatives:<br />
A new Spine Institute, under <strong>the</strong> guidance<br />
<strong>of</strong> spine surgeon Ersno Eromo, MD, in collaboration<br />

Making a world <strong>of</strong> difference<br />
<strong>Columbia</strong> Memorial <strong>Hospital</strong> anes<strong>the</strong>siologist<br />
travels <strong>the</strong> world to heal many wounds<br />
Carolyn LaFleur, MD is a woman on a mission<br />
and she is traveling <strong>the</strong> world to accomplish it.<br />
Dr. LaFleur is an anes<strong>the</strong>siologist at<br />
<strong>Columbia</strong> Memorial <strong>Hospital</strong>, a graduate <strong>of</strong><br />
Tufts University School <strong>of</strong> Medicine in Boston,<br />
and a member <strong>of</strong> <strong>the</strong> International Association<br />
<strong>of</strong> Anes<strong>the</strong>siologists.<br />
Most every year during short vacation breaks<br />
<strong>of</strong> ten days or less she has visited countries<br />
around <strong>the</strong> world <strong>of</strong>fering her pr<strong>of</strong>essional<br />
services to under-served populations working<br />
with a group called Cardio Start and advocating<br />
for <strong>the</strong> organization she started called Medical<br />
Diplomats International.<br />
Engage, Embrace, and Heal<br />
Her work with Medical Diplomats International<br />
serves populations that might not o<strong>the</strong>rwise<br />
have access to a high level <strong>of</strong> care.<br />
“The baseline for <strong>the</strong>se trips is to diplomatically<br />
make friends, while providing pr<strong>of</strong>essional<br />
health care services,” she said. Her organization’s<br />
motto is “Engage, Embrace, and Heal.”<br />
She has done a great deal <strong>of</strong> work in India,<br />
most recently last fall on an operation where a<br />
young woman had osteomyelitis, an acute bone<br />
infection. “The woman had already lost a foot<br />
and we had to operate to remove <strong>the</strong> o<strong>the</strong>r<br />
one as well.” LaFleur is in <strong>the</strong> process <strong>of</strong><br />
raising funds for a bilateral pros<strong>the</strong>sis for<br />
<strong>the</strong> young woman.<br />
She has been named a “leading physician<br />
<strong>of</strong> <strong>the</strong> world” for her humanitarian efforts,<br />
an honor given to only <strong>the</strong> top 1% to 2% <strong>of</strong><br />
physicians worldwide in a given specialty.<br />
Iranian mission not without glitches<br />
It was a trip to Iran in 2005 that was one <strong>of</strong><br />
<strong>the</strong> most memorable in her work to date.<br />
“We went to Iran to assist with open heart<br />
surgery. This was at <strong>the</strong> time when Secretary <strong>of</strong><br />
State Condoleeza Rice was in <strong>the</strong> Middle East<br />
and <strong>the</strong>re was talk about whe<strong>the</strong>r <strong>the</strong> U.S. was<br />
going to invade Iran. Although things were<br />
tense, <strong>the</strong> reception on <strong>the</strong> ground was warm<br />
and without political ramifications,” said<br />
LaFleur.<br />
“We flew into Tehran and <strong>the</strong>n drove south<br />
to a village between Shiraz (central Iran) and<br />
<strong>the</strong> Persian Gulf. There was easy movement<br />
because <strong>of</strong> a guide and a great bus driver.”<br />
“It was early spring and in <strong>the</strong> valley between<br />
<strong>the</strong> mountain ranges crops were coming up.<br />
Everything was clean <strong>the</strong>re.”<br />
LaFleur was involved in cardiac surgeries in<br />
<strong>the</strong> region, in particular a complex operation<br />
on an infant to repair a congenital heart defect.<br />
LaFleur also took part in a cardiac valve<br />
replacement and a bypass.<br />
The operations were not without glitches.<br />
“The equipment we had shipped was stuck in<br />
Japan. We had to borrow equipment from a<br />
village three hours away and we were limited<br />
in what we could do.”<br />
“We had an engineer with us who put in air<br />
conditioning in our operating area and who<br />
could work with generators. The blood gas<br />
machine stopped during one <strong>of</strong> <strong>the</strong> procedures,<br />
we got a second one. The outcome was<br />
fine, but it was nerve wracking.”<br />
LaFleur did a total <strong>of</strong> four cases while <strong>the</strong>re.<br />
“We unfortunately left some things undone.”<br />
Did she fear working in Iran?<br />
“Before I left to go yes, but while <strong>the</strong>re, no.<br />
We had immediate rapport and <strong>the</strong> Iranians<br />
were very giving and gracious and thankful,”<br />
said LaFleur.<br />

The tick disease expert<br />
<strong>Columbia</strong> Memorial physician Ananthakrishnan Ramani, MD,<br />
is an expert in Lyme disease and o<strong>the</strong>r tick-transmitted diseases<br />
Ticks are hungry and we’re on <strong>the</strong> menu.<br />
<strong>Columbia</strong> County is number one in <strong>the</strong> world<br />
not just for <strong>the</strong> prevalence <strong>of</strong> Lyme disease, but<br />
for all tick-borne diseases, according to Dr.<br />
Ananthakrishnan Ramani, an internist and<br />
<strong>Columbia</strong> Memorial <strong>Hospital</strong>’s infectious disease<br />
specialist.<br />
While Lyme disease remains <strong>the</strong> most well<br />
known and widespread, o<strong>the</strong>r tick-transmitted<br />
diseases are on <strong>the</strong> rise, such as, Babesiosis,<br />
Anaplasmosis and Ehrlichiosis. And all <strong>of</strong> <strong>the</strong>m<br />
have gotten a head-start this year.<br />
The new crop <strong>of</strong> tick-borne disease cases<br />
don’t usually start appearing around here until<br />
June or July, but <strong>Columbia</strong> County Public<br />
Health Director Nancy Winch told The <strong>Columbia</strong><br />
Paper that many cases are being seen<br />
now. The reason may lie under <strong>the</strong> leaves that<br />
fell from <strong>the</strong> trees last autumn.<br />
Ticks spend <strong>the</strong> winter dormant under<br />
leaves, and though many <strong>of</strong> <strong>the</strong> blood-sucking<br />
arachnids would normally succumb to winter’s<br />
frozen grip, temperatures this past winter let<br />
<strong>the</strong>m <strong>of</strong>f easy, Ms. Winch explained. Add to <strong>the</strong><br />
mix <strong>the</strong> record-breaking high temperatures <strong>of</strong><br />
March that spurred people to get outdoors for<br />
some early yard clean-up or just for a walk,<br />
making <strong>the</strong>m “interactive” with <strong>the</strong> ticks already<br />
out <strong>the</strong>re, climbing up blades <strong>of</strong> grass looking<br />
for food. As people and pets walked through<br />
<strong>the</strong> grass, ticks grabbed on for <strong>the</strong> ride, eventually<br />
finding a tasty hunk <strong>of</strong> skin to bite into.<br />
Before a tick spreads a disease to a human<br />
<strong>the</strong>re is a mammal involved, said Dr. Ramani.<br />
“The tick bites a deer or a mouse, <strong>the</strong>n <strong>the</strong> tick<br />
bites a human,” who <strong>the</strong>n contracts Lyme,<br />
Anaplasmosis or Ehrlichiosis.<br />
“Two for <strong>the</strong> price <strong>of</strong> one and sometimes<br />
Babesiosis, which makes it three for <strong>the</strong> price<br />
<strong>of</strong> one,” <strong>the</strong> doctor said.<br />
The local task force<br />
Dr. Ramani is a member <strong>of</strong> a local Lyme<br />
Disease Task Force, along with Director Winch,<br />
a representative <strong>of</strong> Cooperative Extension and a<br />
local veterinarian. The group meets every six<br />
weeks to talk about <strong>the</strong> issues, hearing different<br />
perspectives and reviewing <strong>the</strong> latest statistics.<br />
Anaplasmosis, formerly called human granulocytic<br />
Ehrlichiosis and included in <strong>the</strong><br />
Ehrlichiosis category on charts and reports<br />
until recently, is spread by deer ticks and has<br />
<strong>the</strong> same symptoms as Ehrlichiosis: fever,<br />
muscle aches, weakness, headache, nausea<br />
and possible fatality. But <strong>the</strong> Ehrlichiosis is<br />
spread by Lone Star ticks. The two illnesses are<br />
caused by different bacteria.<br />
According to a “quick guide” information<br />
table provided by Dr. Ramani, Lyme disease<br />
symptoms can include <strong>the</strong> “bull’s eye” or “target”<br />
rash, fatigue, myalgia and joint pain.<br />
Within weeks to years—arthritis, meningitis,<br />
facial palsy or heart abnormalities, which are<br />
rare, may develop.<br />
Babesiosis is a malaria-like illness with fever,<br />
chills, myalgia, fatigue and hemolytic anemia.<br />
Severe illness may occur especially in persons<br />
with a history <strong>of</strong> splenectomy or immune suppression<br />
and it can be fatal, according to <strong>the</strong><br />
guide.<br />
Tick borne diseases on <strong>the</strong> rise<br />
Latest figures on <strong>the</strong> New York State Department<br />
<strong>of</strong> Health website released in April 2011,<br />
but dated 2009, list <strong>the</strong> Lyme disease rates per<br />
100,000 populations as follows: <strong>Columbia</strong><br />
County 792.4, Rensselaer County 285.8 and<br />
Duchess County 304.<br />
Ms. Winch said in 2011 <strong>the</strong>re were 237 new<br />
cases <strong>of</strong> Lyme in <strong>Columbia</strong> County and 67 in<br />
<strong>the</strong> Ehrlichiosis category, 58 <strong>of</strong> which were<br />
anaplasmosis. Last year, 5 cases <strong>of</strong> Babesiosis<br />
were recorded here, <strong>the</strong> most ever for a single<br />
year, she said.<br />
The Anaplasmosis number for 2011 translates<br />
to a 92.5 rate in <strong>Columbia</strong> County, compared<br />
to 17.4 in Rensselaer County, 18.4 in<br />
<strong>Greene</strong> County and 20.1 in Duchess County.<br />
In o<strong>the</strong>r Capital Region counties <strong>the</strong> rates<br />
are lower, 5.0 in Albany County, .5 in Saratoga<br />
County and 0 in Schoharie and Warren<br />
counties, according to state Health Department<br />
figures.<br />
Noting <strong>the</strong> “very infectious” nature <strong>of</strong> ticks<br />
in <strong>the</strong> county, Ms. Winch said part <strong>of</strong> <strong>the</strong> reason<br />
for greater numbers <strong>of</strong> tick-borne diseases here<br />
may have something to do with <strong>the</strong> “great job”<br />
local physicians here are doing at testing and<br />
identifying <strong>the</strong> diseases.<br />
Admittedly, “<strong>the</strong> ticks are terrible in<br />
<strong>Columbia</strong> County,” said Ms. Winch, but <strong>the</strong>re’s<br />
no way to control <strong>the</strong> tick population; “you<br />
can’t spray,” she said.<br />
Whe<strong>the</strong>r this year will prove to be a recordbreaker<br />
in terms <strong>of</strong> tick-borne disease cases<br />
remains to be seen, said Ms. Winch, “but<br />
we’re aware <strong>of</strong> <strong>the</strong> situation and continue to<br />
monitor it.” �<br />

<strong>Columbia</strong> Memorial<br />
<strong>Hospital</strong> and resumed<br />
his busy life with<br />
absolutely no lingering<br />
symptoms . . . The<br />

are easy to recognize if<br />
you remember to act<br />
FAST.<br />
Time saved is brain saved<br />
<strong>Hospital</strong> is a New York State Designated Stroke Center<br />
Like most people under 40, Tom never thought<br />
he would be <strong>the</strong> victim <strong>of</strong> a massive stroke. An<br />
active and healthy man, he suddenly had difficulty<br />
speaking and could not hold up his left<br />
arm or move his left leg. When his symptoms<br />
began, Tom quickly called 9-1-1. EMTs rushed<br />
him to <strong>Columbia</strong> Memorial <strong>Hospital</strong>’s<br />
Emergency Department where he was met by<br />
<strong>the</strong> hospital’s Stroke Center Team.<br />
Every minute counts when a stroke is suspected<br />
and <strong>the</strong> Team acted immediately, assessing<br />
his condition, ordering lab tests and obtaining<br />
a CT scan.<br />
Team delivers fast, accurate results<br />
The Team includes highly trained Emergency<br />
Department physicians and nurses, and <strong>the</strong><br />
hospital’s neurologists Alexandr Safarov, MD<br />
and Samuel Koszer, MD.<br />
Supporting <strong>the</strong> Stroke Center Team are laboratory<br />
and medical imaging technicians and<br />
technologists who also played a key role in<br />
Tom’s prompt treatment. They reported <strong>the</strong><br />
results <strong>of</strong> his lab tests and CT scans within 40<br />
minutes <strong>of</strong> Tom’s arrival in <strong>the</strong> Emergency<br />
Department.<br />
Because Tom was so quickly diagnosed, <strong>the</strong><br />
Team could administer <strong>the</strong> “clot busting” drug<br />
TPA. When given within 4½ hours <strong>of</strong> <strong>the</strong> onset<br />
<strong>of</strong> stroke systems, TPA helps dissolve strokecausing<br />
blood clots - clots that deprive <strong>the</strong> brain<br />
<strong>of</strong> oxygenated blood. The longer <strong>the</strong> clot lasts,<br />
<strong>the</strong> more damage is done. But when <strong>the</strong> clot is<br />
broken up quickly with TPA <strong>the</strong> odds <strong>of</strong> survival,<br />
and even complete recovery, vastly<br />
improve.<br />
A few days after having <strong>the</strong> medical scare <strong>of</strong><br />
his life, Tom walked out <strong>of</strong> <strong>Columbia</strong> Memorial<br />
<strong>Hospital</strong> and resumed his busy life with<br />
absolutely no lingering symptoms.<br />
Stroke Center designation<br />
recognizes outstanding patient care<br />
<strong>Columbia</strong> Memorial has been accredited by<br />
New York State as a Designated Stroke Center.<br />
“This means that <strong>Columbia</strong> Memorial’s staff is<br />
making exceptional efforts to foster quality outcomes<br />
in stroke care,” said Barbara Brady,<br />
Stroke Coordinator at <strong>the</strong> hospital.<br />
According to Dr. Koszer, Medical Director <strong>of</strong><br />
<strong>the</strong> Stroke Center, “this designation is important<br />
because we are <strong>the</strong> only Stroke Center for<br />
<strong>the</strong> people <strong>of</strong> <strong>Columbia</strong> and <strong>Greene</strong> counties.<br />
This gives victims <strong>the</strong> opportunity to promptly<br />
receive TPA to reverse <strong>the</strong> effects <strong>of</strong> a stroke.”<br />
