Patient safety is our top priority at Columbia Memorial, and over
the past three years we have made a very significant investment
of time and resources to make your hospital stay as safe as
possible. Every employee and member of our medical staff
completes comprehensive patient safety training when they join
our hospital. This means you can be confident that everyone
from the dietary worker who delivers your breakfast, to the
physician who performs your surgery is working together so
that you can expect the very best, safest care.
The hospital recently joined the NY State Partnership for
Patients (NYSPFP), part of a nationwide initiative to reduce
hospital-acquired conditions and preventable re-admissions.
By joining NYSPFP we are able to receive additional technical
assistance, training and programming at no additional expense,
enabling us to intensify our efforts.
A recognized regional leader in patient safety, we recently
hosted a seminar for staff from Seton Healthcare, Ellis Hospital,
Albany Memorial, Samaritan Hospital, Albany Vascular Group,
Empire State College Nursing Program, Questar, nursing
students from Columbia-Greene Community College and the
hospital’s own staff.
Norm Chapin, MD, Columbia Memorial’s Medical Director,
in talking about safety measures the hospital has taken, said that
“cross-departmental communication is an essential component
of how Columbia Memorial has changed the culture and
improved our safety process,” a stated goal of NYSPFP.
“The model must be patient-centric, always,” he said. “Our
Medical Executive Committee mandates safety training for all
physicians at the hospital. Safety training is also a requirement
for employment for all staff. By providing safety training to
staff and physicians together, we have enhanced communication
and collegiality. For example, a group training session may
include doctors, lab techs, housekeeping staff, security, and
nurses.”
Teamwork in the surgical suite, in the emergency
department, and at the bedside is crucial to preventing
errors. Everyone – the housekeeper, the surgeon, the lab
technician, the nurse – is part of the team and has the
responsibility to speak up to keep you safe.

Hospital physicians help
a“card carrying hipster ”
A grateful patient is on the road to a new life
Ellyn Polay says she is a “card carrying hipster”
but not in the way you normally refer to that
term. When she walks through airport screening
devices, she pulls out a laminated card that
shows that two of her hips are titanium. If it
wasn’t for the collaboration between Columbia
Memorial doctors Tomasz Andrejuk, Catherine
Shin and Louis DiGiovanni, Polay would not
be walking anywhere.
Polay grew up in Manhattan and worked
for years in Florida as a real estate professional.
She has a home in Spencertown, New York
where she paints and illustrates children’s
books.
“Fate brought me up here to meet Dr.
DiGiovanni and have hip surgery.”
Excruciating pain
About two years ago in Florida, Polay started
feeling lots of pain that she thought was in her
back.
“It was excruciating just getting in and out
of my car. It was so bad that I could only do one
thing a day. I never trusted doctors, especially
the ones in Florida,” said Polay. “Moving back
here was the road to my new life in more ways
than one,” she said.
Polay, who got her first degree black belt in
Tae Kwan Do at the age of 50, thought that her
back problem was the result of all the kicking
and athletic moves of the self defense sport.
She also thought the cold up here in Columbia
County was a contributing cause.
From Pain Management to Bone & Joint
Polay turned to Columbia Memorial’s Pain
Management Program to get relief. She
figured that a Cortisone shot would do the
trick. However, when Dr. Andrejuk looked at
her workups, he saw something more serious.
He sent the x-rays on to Dr. Shin at the hospital’s
Bone & Joint Center.
“Dr. Shin came in after examining me and
the x-rays and said ‘I’ve got some good news
and some bad news,’” said Polay. “‘You do not
have a back problem, but both of your hips are
degenerating.’”
“
It was then that Polay knew she had to start
trusting doctors and do something about the
pain.
Passion for his work
“I was introduced to Dr. DiGiovanni and I
learned that both his dad and my dad were
dentists in Columbia County back in the ’60s.
Dr. DiGiovanni was enthusiastic, full of life,
comforting and obviously passionate about his
work. That made my decision easier. He is just
a nice guy, intelligent,” said Polay.
Dr. DiGiovanni then asked me, “‘Which hip
do you want to do first?’And I said, both!”
“I call it a miracle”
“I asked Dr. DiGiovanni what I might expect
after the surgery,” said Polay. “He told me I
wouldn’t jump up and be walking. I would be
nine days in the hospital followed by weeks of
physical therapy. Dr. DiGiovanni visited me in
the hospital and again during rehab.”
“It took at least eight weeks before I could
drive and I almost had to learn to walk again.
But my life has improved enormously. I call it
a miracle. It is truly amazing what they can do
to knees, hips, and shoulders to restore a person’s
functionality.”
“My stay at Columbia Memorial was a great
experience. The nurses were so pleasant. I like
the hospital’s size – you are not a number, you
are a patient with needs.”
Bionic woman
“My daughter Samantha calls me the bionic
woman. I do feel strong, thanks to Dr.
DiGiovanni.”
Polay was so grateful for the experience
that she had a painting from famed Abstract
Expressionist Gino Hollander donated to the
hospital. It now hangs in the lobby of the
Medical Office Building. �

Spinning isn’t just for athletes
Non-surgical help for injured tendons
Things had gotten to the point with Lonny
Kalfus’s left knee that he couldn’t tolerate
walking a few city blocks. Kalfus, now 66, tore
up his knee ligaments when he was 40, but
“I didn’t have it rebuilt because that would
have involved a complicated recovery period,”
he said.
“I work out a lot,” continued the long-time
commercial photographer who concentrates
these days on fine art photography. “I would
experience swelling and things like that with
my knee. Then, a few years ago, I started running
again. That was a mistake. The knee
swelled, got painful, and stayed that way.
We’d be in Hudson walking, but I couldn’t
make it more than two or three city blocks.
Then, an old friend from the city told me
about the platelet rich plasma (PRP) injection.
He said he’d had remarkable results.”
Spinning for healing platelets
The PRP procedure, which takes about 30 minutes
in a doctor’s office, involves drawing a
small amount of blood from the patient, spinning
it in a centrifuge, drawing off the healing
platelets, and injecting those platelets into the
affected area.
Kalfus began looking for a doctor close
to his Copake home who could perform the
procedure.
“When I called Columbia Memorial Bone &
Joint Center, they told me Dr. Pregont could
do it,” he said.
Lonny Kalfus was “on the verge of getting a
knee replacement,” said Dr. Scott Pregont.
“Here was a man who couldn’t walk two city
blocks without experiencing severe pain. He
was at end stage knee arthritis.”
How it works
When a tendon is chronically injured, at some
point the body stops trying to heal it and
patients live with chronic pain.
According to Pregont, “PRP incites a repair
response within the body. Those platelets are
chemical messengers that say ‘heal here.’ The
injection reorganizes the tendon fibers into
normal alignment, which can permanently fix
the problem.”
Kalfus said that, “Over time, I noticed all
the things that used to give me pain no longer
did. Now I walk two to three miles every day,
and briskly. I no longer have to wear a bandage
support. I can play golf. It’s been great.”
The cost of a PRP procedure at Columbia
Memorial Bone & Joint is around $750.
Pregont notes that although the procedure
is not covered under most medical insurance
policies, he feels that in time companies will
offer coverage since the cost of a knee replacement
ranges into the thousands of dollars.
“Amazing” results
“PRP induces the body to heal itself, and the
results we are seeing have been from dramatic
improvement to complete recovery,” says
Pregont. He describes the results he has seen
thus far as “amazing.”
“It’s another tool in our arsenal. The results
have been very satisfying. I’m very pleased with
Mr. Kalfus’s outcome.” �

“We haven’t even had time to take a breath,”
said Columbia Memorial Hospital CEO Jane
Ehrlich. “In our goal to be better, we have experienced
one of the fastest growth spurts in the
hospital’s history, yet one of the most rewarding
in terms of national and state rankings and outcomes
for our patients. As we move ahead,
there will be more of the same.”
It didn’t happen by accident. Ehrlich and
her management team drew up a blueprint for
growth five years ago that would ensure
absolute safety, provide optimum care, and
implement new surgical techniques.
“Columbia Memorial is on the leadingedge
in the healthcare environment. We
have a system-wide focus on recruiting the best
physicians, installing state-of-the-art equipment,
and creating processes that ensure patient
safety, superior surgical outcomes, familycentered
births, and expert emergency care.
We have committed more than $25 million
dollars in the last five years ($60 million in a
decade) and thousands of hours of training
to achieve this position,” she said.
The list of what the hospital is doing is long.
Here’s some of the latest great news:
Columbia Memorial’s Rapid Care Center
will serve walk-in patients of all ages who have
minor medical needs that don’t require a trip
to the Emergency Department. Services include
x-rays, treatment of broken bones and sprains,
suturing of simple cuts, and diagnosis and
treatment of other illnesses, for example. Open
9 am – 7 pm, Monday through Friday, and
weekends from 9am - 9pm. Located at the
Ocean State Job Lots Plaza on Route 9 in
Valatie.
Kinderhook Medical Care and Valatie
Family Care have moved to new, expanded
medical offices, in the Ocean State Job Lots
Plaza in Valatie.
Twin County Cardiology, with offices in
Catskill and Hudson, has joined the Columbia
Memorial family of specialty practices as has
Greene Medical Imaging in Jefferson Heights
in Catskill and Valatie Medical Imaging in
Valatie.
The Columbia Memorial Pediatric Center is
now open in bright, cheerful medical facilities
at 813 Warren Street, Hudson.
The hospital’s 5th and 6th floors are
undergoing significant renovation. When
complete, the 6th floor will be 18 new private
patient rooms and hospice, and the 5th floor
will house a fully renovated unit for mental
health patients.
Kaaterskill Manor offers affordable housing
for the elderly and options for “aging in place”
close to centralized medical services in Greene
County. Columbia Memorial Hospital helped
build the new 21 unit senior housing complex
in Catskill next to Kaaterskill Care Nursing and
Rehabilitation. “The Manor is part of a continuum
of care campus to meet the needs of the
elderly at all levels,” said Todd Zbytniewski, Vice
President of Operations of Kaaterskill Care, an
affiliate of Columbia Memorial. “This is a way
to centralize services for the elderly close to
medical services such as dialysis and rehabilitation.”
New initiatives:
A new Spine Institute, under the guidance
of spine surgeon Ersno Eromo, MD, in collaboration
with pain management physician
Ronnie Kafiluddi, MD. The Spine Institute
will be a multi-disciplinary treatment center
for back pain, including both surgical and
non-surgical options, acupuncture and
chiropractic care. �

for her humanitarian
efforts.

Making a world of difference
Columbia Memorial Hospital anesthesiologist
travels the world to heal many wounds
Carolyn LaFleur, MD is a woman on a mission
and she is traveling the world to accomplish it.
Dr. LaFleur is an anesthesiologist at
Columbia Memorial Hospital, a graduate of
Tufts University School of Medicine in Boston,
and a member of the International Association
of Anesthesiologists.
Most every year during short vacation breaks
of ten days or less she has visited countries
around the world offering her professional
services to under-served populations working
with a group called Cardio Start and advocating
for the organization she started called Medical
Diplomats International.
Engage, Embrace, and Heal
Her work with Medical Diplomats International
serves populations that might not otherwise
have access to a high level of care.
“The baseline for these trips is to diplomatically
make friends, while providing professional
health care services,” she said. Her organization’s
motto is “Engage, Embrace, and Heal.”
She has done a great deal of work in India,
most recently last fall on an operation where a
young woman had osteomyelitis, an acute bone
infection. “The woman had already lost a foot
and we had to operate to remove the other
one as well.” LaFleur is in the process of
raising funds for a bilateral prosthesis for
the young woman.
She has been named a “leading physician
of the world” for her humanitarian efforts,
an honor given to only the top 1% to 2% of
physicians worldwide in a given specialty.
Iranian mission not without glitches
It was a trip to Iran in 2005 that was one of
the most memorable in her work to date.
“We went to Iran to assist with open heart
surgery. This was at the time when Secretary of
State Condoleeza Rice was in the Middle East
and there was talk about whether the U.S. was
going to invade Iran. Although things were
tense, the reception on the ground was warm
and without political ramifications,” said
LaFleur.
“We flew into Tehran and then drove south
to a village between Shiraz (central Iran) and
the Persian Gulf. There was easy movement
because of a guide and a great bus driver.”
“It was early spring and in the valley between
the mountain ranges crops were coming up.
Everything was clean there.”
LaFleur was involved in cardiac surgeries in
the region, in particular a complex operation
on an infant to repair a congenital heart defect.
LaFleur also took part in a cardiac valve
replacement and a bypass.
The operations were not without glitches.
“The equipment we had shipped was stuck in
Japan. We had to borrow equipment from a
village three hours away and we were limited
in what we could do.”
“We had an engineer with us who put in air
conditioning in our operating area and who
could work with generators. The blood gas
machine stopped during one of the procedures,
we got a second one. The outcome was
fine, but it was nerve wracking.”
LaFleur did a total of four cases while there.
“We unfortunately left some things undone.”
Did she fear working in Iran?
“Before I left to go yes, but while there, no.
We had immediate rapport and the Iranians
were very giving and gracious and thankful,”
said LaFleur.
And that’s the simplicity of her mission with
Medical Diplomats International: to provide
top medical care, and create clear communication
between cultures that leads to a lessening
of tensions and a true and fruitful cultural
exchange. �

The tick disease expert
Columbia Memorial physician Ananthakrishnan Ramani, MD,
is an expert in Lyme disease and other tick-transmitted diseases
Ticks are hungry and we’re on the menu.
Columbia County is number one in the world
not just for the prevalence of Lyme disease, but
for all tick-borne diseases, according to Dr.
Ananthakrishnan Ramani, an internist and
Columbia Memorial Hospital’s infectious disease
specialist.
While Lyme disease remains the most well
known and widespread, other tick-transmitted
diseases are on the rise, such as, Babesiosis,
Anaplasmosis and Ehrlichiosis. And all of them
have gotten a head-start this year.
The new crop of tick-borne disease cases
don’t usually start appearing around here until
June or July, but Columbia County Public
Health Director Nancy Winch told The Columbia
Paper that many cases are being seen
now. The reason may lie under the leaves that
fell from the trees last autumn.
Ticks spend the winter dormant under
leaves, and though many of the blood-sucking
arachnids would normally succumb to winter’s
frozen grip, temperatures this past winter let
them off easy, Ms. Winch explained. Add to the
mix the record-breaking high temperatures of
March that spurred people to get outdoors for
some early yard clean-up or just for a walk,
making them “interactive” with the ticks already
out there, climbing up blades of grass looking
for food. As people and pets walked through
the grass, ticks grabbed on for the ride, eventually
finding a tasty hunk of skin to bite into.
Before a tick spreads a disease to a human
there is a mammal involved, said Dr. Ramani.
“The tick bites a deer or a mouse, then the tick
bites a human,” who then contracts Lyme,
Anaplasmosis or Ehrlichiosis.
“Two for the price of one and sometimes
Babesiosis, which makes it three for the price
of one,” the doctor said.
The local task force
Dr. Ramani is a member of a local Lyme
Disease Task Force, along with Director Winch,
a representative of Cooperative Extension and a
local veterinarian. The group meets every six
weeks to talk about the issues, hearing different
perspectives and reviewing the latest statistics.
Anaplasmosis, formerly called human granulocytic
Ehrlichiosis and included in the
Ehrlichiosis category on charts and reports
until recently, is spread by deer ticks and has
the same symptoms as Ehrlichiosis: fever,
muscle aches, weakness, headache, nausea
and possible fatality. But the Ehrlichiosis is
spread by Lone Star ticks. The two illnesses are
caused by different bacteria.
According to a “quick guide” information
table provided by Dr. Ramani, Lyme disease
symptoms can include the “bull’s eye” or “target”
rash, fatigue, myalgia and joint pain.
Within weeks to years—arthritis, meningitis,
facial palsy or heart abnormalities, which are
rare, may develop.
Babesiosis is a malaria-like illness with fever,
chills, myalgia, fatigue and hemolytic anemia.
Severe illness may occur especially in persons
with a history of splenectomy or immune suppression
and it can be fatal, according to the
guide.
Tick borne diseases on the rise
Latest figures on the New York State Department
of Health website released in April 2011,
but dated 2009, list the Lyme disease rates per
100,000 populations as follows: Columbia
County 792.4, Rensselaer County 285.8 and
Duchess County 304.
Ms. Winch said in 2011 there were 237 new
cases of Lyme in Columbia County and 67 in
the Ehrlichiosis category, 58 of which were
anaplasmosis. Last year, 5 cases of Babesiosis
were recorded here, the most ever for a single
year, she said.
The Anaplasmosis number for 2011 translates
to a 92.5 rate in Columbia County, compared
to 17.4 in Rensselaer County, 18.4 in
Greene County and 20.1 in Duchess County.
In other Capital Region counties the rates
are lower, 5.0 in Albany County, .5 in Saratoga
County and 0 in Schoharie and Warren
counties, according to state Health Department
figures.
Noting the “very infectious” nature of ticks
in the county, Ms. Winch said part of the reason
for greater numbers of tick-borne diseases here
may have something to do with the “great job”
local physicians here are doing at testing and
identifying the diseases.
Admittedly, “the ticks are terrible in
Columbia County,” said Ms. Winch, but there’s
no way to control the tick population; “you
can’t spray,” she said.
Whether this year will prove to be a recordbreaker
in terms of tick-borne disease cases
remains to be seen, said Ms. Winch, “but
we’re aware of the situation and continue to
monitor it.” �

Tom walked out of
Columbia Memorial
Hospital and resumed
his busy life with
absolutely no lingering
symptoms . . . The
symptoms of a stroke
are easy to recognize if
you remember to act
FAST.
Time saved is brain saved
Hospital is a New York State Designated Stroke Center
Like most people under 40, Tom never thought
he would be the victim of a massive stroke. An
active and healthy man, he suddenly had difficulty
speaking and could not hold up his left
arm or move his left leg. When his symptoms
began, Tom quickly called 9-1-1. EMTs rushed
him to Columbia Memorial Hospital’s
Emergency Department where he was met by
the hospital’s Stroke Center Team.
Every minute counts when a stroke is suspected
and the Team acted immediately, assessing
his condition, ordering lab tests and obtaining
a CT scan.
Team delivers fast, accurate results
The Team includes highly trained Emergency
Department physicians and nurses, and the
hospital’s neurologists Alexandr Safarov, MD
and Samuel Koszer, MD.
Supporting the Stroke Center Team are laboratory
and medical imaging technicians and
technologists who also played a key role in
Tom’s prompt treatment. They reported the
results of his lab tests and CT scans within 40
minutes of Tom’s arrival in the Emergency
Department.
Because Tom was so quickly diagnosed, the
Team could administer the “clot busting” drug
TPA. When given within 4½ hours of the onset
of stroke systems, TPA helps dissolve strokecausing
blood clots - clots that deprive the brain
of oxygenated blood. The longer the clot lasts,
the more damage is done. But when the clot is
broken up quickly with TPA the odds of survival,
and even complete recovery, vastly
improve.
A few days after having the medical scare of
his life, Tom walked out of Columbia Memorial
Hospital and resumed his busy life with
absolutely no lingering symptoms.
Stroke Center designation
recognizes outstanding patient care
Columbia Memorial has been accredited by
New York State as a Designated Stroke Center.
“This means that Columbia Memorial’s staff is
making exceptional efforts to foster quality outcomes
in stroke care,” said Barbara Brady,
Stroke Coordinator at the hospital.
According to Dr. Koszer, Medical Director of
the Stroke Center, “this designation is important
because we are the only Stroke Center for
the people of Columbia and Greene counties.
This gives victims the opportunity to promptly
receive TPA to reverse the effects of a stroke.”
“Time saved is truly brain saved,” said Brady.
When stroke is suspected
every minute counts
The symptoms of a stroke are easy to recognize
if you remember to act FAST:
F = Face
Ask the person to smile. Does one side of
the face droop?
A = Arms
Ask the person to raise both arms.
Does one arm drift downward?
S = Speech
Ask the person to repeat a simple phrase.
Does their speech sound slurred or
strange?
T = Time
If you observe any of these signs, call 9-1-1
immediately. Every minute counts.

Hot jazz for winter benefit
Winter Blast supports Hospital and Kaaterskill Care
New Orleans landed squarely
in Hudson’s Club Helsinki on
Saturday, February 4, when the
Glen David Andrews Band
straight from the Big Easy
packed the dance floor and
raised the roof at the 8th
annual winter event to benefit
Columbia Memorial Hospital
and Kaaterskill Care Nursing
& Rehab Center.
The event was hosted by
Denise de Diego and Kevin
Delahanty, with AOW
Associates, Inc., serving as presenting
sponsor. Featured
sponsors were A. Colarusso &
Son, Inc., CDPHP, and
Whiteman Osterman & Hanna
LLP. Thanks to the 250 in
attendance, approximately
$115,000 was raised for upcoming
projects for the hospital
and the Kaaterskill facility.
“This is for a great cause,”
de Diego said of the evening’s
festivities, in presenting
Richard and Deborah Oliver
of AOW Associates with a
plaque honoring them for
their contribution.
The proceeds from last
year’s Winter Blast had been
dedicated to the upgrade of
room furnishings and decor,
renovations which “turned out
beautifully,” said Kaaterskill’s
Director of Nursing Regina
Doebler.
Jane Ehrlich, president and
CEO of Columbia Memorial
Hospital, said the hospital
plans to continue to work on
improving access to care and
to “get our services out into
the county, and to try to get
our specialists more involved
in Greene County. We want to
try to grow those areas, while
keeping our primary care practices
flourishing.”

Greene Hospital Foundation
Executive Director Betsy
Gramkow for her excellent
work coordinating the event.

“Betsy also did a great job putting
together the Silent
Auction,” she said.
Among the items up for
auction were a South African
photo safari, a week at a private
estate in South Africa, a
Hudson Valley ballooning
excursion, a wine and cheese
tasting for ten at the Hudson-
Chatham Winery, and a weekend
for four at Otsego Lake.
Gramkow added that some
of the proceeds from Winter
Blast would also be going
toward the renovation and
expansion of the mental
health unit at the hospital.
“While mental health units
across the state are closing, we
are being proactive in patient
services,” she said.

hosting Winter Blast again this

year.” When asked how the
choice of the Glen David
Andrews Band came about, he
said, “Betsy wanted a party.
Well, Glen is a party. He could
raise the dead. He is a real
powerhouse.”
Over the course of the twoset
show Andrews and his fivepiece
band proved Schafler’s
statement true, with the band
time and again bringing the
crowd to its feet and dancers
spilling onto the dance floor.
Winter Blast is a “fun event.
It’s informal, with more mingling
and mixing, and Club
Helsinki is great. What began
as a very small event now generates
some serious funds,”
