Message from Ken Loving, MD<br />

Chief Executive Officer<br />

Thanks to the generosity of many in our community, we successfully raised $6.5 million for our new South Side<br />

Clinic in the last year. It’s humbling to see the level of commitment from those who share our vision of increasing<br />

care to people who need it most.<br />

Most of the funds were pledged during the quiet phase of our Improving <strong>Health</strong> Improving Lives Capital<br />

Campaign. Donations came from across the community, from individuals, businesses and foundations, including a<br />

$1.4 million gift from the family of Marshall and Joyce Erdman, as well as a Lead<br />

Gift from the BerbeeWalsh Foundation. The remainder came from a $3 million<br />

grant, part of the Affordable Care Act, awarded by the U.S. Department of<br />

<strong>Health</strong> and Human Services’ Capital Development – Building Capacity Program.<br />

We’re thankful to have the support of these donors who will make our goal to<br />

provide care to thousands more people a reality. These funds will allow us to<br />

expand medical and behavioral health care as well as add a large dental clinic<br />

and low-cost pharmacy services for thousands more people, many of whom do<br />

not have access to care.<br />

As we mark our 30th year of service, this support for our work is tremendously gratifying. At the same time, we<br />

know the need for services in our community continues to grow. Since 2009, community health centers across<br />

the nation have served an additional 3 million patients and are expected to see 1.3 million more new patients in<br />

the next two years. At <strong>Access</strong>, we’ve almost doubled the number of patients we’ve seen from 13,689 in 2009 to<br />

25,227 in 2011.<br />

We’re looking toward the future by building services that are most needed in our community. Next year, we’ll<br />

open a new Wingra Family Medical Center in conjunction with the University of Wisconsin Department of<br />

Family Medicine on South Park Street at Fish Hatchery Road. The building is currently under construction and it<br />

will also house a number of UW <strong>Health</strong> programs. We will begin building our new South Side Clinic next year<br />

and open it in 2014. And this year, we expanded our dental services at a new Dodgeville Clinic where we have<br />

eight dental operatories to care for patients.<br />

We are making progress, but we know the need for our services persists. This Fall, we will expand our oral health<br />

services to children and new mothers in Dane County through the new Celebrate Smiles Plus. This innovative<br />

initiative will expand dental care to children at their schools as well as integrate well-child exams for children and<br />

prenatal visits for women with dental services and education at <strong>Access</strong>’ clinics. This initiative has the opportunity<br />

to broadly impact a generation of children by providing them better oral health.<br />

In the midst of uncertain times, with both the future economic and health care landscapes hard to predict,<br />

it is important to recognize that there remains a critical need for the expanded services we provide at <strong>Access</strong><br />

<strong>Community</strong> <strong>Health</strong> <strong>Centers</strong>. We feel the mandate to do more, and to serve more people. Through the continued<br />

support of our community, we will do just that.<br />

Initiatives<br />

South Side Clinic<br />

More services for more patients, right where it is needed most<br />

<strong>Access</strong> raised $6.5 million for a new South Side Clinic on South Park Street in The Village on Park mall. Through<br />

generous private donations and a $3 million grant from the U.S. Department of <strong>Health</strong> and Human Services’<br />

Capital Development – Building Capacity Program, <strong>Access</strong> will build a new facility that will expand medical and<br />

behavioral health services and add a large dental clinic and pharmacy services. This facility will give us the ability to<br />

serve more people in need.<br />

The new 21,000-square-foot health center will be built next to the Urban League of Greater Madison, in front of<br />

the building where we currently have an 8,000 square foot clinic.<br />

Our new facility will help us better integrate our prenatal and pediatric care, and provide health education and<br />

other community services. We will continue to offer our patients with chronic diseases like diabetes, depression or<br />

asthma an array of services, education, support and care to help them better manage their health. We will also link<br />

prenatal visits and well-child check-ups with dental care.<br />

The new clinic will increase yearly capacity to serve thousands more<br />

people:<br />

• Medical Care: We will have more exam rooms<br />

to serve more people.<br />

• Behavioral <strong>Health</strong>: An increase in medical services means more<br />

behavioral health services using our integrated approach that<br />

addresses physical and mental health during the same clinic visit.<br />

• Dental Care: This facility will add much needed dental services<br />

in South Madison, providing care to more than 4,500 people.<br />

• Pharmacy Services: Establishing pharmacy services at this<br />

location will give patients access to low cost medications.<br />

The clinic will benefit South Madison and the community as a whole – about half (53 percent) of current South<br />

Side Clinic patients come from the immediate area surrounding the clinic.<br />

Celebrate Smiles Plus<br />

An initiative providing dental care to women and children in need<br />

Our new Celebrate Smiles Plus initiative will increase dental health services<br />

to women and children. This initiative will bring dental services to children<br />

in their schools and integrate medical and dental services for children and<br />

women at our clinics.<br />

This initiative will broadly impact a generation of women and children by<br />

providing them with healthier teeth. By 2016 this initiative will provide<br />

school-based dental services to every elementary school child in Madison<br />

and to middle school students with the greatest need. In addition we will integrate dental and medical services for<br />

mothers and children in <strong>Access</strong> clinics.<br />

During the past two school years, Celebrate Smiles, the school-based dental program, screened over 9,000<br />

students in Kindergarten through fifth grade. In year one, 20 percent of children had tooth decay. In year two,<br />

decay dropped by five percent, and children needing urgent dental treatment decreased 3.7 percent.<br />

School-based dental care<br />

The program ensures that all children in our community have healthy teeth by bringing dental services to schools.<br />

<strong>Access</strong> dental staff screens and treats children using portable dental equipment at schools to determine if students<br />

have cavities and if they would benefit from preventive dental sealants and fluoride varnish. Children with more<br />

extensive needs are referred to one of <strong>Access</strong>’ dental clinics or to a private dentist. Generous financial support<br />

from community partners allows children without dental insurance to receive care free of charge. BadgerCare pays<br />

for the majority of the students who receive services.<br />

Same-day medical and dental services<br />

Coordinating medical and dental check-ups for children age two to eleven removes the most common barriers to<br />

dental care – time off of work and school, and lack of transportation. When children schedule for a medical wellchild<br />

visit, they will also be offered a same-day, same-location dental exam. Early dental exams will enable children<br />

to get treatment for any dental decay before it becomes a significant issue. Services will begin at our William T.<br />

Evjue Clinic, and be added in spring 2014 at the new South Side Clinic.<br />

In September 2012, <strong>Access</strong> will integrate dental services to prenatal visits at our William T. Evjue Clinic. Early in<br />

pregnancy, expectant mothers will be offered a same-day, same-location dental exam. These appointments will<br />

also be offered to prenatal patients at <strong>Access</strong>’ South Side Clinic when the new clinic opens in 2014.<br />

<strong>Access</strong> Essentials<br />

What is a <strong>Community</strong> <strong>Health</strong> Center?<br />

To be called a community health center, a health care organization must meet several very important requirements: it<br />

must be organized as a non-profit, must help people who otherwise have no medical care available to them and must offer<br />

services regardless of a patient’s ability to pay. In addition, the center’s board of directors must include equal numbers of<br />

community leaders and patients, so when decisions are being made about services, budgets and policies, the people who<br />

have the most at stake – patients– have an equal voice.<br />

<strong>Access</strong> has five different locations (or centers) where patients can receive services. Each of these clinics provides care to<br />

residents in the local neighborhoods and broader regions where they are located. These clinics are also called “health<br />

centers” because they offer more than just medical services. At <strong>Access</strong>, patients can also see a dentist, behavioral health<br />

consultant, community resource specialist and financial counselor. Each of the more than 25,000 patients who call <strong>Access</strong><br />

their health care home know that they can count on services that take into account all the circumstances of their lives.<br />

Who is an <strong>Access</strong> patient?<br />

Patients represent a broad reflection of their communities. People 15,000 of all ages, ethnicities and cultures are among <strong>Access</strong><br />

patients. Most importantly, <strong>Access</strong> provides healthcare to people with very limited resources. Some patients have<br />

<strong>Access</strong> because they know it offers affordable, high quality care to everyone.<br />

Preventive care for a stronger community<br />

Our community has a firm belief that people should have<br />

access to the resources they need to thrive and be healthy.<br />

We share the value that health care should be available to<br />

people based on their need. <strong>Access</strong> <strong>Community</strong> <strong>Health</strong><br />

<strong>Centers</strong> goes beyond providing services to cure illness –<br />

it also address the causes of poor health and gives people<br />

a chance to prevent illness before it happens.<br />

Through immunizations and patient education, low-cost<br />

medications and other services, <strong>Access</strong> enables tens of<br />

thousands of people all around us to be healthier, to have<br />

healthier children and to foster a healthier community.<br />

This provides a benefit that goes far beyond cost savings<br />

when someone doesn’t have to seek treatment in the<br />

emergency room for something that could be taken care<br />

of in a doctor’s office. Having schools and workplaces with<br />

healthy people strengthens our region and helps it become<br />

more economically secure; and that’s something that is<br />

good for us all.<br />

Medical Care<br />

Maternal & Prenatal Care<br />

<strong>Access</strong> offers a comprehensive approach to prenatal<br />

care that assures babies are born healthy and families<br />

receive needed support and education before and<br />

after a child is welcomed. Prenatal services at <strong>Access</strong><br />

are provided by Board Certified Family Practice<br />

Physicians and Certified Nurse Midwives. In addition<br />

to routine prenatal visits, patients also receive our<br />

integrated behavioral health services, free childbirth<br />

and breastfeeding classes (in collaboration with Public<br />

<strong>Health</strong> Madison & Dane County) and the Group<br />

Prenatal Education and Support Program.<br />

Pediatric Care<br />

Our goal is to provide a health care home for each<br />

child from birth through childhood and adolescence,<br />

reaching out to parents to ensure that their child<br />

receives care regardless of financial status or other<br />

circumstances.<br />

Children age one to eleven receive regular well-child<br />

check-ups that offer medical providers a chance to<br />

examine children, provide education and discuss<br />

parenting strategies. Children also receive immunizations,<br />

lead screening, and vision and hearing screenings<br />

from providers including Pediatricians, Family<br />

Practice Physicians, Physician Assistants and Nurse<br />

Practitioners. During these visits, Behavioral <strong>Health</strong><br />

Consultants also assess a child’s development and<br />

mental health. We offer support for children and<br />

families through parenting classes. In addition to<br />

prevention, our care for children focuses on treating<br />

acute conditions with same day visits for those who<br />

are ill.<br />

Adult & Chronic Care<br />

<strong>Access</strong> services to adults are rooted in the belief<br />

that screenings and preventive check-ups and services<br />

are essential to staying healthy. Our health care teams<br />

partner with patients to proactively find and treat<br />

any health issues that may arise. Staff is dedicated to<br />

empowering patients with information, and our clinical<br />

care utilizes the latest evidence-based practices.<br />

For those patients who suffer from long-term, ongoing<br />

illnesses, we offer specialized care to help explore<br />

the best options for optimal health given all the<br />

circumstances of each individual. Our approach focuses<br />

on developing high-quality solutions that are realistic,<br />

affordable and manageable.<br />

Dental Care<br />

The issue of access to dental services for people with<br />

limited incomes has been highlighted locally and<br />

nationally in the past year. At <strong>Access</strong>, we continue<br />

to increase our capacity to see those with the most<br />

need for dental care at three Dental Clinics located in<br />

Madison, Sun Prairie and Dodgeville. This past year,<br />

we expanded our clinic in Dodgeville, opening our<br />

practice there in a new building with increased dental<br />

operatories to see more patients.<br />

We also served more children at their school through<br />

the Celebrate Smiles program, which over the past two<br />

years served 9,000 students, reducing the amount of<br />

decay through providing sealants and fluoride varnish.<br />

Our dental services take care of our patients’ urgent<br />

needs for restorative care that preserves their teeth<br />

and addresses dental health issues. Services also<br />

include regular preventative care for children starting<br />

at age one and for adults stressing good oral health<br />

practices that prevent dental issues from arising.<br />

Behavioral<br />

<strong>Health</strong> Care<br />

Gaining access to mental health or behavioral health<br />

care can be difficult, leading to people who are<br />

greatly underserved. Because life’s circumstances<br />

can be challenging, we offer integrated behavioral<br />

health services along with primary care visits.<br />

This innovative approach gives patients the<br />

opportunity to meet with a behavioral health<br />

consultant when they see their primary care<br />

provider. This leads to a better assessment of a<br />

patient’s health as well as a more meaningful way to<br />

treat the underlying issues.<br />

Pharmacy<br />

Services<br />

Another hallmark of community health centers<br />

like <strong>Access</strong> is that we offer medications that are<br />

affordable to patients. Having access to a pharmacy<br />

for patients who need medications helps them get the<br />

prescriptions they need right away.<br />

Also, our pharmacy staff can explain how and when to<br />

take their medications and what to do about any side<br />

effects. Because we can offer affordable medication,<br />

we can help patients achieve optimal health and better<br />

manage illness.<br />

Patient Resource<br />

Services<br />

As a Federally Qualified <strong>Health</strong> Center, <strong>Access</strong> offers<br />

much more than health services. We provide additional<br />

support services that make the best possible health<br />

outcome accessible to those in need. These resources<br />

include education, translation, health benefits counseling,<br />

transportation and other aid that promote access to<br />

health care.<br />

Through our <strong>Community</strong> Resource Specialists, on-site<br />

interpreters and other trained staff, <strong>Access</strong> helps patients<br />

find the resources that help them get to medical and<br />

dental appointments, understand providers in their own<br />

language as well as find and apply for benefits that help<br />

pay for their care.<br />
